TLDR
The efficacy of the macrolide agent azithromycin for treating shigellosis was evaluated and the oral form of the extended-spectrum cephalosporin cefixime, although active in vitro against multidrug-resistant Shigella strains, was ineffective in achieving clinical and bacteriologic cure.Abstract:
Background: Treatment of shigellosis is currently limited by the high prevalence of multidrug-resistant strains of Shigella. Objective: To determine the efficacy of azithromycin in the treatment of...read more

TL;DR: Prevention by avoidance of undercooked meat or seafood, avoidance of unpasteurized milk or soft cheese, and selected use of available typhoid vaccines for travelers to areas where typhoid is endemic are key to the control of infectious diarrhea.

Acute Infectious Diarrhea
TL;DR: An otherwise healthy 23-year-old man presents after the acute onset of watery diarrhea that has persisted for two days, with associated nausea and cramping but no emesis and is febrile, with a temperature of 38.7°C (101°F).

The macrolides: erythromycin, clarithromycin, and azithromycin.
TL;DR: In addition to erythromycin, macrolides now available in the United States include azithromycin and clarithromycin this article, which have excellent activity against the atypical respiratory pathogens (C. pneumoniae and Mycoplasma species) and Legionella species.

TL;DR: Strong evidence supports the effectiveness of antimicrobial therapy in most cases of moderate to severe travelers' diarrhea, while either increasing intake of fluids only or loperamide or bismuth subsalicylate may suffice for most Cases of mild diarrhea.

TL;DR: The widespread distribution of fibroblasts in tissues suggests a potential for these cells, and possibly other lysosome-containing tissue cells, to serve as a reservoir for azithromycin, slowly releasing it for activity against extracellular organisms at sites of infection and passing it to phagocytes for activityagainst intracellular pathogens and potential transport to Sites of infection.
