TLDR
Precore mutant HBV can influence the response to interferon when it reaches significant serum levels (> 20% of total viremia) and chronic hepatitis B should be treated as early as possible in its natural history before precore mutant HBVs is selected as a prevalent virus.

Efficacy of lamivudine in patients with hepatitis B e antigen-negative/hepatitis B virus DNA-positive (precore mutant) chronic hepatitis B. Lamivudine Precore Mutant Study Group.

TL;DR: Lamivudine treatment results in a significant virological and biochemical improvement compared with placebo, induces an improvement or no change in histology in most patients, and is well tolerated.
Journal ArticleDOI

World-wide epidemiology of HBeAg-negative chronic hepatitis B and associated precore and core promoter variants

TL;DR: It is revealed that e‐CHB is more common than previously suspected and that it is present worldwide with marked variations in the prevalence of associated HBV variants across different geographical regions.
Journal ArticleDOI

HBV genotype B is associated with better response to interferon therapy in HBeAg(+) chronic hepatitis than genotype C

TL;DR: HBV genotype B was associated with a higher rate of IFN-induced HBeAg clearance compared with genotype C, and Stratification for HBV genotypes should be considered in future clinical trials of antiviral therapy of chronic hepatitis B.
Book ChapterDOI

Naturally occurring variants of hepatitis B virus.

TL;DR: The data from extensive research on the genetic variability of HBV done during the past decade is discussed and the implications for the life cycle of the virus, immune recognition, and pathogenesis, and currently available data on the epidemiology of mutants are discussed.

Formulation and application of a numerical scoring system for assessing histological activity in asymptomatic chronic active hepatitis

TL;DR: A Histology Activity Index has been developed which generates a numerical score for liver biopsy specimens obtained from patients with asymptomatic chronic active hepatitis that provides definitive endpoints for statistical analysis of serial changes in liver histology and offers an alternative to the use of conventional pathological descriptions.
Journal ArticleDOI

MUTATION PREVENTING FORMATION OF HEPATITIS B e ANTIGEN IN PATIENTS WITH CHRONIC HEPATITIS B INFECTION

TL;DR: To test the hypothesis that the absence of HBeAg production may be due to a change in the nucleotide sequence of the pre-core region of the genome, 18 Greek and 3 non-Greek patients positive for HB surface antigen underwent direct sequencing of HBV-DNA amplified from sera.
Journal ArticleDOI

Hepatitis B viruses with precore region defects prevail in persistently infected hosts along with seroconversion to the antibody against e antigen.

TL;DR: Results would indicate a selection of pre-C-defective mutants in persistently infected hosts, along with seroconversion to anti-HBe, by immune elimination of hepatocytes harboring nondefective HBV with the expression of HBeAg.
Journal ArticleDOI

Mutations in the Precore Region of Hepatitis B Virus DNA in Patients with Fulminant and Severe Hepatitis

TL;DR: The presence of a mutant viral strain is associated with and may be involved in the pathogenesis of fulminant hepatitis B and severe exacerbations of chronic hepatitis B.
