TLDR
Despite good adherence and program-related findings, antiretroviral treatment is challenged by a range of hierarchical and interrelated factors and there is substantial room for improvement of ART programs in sub-Sahara African countries.Abstract:
Background. There are only a few comprehensive studies of adherence to ART and its challenges in Africa. This paper aims to assess the evidence on the challenges and prospects of ART adherence in sub-Saharan Africa. Methods. The authors reviewed original and review articles involving HIV-positive individuals that measured adherence to ART and its predictors in the past decade. Findings. Against expectations, sub-Saharan Africa patients have similar or higher adherence levels compared to those of developed countries. The challenges to ART adherence include factors related to patients and their families, socioeconomic factors, medication, and healthcare systems. Conclusion. Despite good adherence and program-related findings, antiretroviral treatment is challenged by a range of hierarchical and interrelated factors. There is substantial room for improvement of ART programs in sub-Sahara African countries.read more

TL;DR: More research using longitudinal designs and rigorous measures of adherence is required in order to identify the range of factors influencing ART adherence as adolescents living with HIV/AIDS in LMICs grow into adulthood.

Adherence to antiretroviral therapy and retention in care for adolescents living with HIV from 10 districts in Uganda

TL;DR: Being in rural areas was associated with poor adherence to ART and 1 year retention in care was very good among adolescents who were newly started on ART, while Stigma and disclosure issues remain the main barriers to adherence among adolescents.

Factors impacting antiretroviral therapy adherence among human immunodeficiency virus-positive adolescents in Sub-Saharan Africa: a systematic review.
TL;DR: Stigma reflects difficult relations between ALHIV and their HIV-negative peers and adults, suggesting a policy shift towards the wider community could be beneficial, and recommends engaging religious leaders and schools to change negative societal attitudes.

TL;DR: Interviews with participants and providers in a Phase IIb study evaluating an injectable LA regimen in the U.S. and Spain found that LA was seen as preferable to daily oral ART among PLHIV and appropriate candidates were needed, including with women and “non-adherent” populations across settings.

TL;DR: The findings indicate that favorable levels of adherence, much of which was assessed via patient self-report, can be achieved in sub-Saharan African settings and that adherence remains a concern in North America.
