Enhanced recovery after surgery is well established in specialties such as colorectal surgery. It is achieved through the introduction of multiple evidence-based perioperative measures that aim to diminish postoperative organ dysfunction while facilitating recovery. This review aims to present consensus recommendations for the optimal perioperative management of patients undergoing thoracic surgery (principally lung resection). A systematic review of meta-analyses, randomized controlled trials, large non-randomized studies and reviews was conducted for each protocol element. Smaller prospective and retrospective cohort studies were considered only when higher-level evidence was unavailable. The quality of the evidence base was graded by the authors and used to form consensus recommendations for each topic. Development of these recommendations was endorsed by the Enhanced Recovery after Surgery Society and the European Society for Thoracic Surgery. Recommendations were developed for a total of 45 enhanced recovery items covering topics related to preadmission, admission, intraoperative care and postoperative care. Most are based on good-quality studies. In some instances, good-quality data were not available, and subsequent recommendations are generic or based on data extrapolated from other specialties. In other cases, no recommendation can currently be made because either equipoise exists or there is a lack of available evidence. Recommendations are based not only on the quality of the evidence but also on the balance between desirable and undesirable effects. Key recommendations include preoperative counselling, nutritional screening, smoking cessation, prehabilitation for high-risk patients, avoidance of fasting, carbohydrate loading, avoidance of preoperative sedatives, venous thromboembolism prophylaxis, prevention of hypothermia, short-acting anaesthetics to facilitate early emergence, regional anaesthesia, nausea and vomiting control, opioid-sparing analgesia, euvolemic fluid management, minimally invasive surgery, early chest drain removal, avoidance of urinary catheters and early mobilization after surgery. These guidelines outline recommendations for the perioperative management of patients undergoing lung surgery based on the best available evidence. As the recommendation grade for most of the elements is strong, the use of a systematic perioperative care pathway has the potential to improve outcomes after surgery.

Obesity is a multifactorial disease characterized by an excessive weight for height due to an enlarged fat deposition such as adipose tissue, which is attributed to a higher calorie intake than the energy expenditure. The key strategy to combat obesity is to prevent chronic positive impairments in the energy equation. However, it is often difficult to maintain energy balance, because many available foods are high-energy yielding, which is usually accompanied by low levels of physical activity. The pharmaceutical industry has invested many efforts in producing antiobesity drugs; but only a lipid digestion inhibitor obtained from an actinobacterium is currently approved and authorized in Europe for obesity treatment. This compound inhibits the activity of pancreatic lipase, which is one of the enzymes involved in fat digestion. In a similar way, hundreds of extracts are currently being isolated from plants, fungi, algae, or bacteria and screened for their potential inhibition of pancreatic lipase activity. Among them, extracts isolated from common foodstuffs such as tea, soybean, ginseng, yerba mate, peanut, apple, or grapevine have been reported. Some of them are polyphenols and saponins with an inhibitory effect on pancreatic lipase activity, which could be applied in the management of the obesity epidemic.

Glass Ceramic Phosphors: Towards Long-Lifetime High-Power White Light-Emitting-Diode Applications–A Review

Atomic monolayers of transition metal dichalcogenides represent an emerging material platform for the implementation of ultracompact quantum light emitters via strain engineering. In this framework, we discuss experimental results on creation of strain induced single photon sources using a WSe2 monolayer on a silver substrate, coated with a very thin dielectric layer. We identify quantum emitters that are formed at various locations in the sample. Their emission is highly linearly polarized, stable in linewidth, and decay times down to 100 ps are observed. We provide numerical calculations of our monolayer-metal device platform to assess the strength of the radiative decay rate enhancement by the presence of the plasmonic structure. We believe that our results represent a crucial step toward the ultracompact integration of high performance single photon sources in nanoplasmonic devices and circuits.

Spontaneous Emission Enhancement in Strain-Induced WSe2 Monolayer-Based Quantum Light Sources on Metallic Surfaces

We discuss coupling of site-selectively induced quantum emitters in exfoliated monolayers of WSe2 to plasmonic nanostructures. Gold nanorods of 20 nm-240 nm size, which are arranged in pitches of a few micrometers on a dielectric surface, act as seeds for the formation of quantum emitters in the atomically thin materials. We observe characteristic narrow-band emission signals from the monolayers, which correspond well with the positions of the metallic nanopillars with and without thin dielectric coating. Single photon emission from the emitters is confirmed by autocorrelation measurements, yielding g2(τ = 0) values as low as 0.17. Moreover, we observe a strong co-polarization of our single photon emitters with the frequency matched plasmonic resonances, as a consequence of light-matter coupling. Our work represents a significant step towards the scalable implementation of coupled quantum emitter-resonator systems for highly integrated quantum photonic and plasmonic applications.

Deterministic coupling of quantum emitters in WSe2 monolayers to plasmonic nanocavities.

Silicon carbide films have been grown onto graphite substrates by low pressure chemical vapor deposition using MTS (CH3SiCl3) as a source precursor and H2 or N2 as a diluent gas. The experiments were performed at fixed conditions of a flow rate of 100 sccm for each MTS and carrier gas, a flow rate of 300 sccm for diluent gas addition, and a total pressure of 5 torr. The effect of temperature from 900°C to 1350°C and the alteration of diluent gas species on the growth rate and structure of deposits have been studied. The experimental results showed that the deposition rate increased with increasing deposition temperature irrespective of diluent gases and reactant depletion effect increased especially at H2 diluent gas ambient. At MTS-H2 system, the deposition mechanism changed from chemical reaction to mass transfer controlled reaction with temperature. Otherwise, For MTS-H2-N2 system, surface chemical reaction controlled the growth process at whole deposition temperature ranges. For N2 addition, surface morphology of leaf-like structure appeared, and for H2, faceted structure at 1350°C. The observed features were involved by crystalline phase of β-SiC and surface composition with different gas ambient.
