TL;DR: It is concluded that deep biopsy is often required and differential diagnoses that mimic the condition can be excluded with special histological stains, and targeted therapies and immunotherapies are being developed.

Treatment of cutaneous squamous cell carcinoma with perineural invasion using Mohs micrographic surgery: report of two cases and review of the literature.
TL;DR: Unpredictable tumor spread along the loose connective tissue of the perineurium allows the movement of single tumor cells away from the bulk of the tumor proximally and distally, making histologic identification and complete excision difficult.
