Abstract: Two patients with hemochromatosis and what appears to be a distinctive arthritis are presented. Both showed prominent involvement of the hands, minimal evidence of inflammation, significant x-ray changes and synovial hemosiderin deposition. Five other cases of a similar arthritis were found in a review of 23 cases of idiopathic hemochromatosis. The arthritis is postulated to be the result of iron excess damaging articular tissue in a yet undefined manner.

Abstract: Injection of commercially available microcrystalline adrenocorticosteroid esters into normal canine joints was ac-companied by a leukocyte-rich exudative response, roughly proportional to the dose injected. No tenderness or evidence of pain was noted. Injection of these preparations into normal human joints often induced similar reactions which were sometimes painful. The response varied with the preparation employed and in the case of hydrocortisone acetate, with the manufacturer of the drug. The possible injurious effects of this relatively painless inflammatory response are discussed.

Abstract: A study was made of serum antibody to DNA and nucleoprotein in 35 patients with SLE, using immunofluorescent spot techniques. Anti DNA was confirmed by complement fixation and observation of the “shaggy” nuclear fluorescent pattern. A striking correlation was observed between presence of anti DNA and the acute phase of SLE. Anti-nucleoprotein, the most constant finding, showed only a general correlation of titer with phase of disease. Diagnostic and pathogenetic implications are discussed. Abstract: Kashin-Beck or Urov disease is described with authority by an astute scholar and clinician, Prof. Nesterov, Director of the Institute for Rheumatism and member of the Academy of Medical Sciences of the USSR in Moscow. The disease is a chronic, disabling, degenerative, generalized osteoarthrosis involving peripheral (symmetrical) joints and spine un-associated with systemic or visceral manifestations. It occurs principally in childhood and results in growth disturbances. It is endemic in Eastern Siberia, Northern China and Northern Korea and is believed to be due to ingestion of cereal grain grown in these areas and infected with the fungus, Fusaria sporotrichiella. The incidence has been considerably reduced and hopefully will soon be eradicated by importing cereals from unaffected areas. This is a classic example of a generalized degenerative joint disease due to environmental causes, unrelated to genetic factors or inborn errors of metabolism.

Abstract: HEN LOOKING for the major cause of deformity in the rheumatoid W metacarpophalangeal joint, several clinical observations offer clues. The characteristics of the early deformity recur with remarkable consistency, being comprised of varying degrees of luxation of the proximal phalanx in a volar and an ulnar direction, and deviation in an ulnar dir~ti0n.l.~ Secondly, the deformity has both passive and dynamic aspects. Passively, the finger can be luxated and deviated more than normal, even though it may be readily returned to a normal position in many patient^.^^^ Dynamically, the finger tracks along a more luxated and ulnarly deviated path during voluntary active flexion.? The passive deformity implies a loss of integrity of the specific structures normally preventing deviation and luxation. This damage in turn is indicative of forces acting consistently in the direction of the passive deformity. The dynamic deformity implies a new balance of forces during MCP flexion, which are in dynamic equilibrium only when the finger is tracking along a more ulnar and luxated path. Abstract: Five thousand four hundred and sixty-one normal individuals were tested for rheumatoid factor. The latex slide test was positive in 4.1 per cent SHC in 2.4 per cent and SSC in 0.18 per cent. There was no positive correlation of presence of rheumatoid factor and race or sex, but all tests increased in positivity with age. Physical examinations of individuals with at least one test positive for rheumatoid factor showed 5.4 percent had possible or definite rheumatoid arthritis.

Abstract: Daily injection of a crude papain solution into the knee joint of young rabbits continued over a six day period resulted in a degeneration of the articular cartilage within two weeks. The initial findings were elevation and loosening of the cartilage in the weight bearing area followed by thinning and fibrillation of the remaining surface. Microscopic changes consisted of disruption of the cartilage adjacent to the subchondral bone and loss of the superficial layers. Changes in the articular cartilage were similar to changes seen in clinical forms of degenerative arthritis. Abstract: Methotrexate, 6-mercaptopurine and total body x-irradiation, given to rats injected with mycobacterial adjuvant, suppressed the development of polyarthritis without altering hypersensitivity to old tuberculin. Mechlorethamine hydrochloride was without effect. This dissociation of arthritis and tuberculin hypersensitivity suggests that sensitization to tuberculoprotein plays no direct role in producing the arthritis. Methotrexate and 6-mercaptopurine also suppressed the early inflammation at the site of adjuvant injection indicating an antiinflammatory action which is independent of their effects on immunologic mechanisms. This agent is cyanide and heat sensitive and has typical enzyme behavior. It splits the mucoprotein at or near the P-P bond. This enzyme may initiate the degradation of ground substance seen in early osteoarthritis. Abstract: An immunofluorescent technique used for demonstrating antinuclear factor is discussed and the possible influence of differences in method on A.N.F. findings in family studies already reported is considered. The incidence of serum antinuclear factor in first degree relatives of patients suffering from systemic lupus erythematosus was higher than in matched population controls; the difference was a small but a significant one. Abstract: Lactic acid dehydrogenase (LDH) and glutamic-oxalacetic transaminase determinations were carried out on the serums and synovial fluids of patients with a variety of rheumatic diseases. The LDH activity of the synovial fluid was elevated in patients with rheumatoid degenerative joint disease. Transaminase activity was normal in both serum and synovial fluid in all cases. Starch gel zymograms of LDH showed qualitatively increased amounts of the isozyme LDH 5 in the pathological synovial fluids.

Abstract: Total, free (diffusible) and proteinbound salicylate concentrations were determined in synovial fluid samples obtained from 12 patients with rheumatoid arthritis, and compared with simultaneously obtained plasma concentrations. Free salicylate concentrations were essentially identical in the two fluids, implying complete equilibrium. Both the total and protein-bound salicylate concentrations were consistently lower in the synovial fluids than in the corresponding plasmas; this difference may be explained by the lower albumin concentration in the joint fluid. Abstract: The urinary excretion of tryptophan metabolites was measured in control subjects, in patients with active rheumatoid arthritis, and in patients with related disorders. After tryptophan loading, 12/15 female patients and 4/5 male patients with active rheumatoid arthritis and a positive latex test had significantly elevated levels of one or more of the tryptophan metabolites measured. Hydroxykynurenine and kynurenine were the metabolites most frequently elevated. Abstract: Chloroquine diphosphate was demonstrated to be capable of blocking the sulfhydryl disulfide interchange reaction that ordinarily occurs during the denaturation of bovine serum albumin by either heat or urea. Chloroquine was also shown to inhibit the reduction of ferricyanide by cysteine, and to bind to certain sulfhydryl groups of human serum protein when present in concentrations found in patients taking the compound for rheumatoid arthritis. Abstract: Five tryptophan metabolites were measured in the urine after an oral tryptophan load in patients with rheumatoid arthritis, scleroderma, systemic lupus and a variety of other diseases. An excessive excretion of kynurenine and 3-hydroxykynurenine was a common but nonspecific occurrence in rheumatic disease. The excretion did not correlate well with various clinical parameters. Inhibition of pyridoxal-dependent hepatic enzymes may be the cause of the abnormality but the mechanism is unknown. Further analyses with regard to hostility were also undertaken. Abstract: The treatment of calcinosis by chelation in two children with dermatomyositis is described. The amount of calcium in skin, muscle and fascia was chemically determined before and after chelation in one child. The amount in fascia was strikingly less after chelation than before chelation, though still elevated. Neither child showed clinical improvement. Treatment of calcinosis with chelating agents might be more effective if the drug were given over more prolonged periods. Current studies have extended the knowledge regarding the hematologic and biochemical abnormalities. Comparison of the abnormalities of this disease to human connective-tissue diseases is made. The arthritis following gonorrhea was similar to that following non-gonococcal urethritis and in both groups extramarital venereal exposure was the usual initiating event. No evidence could be found that mycoplasmal or viral agents were responsible for the arthritis. Abstract: Thirty patients with active rheumatoid arthritis were transfused with chromium-51 labeled erythrocytes from either normal or rheumatoid donors. Three sets of identical twins discordant for rheumatoid arthritis had a cross transfusion from a healthy donor to the affected recipient. In no instance was there any significant shortening of erythrocyte survival which could not be attributed to minor blood group incompatibility. The presence of rheumatoid factor in either the donor, the recipient, or both did not effect erythrocyte survival.
