Abstract: Walking speed (WS) is a valid, reliable, and sensitive measure appropriate for assessing and monitoring functional status and overall health in a wide range of populations. These capabilities have led to its designation as the "sixth vital sign". By synthesizing the available evidence on WS, this scholarly review article provides clinicians with a reference tool regarding this robust measure. Recommendations on testing procedures for assessing WS, including optimal distance, inclusion of acceleration and deceleration phases, instructions, and instrumentation are given. After assessing an individual's WS, clinicians need to know what this value represents. Therefore, WS cut-off values and the corresponding predicted outcomes, as well as minimal detectable change values for specific populations and settings are provided.

Abstract: Background/objectives: Sedentary behavior (SB), defined as sitting (nonexercising), reclining, and lying down (posture), or by low energy expenditure, is a public health risk independent to physical activity. The objective of this systematic literature review was to synthesize the available evidence on amount of SB reported by and measured in older adults. Data source: Studies published between 1981 and 2014 were identified from electronic databases and manual searching. Large-scale population studies/surveys reporting the amount of SB (objective/subjective) in older adults aged ≥ 60 years of age were included. Appraisal and synthesis was completed using MOOSE guidelines. Results: 349,698 adults aged ≥ 60 within 22 studies (10 countries and 1 EU-wide) were included. Objective measurement of SB shows that older adults spend an average of 9.4 hr a day sedentary, equating to 65–80% of their waking day. Abstract: To identify and characterize the scientific literature on the effects of exercise on Alzheimer's disease, research was conducted in the following databases: MEDLINE, CINAHL, Web of Science, and Scopus. These MeSH terms--"exercise", "motor activity", "physical fitness", "Alzheimer disease", and its synonyms in English--were used in the initial search to locate studies published between 2003 and 2013. After reading the 12 final articles in their entirety, two additional articles, found by a manual search, were included. Of these, 13 had beneficial results of exercise in Alzheimer's disease. Given the results discussed here, the exercise may be important for the improvement of functionality and performance of daily life activities, neuropsychiatric disturbances, cardiovascular and cardiorespiratory fitness, functional capacity components (flexibility, agility, balance, strength), and improvements in some cognitive components such as sustained attention, visual memory, and frontal cognitive function in patients with AD.

Abstract: We investigated the reliability of physical activity monitoring based on trunk accelerometry in older adults and assessed the number of measured days required to reliably assess physical activity. Seventy-nine older adults (mean age 79.1 ± 7.9) wore an accelerometer at the lower back during two nonconsecutive weeks. The duration of locomotion, lying, sitting, standing and shuffling, movement intensity, the number of locomotion bouts and transitions to standing, and the median and maximum duration of locomotion were determined per day. Using data of week 2 as reference, intraclass correlations and smallest detectable differences were calculated over an increasing number of consecutive days from week 1. Reliability was good to excellent when whole weeks were assessed. Our results indicate that a minimum of two days of observation are required to obtain an ICC ≥ 0.7 for most activities, except for lying and median duration of locomotion bouts, which required up to five days.

Abstract: Objective: To determine the effectiveness of unsupervised Nintendo Wii Fit balance training in older adults. Methods: Forty-one older adults were recruited from local retirement villages and educational settings to participate in a six-week two-group repeated measures study. The Wii group (n = 19, 75 ± 6 years) undertook 30 min of unsupervised Wii balance gaming three times per week in their retirement village while the comparison group (n = 22, 74 ± 5 years) continued with their usual exercise program. Participants’ balance abilities were assessed pre- and postintervention. Results: The Wii Fit group demonstrated significant improvements (P < .05) in timed up-and-go, left single-leg balance, lateral reach (left and right), and gait speed compared with the comparison group. Reported levels of enjoyment following game play increased during the study. Conclusion: Six weeks of unsupervised Wii balance training is an effective modality for improving balance in independent older adults.

Abstract: Lack of activity during hospitalization may contribute to functional decline. The purpose of this study was to investigate (1) the time spent walking during hospitalization by geriatric patients referred to physical and/or occupational therapy and (2) the development in time spent walking during hospitalization. In this observational study, 24-hr accelerometer data (ActivPal) were collected from inclusion to discharge in 124 patients at an acute geriatric ward. The median time spent walking was 7 min per day. During the first quartile of hospitalization, the patients spent 4 (IQR:1;11) min per day walking, increasing to 10 (IQR:1;29) min during the last quartile. Improvement in time spent walking was primarily observed in the group able to perform the Timed Up & Go task at admission. When walking only 7 min per day, patients could be classified as inactive and at risk for functional decline; nonetheless, the physical activity level increased significantly during hospitalization.

Abstract: Accelerometer step count accuracy may be affected by gait speed, device positioning, and analysis algorithm selection. We assessed ActiGraph GT3X+ step count accuracy related to device placement and analysis algorithm in older adults with walking aids (n = 13) and without walking aids (n = 22). Participants (81.5 ± 5.0 years of age) completed a timed 100-m walk wearing five GT3X+ monitors (hips, ankles, lumbar spine). Individuals with walking aids had slower gait speeds (0.8 ± 0.20 m/s versus 1.2 ± 0.20 m/s without walking aids, p < .001). Intraclass correlation coefficient values for observed versus monitored steps were highest when ankle placement and the low frequency extension (LFE) algorithm were used (left ankle ICC = .989, right ankle ICC = .998). Using the GT3X+ ankle placement and analyzing data with the LFE algorithm resulted in the most accurate step counts in older adults.

Abstract: Aim: To investigate the relationships between falls, fear of falling, and activity limitations in individuals with Parkinson’s disease (PD). Design/methods: Cross-sectional study of individuals with mild to moderate PD (N = 83). Associations among demographic data, fall frequency, disease severity, motor impairment, ability to perform activities of daily living (ADL), Activities Balance Confidence Scale, Iowa Fatigue Scale, Comorbidity Index, and Physical Activity Scale for Elders were studied. Results: Frequent fallers had more ADL limitations than nonfallers (p < .001) and rare fallers (p = .004). Frequent fallers reported a lower percentage of ability to perform ADL than nonfallers (p = .003). Frequent fallers and rare fallers were less physically active than nonfallers (p = .015 and p = .040, respectively). Frequent fallers and rare fallers reported a higher level of fear of falling than nonfallers (p = .031 and p = .009, respectively). Abstract: Mobility is critical in maintaining independence in older adults. This study aims to systematically review the scientific literature to identify measures of mobility limitation for community-dwelling older adults. A systematic search of PubMed, CINAHL, and psycINFO, using the search terms "mobility limitation", "mobility disability", and "mobility difficulty" yielded 1,847 articles from 1990 to 2012; a final selection of 103 articles was used for the present manuscript. Tools to measure mobility were found to be either self-report or performance-based instruments. Commonly measured constructs of mobility included walking, climbing stairs, and lower extremity function. There was heterogeneity in ways of defining and measuring mobility limitation in older adults living in the community. Given the lack of consistency in assessment tools for mobility, a clear understanding and standardization of instruments are required for comparison across studies and for better understanding indicators and outcomes of mobility limitation in community-dwelling older adults.

Abstract: Dancing is a complex sensorimotor activity involving physical and mental elements which have positive effects on cognitive functions and motor control. The present randomized controlled trial aims to analyze the effects of a dancing program on the performance on a motor-cognitive dual task. Data of 35 older adults, who were assigned to a dancing group or a health-related exercise group, are presented in the study. In pretest and posttest, we assessed cognitive performance and variability of minimum foot clearance, stride time, and stride length while walking. Regarding the cognitive performance and the stride-to-stride variability of minimum foot clearance, interaction effects have been found, indicating that dancing lowers gait variability to a higher extent than conventional health-related exercise. The data show that dancing improves minimum foot clearance variability and cognitive performance in a dual-task situation. Multi-task exercises (like dancing) might be a powerful tool to improve motor-cognitive dual-task performance.

Abstract: To investigate reasons for the age-related reduction in physical function, we determined the relationships between muscle size, strength, and power with 6-min walk distance (6MWD) and timed up-and-go performance in 49 young (23 ± 3.1 years) and 66 healthy, mobile older adults (72 ± 5 years). While muscle mass, determined by DXA and MRI, did not correlate with performance in the older adults, power per body mass, determined from a countermovement jump, did correlate. The 40% lower jumping power observed in older adults (p < .05) was due to a lower take-off velocity, which explained 34% and 42% of the variance in 6MWD in older women and men, respectively (p < .01). The lower velocity was partly attributable to the higher body mass to maximal force ratio, but most was due to a lower intrinsic muscle speed. While changes in muscle function explain part of the age-related reduction in functional performance, ~60% of the deficit remains to be explained.

Abstract: Background: Physical activity is important for maintaining independence and quality of life in older people living in care homes. Little is known about patterns of physical activity or sedentary behavior in this population. Methods: Thirty-three care home residents (82.6 ± 9.2 years) wore an ActiGraph GTX3 accelerometer for seven days, which provided minutes of sedentary behavior and low, light, and moderate-to-vigorous physical activity. Participants undertook the Mini-Mental State Examination and care staff reported activities of daily living (Barthel index) and functional ambulation classification (FAC) for each participant. Results: Participants spent on average 79% of their day sedentary, 14% in low, 6% in light, and 1% in moderate-to-vigorous physical activity. Abstract: We have compared the effects of different 12-week exercise programs on simple and choice reaction and movement times in persons 61 to 84 years old. One hundred thirty-eight volunteers were randomized to either a control group, a two-day exercise group (two 60-min sessions a week of aerobic exercises), or a two-day physical plus cognitive exercise group (two 60-min sessions a week of aerobic and cognitive exercises). At follow-up, the aerobic and cognitive exercise program was found to have resulted in significant positive effects. Improvements were found in the two-day physical plus cognitive exercise group in all of the reaction parameters, particularly improvement in choice reaction time, which is used in most daily activities. Our results suggest that to improve reaction time values, it is advisable to include cognitive features into a physical exercise routine.

Abstract: The purpose of this study was to compare estimates of sedentary time on weekdays vs. weekend days in older adults and determine if these patterns vary by measurement method. Older adults (N = 230, M = 83.5, SD = 6.5 years) living in retirement communities completed a questionnaire about sedentary behavior and wore an ActiGraph accelerometer for seven days. Participants engaged in 9.4 (SD = 1.5) hr per day of accelerometer-measured sedentary time, but self-reported engaging in 11.4 (SD = 4.9) hr per day. Men and older participants had more accelerometer-measured sedentary time than their counterparts. The difference between accelerometer-measured weekday and weekend sedentary time was nonsignificant. However, participants self-reported 1.1 hr per day more sedentary time on weekdays compared with weekend days. Findings suggest self-reported but not accelerometer-measured sedentary time should be investigated separately for weekdays and weekend days, and that self-reports may overestimate sedentary time in older adults.

Abstract: This study aimed to determine whether associations between the perceived environment and physical activity are moderated by urban-rural status among midolder aged adults. Environmental (safety, aesthetics, physical activity environment) and physical activity (total, leisure, transport) data from 3,888 adults (55 to 65 years) from urban and rural areas of Victoria, Australia, were analyzed. Multinomial logistic regression examined interactions between urban-rural status and environments in associations with physical activity. Significant (P < .05) interactions were evident and indicated positive associations only among older rural adults for both safety and aesthetics with total and transport physical activity (e.g., rural adults reporting higher safety were 91% to 118% more likely to have higher activity than rural adults reporting low safety). In contrast, the physical activity environment was positively associated with leisure activity among only urban adults. Findings suggest that some tailoring of physical activity promotion strategies targeting the environment may be required for urban and rural midolder aged adults.

Abstract: Regular physical activity is protective against, and beneficial for, mild cognitive impairment (MCI), dementia, and Alzheimer's disease. The mechanisms underlying these benefits remain unknown although it has been suggested that exercise-induced changes in the circadian pattern of cortisol secretion may be implicated. Fitness, salivary cortisol levels (0 and 30 min postawakening, midday, 5 p.m., and 9 p.m.), and cognitive function were determined in a group of amnestic MCI patients (n = 39) before and after a three-month exercise program (n = 19) or usual care (n = 20). At baseline, fitness measures were positively correlated with peak levels of cortisol and a greater fall in cortisol concentration from peak levels to midday. The exercise intervention successfully increased fitness and resulted in a greater fall in cortisol concentration from peak to midday, compared with the control group. The exercise intervention enhanced indices of executive function, although memory, mood, and functionality were not affected.

Abstract: Studies suggest improvements of neurocognitive function among older adults who undergo aerobic exercise training. This study sought to examine the impact of an aerobic exercise intervention on verbal fluency in sedentary older adults. Twenty community-dwelling older adults were recruited and enrolled in either a spin exercise group or a control condition. Participants were evaluated with an estimated V02max test and on measures of letter, category, and switching verbal fluency both before and after a 12-week intervention period. Spin exercise resulted in a significant improvement in category (semantic) verbal fluency when compared with the control group (15% vs. 2% increase, respectively; P = .001). Spin exercise also resulted in a significant improvement in estimated V02max (P = .005). Also important, the spin exercise group demonstrated a high level of adherence (mean adherence = 82.5%). Spin exercise can be an effective mode of aerobic exercise to improve semantic fluency in previously sedentary older adults.

Abstract: Evidence-based recommendations for interventions to reduce fall risk in older adults with visual impairment are lacking. Adapted tango dance (Tango) and a balance and mobility program (FallProof) have improved mobility, balance, and quality of life (QOL) in individuals with movement impairment. This study compared the efficacy of Tango and FallProof for 32 individuals with visual impairment (age: M = 79.3, SD =11 [51-95 years]). Participants were assigned to Tango or FallProof to complete twenty, 90-min lessons within 12 weeks. Participants underwent assessment of balance, dual-tasking, endurance, gait, and vision-related QOL. The balance reactions of participants in both groups improved (p < .001). Endurance, cognitive dual-tasking, and vision-related QOL may have improved more for Tango than FallProof. Group differences and gains were maintained across time. Both programs could be effective options for motor rehabilitation for older adults with visual impairment because they may improve mobility and QOL while reducing fall risk.

Abstract: This study examined whether the weekly volume and frequency of moderate to vigorous physical activity (MVPA) and light walking (LW) were associated with quality of life (QOL) domains of 1,806 older women from Brazil. The WHOQOL-BREF and WHOQOL-OLD instruments were used to measure QOL, while the weekly volume and frequency of MVPA and LW were assessed by IPAQ. An ordinal logistic regression was used as a measure of association. The weekly volumes of MVPA and LW were associated with several domains of QOL. Higher frequency of MVPA was associated with better scores in 10 QOL domains. The weekly frequency of LW, in turn, was associated with all QOL domains. In conclusion, promoting active transport and encouraging physical activity in older adults, for at least 150 min and distributed several days per week, help to increase QOL.

Abstract: Age-related changes in physical abilities, such as strength and flexibility, contribute to functional losses. However, older individuals may be unaware of what specific physical abilities compromise independent functioning. Three groups of women, aged 60 to 69, 70 to 79, and 80 to 92 years, were administered the Senior Fitness Test (SFT) to determine age differences in physical abilities and risk for functional losses. The oldest group showed significant differences in lower body strength, aerobic endurance, and agility and dynamic balance when compared with the other groups who performed similarly. Across all groups, a faster rate of decline was found for lower body strength (50.6%) and dynamic balance and agility (45.7%) than upper body strength (21.3%) and aerobic endurance (33.6%). Criterion-referenced (CR) fitness standards suggested that 45% of the individuals were at risk for loss of independent functioning. This study highlights age-related differences in physical abilities and the risk for the loss of independence in later life.

Abstract: Previous research has suggested that gardening activity could be an effective form of regular exercise for improving physical and psychological health in later life. However, there is a lack of data regarding the exercise intensities of various gardening tasks across different types of gardening and different populations. The purpose of this study was to examine the exercise intensity of gardening activity for older adult allotment gardeners in Wales, UK following a similar procedure used in previous studies conducted in the USA and South Korea by Park and colleagues (2008a; 2011). Oxygen consumption (VO2) and energy expenditure for six gardening tasks were measured via indirect calorimetery using the portable Oxycon™ mobile device. From these measures, estimated metabolic equivalent units (METs) were calculated. Consistent with Park et al. (2008a; 2011) the six gardening tasks were classified as low to moderate-high intensity physical activities based on their metabolic values (1.9 - 5.7 METs).

Abstract: To study mobility in older populations it can be advantageous to use portable gait analysis systems, such as inertial measurement units (IMUs), which can be used in the community. To define a normal range, 136 active subjects were recruited with an age range of 18 to 97. Four IMUs were attached to the subjects, one on each thigh and shank. Subjects were asked to walk 10 m at their own self-selected speed. The ranges of motion of thigh, shank, and knee in both swing and stance phase were calculated, in addition to stride duration. Thigh, shank, and knee range of movement in swing and stance were significantly different only in the > 80 age group. Regressions of angle against age showed a cubic relationship. Stride duration showed a weak linear relationship with age, increasing by approximately 0.1% per year.

Abstract: The purpose of this study was to explore the subjective experience of older adults interacting with both virtual and real environments. Thirty healthy older adults engaged with real and virtual tasks of similar motor demands: reaching to a target in standing and stepping stance. Immersive tendencies and absorption scales were administered before the session. Game engagement and experience questionnaires were completed after each task, followed by a semistructured interview at the end of the testing session. Data were analyzed respectively using paired t tests and grounded theory methodology. Participants preferred the virtual task over the real task. They also reported an increase in presence and absorption with the virtual task, describing an external focus of attention. Findings will be used to inform future development of appropriate game-based balance training applications that could be embedded in the home or community settings as part of evidence-based fall prevention programs.

Abstract: Walking is a suitable activity for older adults and has physical and mental health benefits. To devise interventions that impact levels of walking it is necessary to first understand the purposes for which people walk and the destinations to which they walk. Using a 7-day diary and accelerometry, this study investigated destinations and purposes of walking in older adult residents of an ultra-dense Asian city. Participants reported an average of 17.1 walking trips per week and total weekly accelerometer/diary determined trip walking time averaged 735 min per week; much higher than reported for older adults in non-Asian settings. The most common destinations were within the neighborhood: parks and streets for recreation walking and shops and eating places for transport-related walking. Errands and eating were the most common purposes for transportation trips. The study results can help inform urban design to encourage walking.

Abstract: Gait speed predicts survival in older adults; however, gait has not been evaluated in late middle-aged (LMA) populations. Purpose: Evaluate single- and dual-task gait speeds among sedentary (SED), recreationally active (RA), and masters athlete (MA) LMA adults. Methods: Participants were SED (n = 20, age = 61.0 ± 5.8), RA (n = 57, age = 63.5 ± 8.4), and MA (n = 25, age = 57.5 ± 7.9). Two trials of each task (10 m) were completed: habitual speed (HS), maximal speed (MS), dual-task (counting backward from a number by 3) habitual speed (DT-HS), and dual-task maximal speed (DT-MS). Results: MA (2.08 ± 0.63 m/s) had significantly (p < .05) greater MS compared with SED (1.94 ± 0.30 m/s) and RA (1.99 ± 0.53 m/s). Similar differences existed for DT-MS (SED = 1.77 ± 0.32 m/s, RA = 1.80 ± 0.51 m/s, MA = 1.89 ± 0.63 m/s). MA had smaller MS and DT-MS changes (difference between MS and DT-MS speeds) compared with RA (12%) and SED (13%). Abstract: This study evaluates the impact of Brain Gym (BG) training in active older adults. Eighty-five participants were assigned to four training groups: BG (n = 18), BG plus water-based exercise (n = 18), land-based exercise (n = 30), and land plus water-based exercise (n = 19). The effects of the programs on the attention and memory functions were assessed by means of the symbol digit modality test. The two-min step and the eight-foot up-and-go tests were used to evaluate their impact on fitness level. No program had a significant influence on the participant's cognitive performance, while different effects on the sample' fitness levels were observed. These findings suggest that the effects of BG on the cognitive performance and fitness level of active older adults are similar to those obtained after the practice of a traditional exercise program. Whether BG is performed in isolation or combined with other exercise programs seems to have no influence on such effects.

Abstract: Older adults traditionally adapt their discrete aiming movements, thereby traveling a larger proportion of the movement under closed-loop control. As the beneficial impact of a physically active lifestyle in older age has been described for several aspects of motor control, we compared the aiming performance of young controls to active and sedentary older adults. To additionally determine the contribution of visual feedback, aiming movements were executed with and without saccades. Results showed only sedentary older adults adopted the typical movement changes, highlighting the impact of a physically active lifestyle on manual aiming in older age. In an attempt to reveal the mechanism underlying the movement changes, evidence for an age-related decline in force control was found, which in turn resulted in an adapted aiming strategy. Finally, prohibiting saccades did not affect older adults’ performance to a greater extent, suggesting they do not rely more on visual feedback than young controls.

Abstract: This study evaluates the separate effect and retention of 12-week traditional (TE) and horse (HE) exercise programs on physical function in healthy older participants (61 to 87 years old). Thirty-eight participants were randomly assigned to three groups: TE (n = 17), HE (n = 10), and control group (n = 11). TE and HE underwent a supervised exercise program (3 day/week). Maximal gait speed, muscle strength, and body balance were assessed at weeks 0, 12, and 16. Only TE and HE displayed significant improvements (P < .05) in knee extensor strength, and only HE had faster gait speed. Marginal balance improvements were found only in HE in the medial-lateral direction. However, TE showed larger improvements in handgrip than HE. The largest retention was in knee extensor strength but most of the exercise effects were lost in the follow-up. Besides TE, exercise with a horse may be an alternative option to older adults, provided that they want to interact with the animal.

Abstract: The effects of high-intensity progressive resistance training (HIPRT) on cardiovascular function and autonomic neural regulation in older adults are unclear. To investigate this issue, 25 older adults were randomly divided into two groups: control (CON, N = 13, 63 ± 4 years; no training) and HIPRT (N = 12, 64 ± 4 years; 2 sessions/week, 7 exercises, 2–4 sets, 10–4 RM). Before and after four months, maximal strength, quadriceps cross-sectional area (QCSA), clinic and ambulatory blood pressures (BP), systemic hemodynamics, and cardiovascular autonomic modulation were measured. Maximal strength and QCSA increased in the HIPRT group and did not change in the CON group. Clinic and ambulatory BP, cardiac output, systemic vascular resistance, stroke volume, heart rate, and cardiac sympathovagal balance did not change in the HIPRT group or the CON group. In conclusion, HIPRT was effective at increasing muscle mass and strength without promoting changes in cardiovascular function or autonomic neural regulation.
