Prospective Cohort Study

Guidelines summarize and evaluate all currently available evidence on a particular issue with the aim of assisting physicians in selecting the best management strategy for an individual patient suffering from a given condition, taking into account the impact on outcome, as well as the risk–benefit ratio of particular diagnostic or therapeutic means. Guidelines are no substitutes for textbooks. The legal implications of medical guidelines have been discussed previously.

A large number of Guidelines have been issued in recent years by the European Society of Cardiology (ESC) as well as by other societies and organizations. Because of the impact on clinical practice, quality criteria for development of guidelines have been established in order to make all decisions transparent to the user. The recommendations for formulating and issuing ESC Guidelines can be found on the ESC Web Site (http://www.escardio.org/guidelines-surveys/esc-guidelines/about/Pages/rules-writing.aspx).

In brief, experts in the field are selected and undertake a comprehensive review of the published evidence for management and/or prevention of a given condition. A critical evaluation of diagnostic and therapeutic procedures is performed, including assessment of the risk–benefit ratio. Estimates of expected health outcomes for larger societies are included, where data exist. The level of evidence and the strength of recommendation of particular treatment options are weighed and graded according to pre-defined scales, as outlined in Tables 1 and 2 .

Given an increase in the aging population and its impact on healthcare systems, policy makers for provision of health and social services are aiming to keep older adults in good health for longer, in other words towards ‘healthy aging’. Our study objective is to show that rehabilitation with cochlear implant treatment in the elderly with hearing impairment improves the overall health-related quality of life and general well-being that translate into healthy aging. The multicentre, prospective, repeated measures, single-subject, clinical observational study will accrue 100 elderly, first-time, unilateral CI recipients (≥ 60 years) and analyze changes on specific measurement tools over ca. 20 months from preimplant to postimplant. Evaluations will consist of details collected through case history and interview questionnaires by clinicians, data logging, self-report questionnaires completed by the recipients and a series of commonly used audiometric measures and geriatric assessment tools. The primary indicator of changes in overall quality of life will be the HUI-3. The protocol is designed to make use of measurement tools that have already been applied to the hearing-impaired population in order to compare effects of CI rehabilitation in adults immediately before their implantation, (pre-implant) and after gaining 1–1.5 years of experience (post-implant). The broad approach will lead to a greater understanding of how useful hearing impacts the quality of life in elderly individuals, and thus improves potentials for healthy aging. Healthy aging in elderly cochlear implant recipients: a multinational observational study.

Resume Introduction La « Confusion Assessment Method » (CAM), validee en langue anglaise par Inouye et collaborateurs, est l’un des outils de reference pour poser le diagnostic de confusion mentale et en apprecier la severite, tant en pratique qu’en recherche clinique. L’objectif de ce travail a ete de produire une version francophone consensuelle de l’outil CAM et du guide de formation a l’utilisation de la CAM. Methode Demarche d’adaptation transculturelle des questionnaires psychologiques. Resultats Les criteres de qualite d’une methodologie d’adaptation transculturelle de questionnaire ont ete reunis : recrutement de traducteurs qualifies de plusieurs nationalites et d’un linguiste, analyse de la bonne adaptation des composantes de l’outil a la culture cible, double traduction puis retro-traduction en aveugle, synthese et obtention d’une version experimentale preliminaire par methode du comite reunissant des cliniciens experimentes, pre-test de la version experimentale. Conclusion La version francophone de la CAM et de son guide d’utilisation va permettre une utilisation homogene de cet outil dans les pays francophones, d’ameliorer la communication entre les professionnels, d’optimiser le diagnostic du syndrome confusionnel. Cette demarche a permis la mise en place d’une etude clinique de validation des proprietes psychometriques de cette version francophone. L’appropriation de la CAM par les cliniciens ouvre des perspectives d’amelioration de la qualite des soins et de recherche dans les pays francophones.
