TLDR
A new method for target control infusion (TCI) for neuromuscular blockade (NMB) level control of patients subject to general anaesthesia is presented, which combines an inversion of the pharmacokinetic/pharmacodynamic (PK/PD) model with a hybrid parameter estimation method that uses on-line data from the initial bolus response to estimate the model parameters.Abstract:
The paper presents a new method for target control infusion (TCI) for neuromuscular blockade (NMB) level control of patients subject to general anaesthesia. The method combines an inversion of the pharmacokinetic/pharmacodynamic (PK/PD) model with a hybrid parameter estimation method that uses on-line data from the initial bolus response to estimate the model parameters. Although atracurium is considered as relaxant, the newly proposed method may be applied to other drugs for which the PK/PD model is available. Simulation results on a bank of 100 patient models are presented to demonstrate the achievable performance.read more

Neuromuscular blockade nonlinear model identification
TL;DR: Computer simulations show that the proposed identification methodology is able to provide good results even when the pharmacokinetic dynamics has an order higher that two, which suggests that the methodology may be employed in neuromuscular blockade automated control as a predictive model, to help the initial tuning of the controller parameters or in adaptive control to get a first model that can be improved with online identification.

Depth of anaesthesia patient models and control
Shahab Abdulla,Peng Wen +1 more
TL;DR: In this study the simulation shows that the internal model control performed better than the PID controller for anaesthesia depth, and this study uses aninternal model control structure for the adjustment of Bispectral Index (BIS).

Depth of anaesthesia control investigation using robust deadbeat control technique
Shahab Abdulla,Peng Wen +1 more
TL;DR: In this article, the authors investigated the depth of anaesthesia (DoA) control system using robust deadbeat technique and developed a robust controller, which has about 15% less overshoot, shorter settling time (about 1.5 minutes shorter) and more robust to disturbances caused by parameter changes.

Michel Struys,Tom De Smet,Linda Versichelen,Stijn Van de Velde,Rudy Van den Broecke,Eric Mortier +5 more
TL;DR: A closed-loop system for propofol administration using the BIS as a controlled variable together with a model-based controller is clinically acceptable during general anesthesia.

The development of ‘Diprifusor’: a TCI system for propofol
TL;DR: Standardisation of the delivery performance of commercial systems has been achieved with a laboratory performance specification and clinical studies indicate that the actual blood concentrations achieved were about 16% greater than the calculated values displayed by the ‘Diprifusor’ infusion system.

Manual compared with target-controlled infusion of propofol.
TL;DR: Significantly more propofol was administered during both induction and maintenance of anaesthesia with the target-controlled system, associated with significantly increased end-tidal carbon dioxide measurements during the middle period of maintenance only, but recovery from anaesthesia was not significantly prolonged in thetarget-controlled group.

TL;DR: The revised closed-loop system using the bispectral index for automatic control of propofol anaesthesia was able to provide clinically adequate anaesthesia in all patients, with better accuracy of control than in the previous study.

A Model-Predictive Hypnosis Control System Under Total Intravenous Anesthesia
TL;DR: A hypnosis control system, which administers propofol as an anesthetic drug to regulate the bispectral index (BIS), an electroencephalography-derived index reflecting the hypnosis of a patient, can reduce the total amount of prop ofol infusion and maintain the BIS more accurately than anesthesiologist's manual adjustment.
