TL;DR: It is concluded that Parkinson patients are less inclined than healthy persons to maintain a safe gait and treat all elements of a complex task with equal priority, which in daily life may go at the expense of maintaining balance and lead to falls.

TL;DR: It is concluded that the presented approach objectively quantifies the contribution of each ankle to balance maintenance and application of this method in longitudinal surveys of balance rehabilitation makes it possible to disentangle the different recovery mechanisms.

Journal ArticleDOI
TL;DR: A main underlying cause of postural instability in SCA patients appears to be "locking" of the knees, which may reflect compensation (by reducing interaction between body links) or reduced vestibulocerebellar control over leg muscles.

TL;DR: Tandem gait performance has a good diagnostic ability to differentiate patients with atypical parkinsonism from those with Parkinson’s disease, and might be used as an additional “red flag” to assist existing clinical tests in identifying atypicism.
Abstract: Background: Balance impairment is a frequently encountered problem in patients with Parkinson’s disease. A profound balance disorder, however, is an atypical feature. Methods: Tandem gait performance (10 consecutive tandem steps) was judged in 36 consecutive patients with Parkinson’s disease and 49 consecutive patients with atypical parkinsonism. Results: Only 9 (18%) patients with atypical parkinsonism had a fully normal tandem gait (not a single side step) as opposed to 33 (92%) patients with Parkinson’s disease. Analysis for the subgroup of patients with a disease duration of Conclusions: Tandem gait performance has a good diagnostic ability to differentiate patients with atypical parkinsonism from those with Parkinson’s disease, and might be used as an additional “red flag” to assist existing clinical tests in identifying atypical parkinsonism.

TL;DR: This review of studies that have investigated the possible therapeutic effects of rTMS on mood and motor function in PD patients highlights some methodological inconsistencies and problems, including the difficulty to define the most effective protocol for rT MS or to establish an appropriate placebo condition.

TL;DR: It is concluded that falling is a clinically important problem in patients with neuromuscular disorders and in age-matched controls.

TL;DR: It is suggested that focal encephalopathy resulting from postradiation edema secondary to microangiopathy led to dysfunction of the globus pallidus and thalamus, and secondary parkinsonism can occur in children after radiotherapy.
