Abstract: A regime of four topical applications of 1.0% sodium fluoride-1.0% chlorhexidine digluconate plus daily rinses with an 0.05% sodium fluoride-0.2% chlorhexidine solution prevented radiation caries completely. It also resulted in remineralization of incipient existing lesions. Use of the chlorhexidine-fluoride rinses alone also stopped radiation caries but did not permit remineralization to occur. Use of four topical applications with a fluoride gel and daily rinses with an 0.05% sodium fluoride solution was not sufficient to prevent radiation caries. No chlorhexidine staining was observed. The patients' acceptance of and compliance with the program and the rinses was excellent.

Abstract: There is a road that winds down from Kpayerkolli toward the sea, and in the dry season it is a road of red dust. In West Africa, where the villages are small and scattered in the bush, Kpayerkolli is just one of many such villages, and if you do not go with someone who knows it you will never find it. If you walk there, you will be coated with dust; it will sift into your ears and eyes and cover your clothes. The road is always full of people, especially children, since there are so many of them. A road means freedom to walk, freedom from poisonous snakes, and from the rough jungle terrain. If you talk to the children on the road, they will shyly say they are glad to meet you, that they are well (even if they are not), and that they are going to school

Abstract: A device designed to provide continuous topical fluoride therapy was evaluated in 11 adult males for one month; the devices released 0.5 mg of fluoride on the buccal surface of both maxillary first molars. Significant elevations in the level of fluoride were found in both whole saliva and plaque.

Abstract: Patients with renal failure have characteristic systemic and oral conditions that require special precautions during dental treatment. Drugs must be administered with caution and patients undergoing hemodialysis must receive special consideration. Both hemodialysis and transplant patients must be protected against infection. This is achieved by early, aggressive, and thorough dental treatment. Use of antibiotics during dental treatment is essential in transplant patients and is often necessary in hemodialysis patients to protect them against bacteremia. Bacterial endocarditis is a concern in patients with renal failure and with heart disease and, in these patients, antibiotic prophylaxis is essential. Hepatitis is often present in patients with renal failure, and precautions should be taken against its spread. Before dental treatment of patients with renal failure is begun, the dentist should consult the patient's physician regarding specific precautions. A treatment plan should be constructed which best restores the patient's dentition and protects from potentially severe infection of dental origin.

Abstract: The dentist who uses nitrous oxide sedation should be familiar with the characteristics of hypnotic behavior. A patient can enter the hypnotic state spontaneously and unintentionally, even more so under nitrous oxide, and can hallucinate sexual phenomena. Knowledge of hypnosis and the presence of a third person in the operatory during treatment can minimize the consequences of this phenomenon.

Abstract: Recurrent herpes simplex virus lesions of fingers, hand, or eyes are suspected to be encountered in health practitioners in greater frequency than in the general population. To determine whether an increased risk of disease contraction coincident to practicing dentistry exists and to determine the magnitude of risk, a survey of dentists practicing in the state of Michigan was conducted. An age and sex matched nondentist patient from each respondent's practice provided a control population. Frequency of herpes labialis and herpes infection of the eye were found be lower in dentists than in the control population. Conversely, herpetic whitlow was found to be more frequent in practicing dentists than in the control population. If the occurrence of herpes labialis in each of the two groups is taken as the reference point, the frequency of herpetic whitlow is significantly higher among practicing dentists than among the control population (P less than .01).
