Journal ArticleDOI
Effect of a Resuscitation Strategy Targeting Peripheral Perfusion Status vs Serum Lactate Levels on 28-Day Mortality Among Patients With Septic Shock: The ANDROMEDA-SHOCK Randomized Clinical Trial.
Glenn Hernandez,Gustavo A. Ospina-Tascón,Lucas P. Damiani,Elisa Estenssoro,Arnaldo Dubin,Javier Hurtado,Javier Hurtado,Gilberto Friedman,Ricardo Castro,Leyla Alegría,Jean-Louis Teboul,Maurizio Cecconi,Giorgio Ferri,Manuel Jibaja,Ronald Pairumani,Paula Virginia Fernández,Diego Barahona,Vladimir Granda-Luna,Alexandre Biasi Cavalcanti,Jan Bakker +19 more
TL;DR: Among patients with septic shock, a resuscitation strategy targeting normalization of capillary refill time, compared with a strategy targeting serum lactate levels, did not reduce all-cause 28-day mortality.
Journal ArticleDOI
The endothelium in sepsis.
Can Ince,Philip R. Mayeux,Trung C. Nguyen,Hernando Gomez,John A. Kellum,Gustavo A. Ospina-Tascón,Glenn Hernandez,Patrick T. Murray,Daniel De Backer +8 more
TL;DR: The response of the endothelial cell lining to sepsis in the kidney, liver, and lung is discussed and evidence as to whether the EC response toSepsis is adaptive or maladaptive is discussed.
Journal ArticleDOI
Second consensus on the assessment of sublingual microcirculation in critically ill patients: results from a task force of the European Society of Intensive Care Medicine
Can Ince,E. Christiaan Boerma,Maurizio Cecconi,Daniel De Backer,Nathan I. Shapiro,Jacques Duranteau,Michael R. Pinsky,Antonio Artigas,Jean-Louis Teboul,Irwin K.M. Reiss,Cesar Aldecoa,Sam Hutchings,Abele Donati,Marco Maggiorini,Fabio Silvio Taccone,Glenn Hernandez,Didier Payen,Dick Tibboel,Daniel Martin,Daniel Martin,Alexander Zarbock,Xavier Monnet,Arnaldo Dubin,Jan Bakker,Jan Bakker,Jan Bakker,Jean Louis Vincent,Thomas Scheeren +27 more
TL;DR: This consensus aims to provide clinicians with guidelines for practical use and interpretation of the sublingual microcirculation and promote the integration of routine application of HVM microcirculatory monitoring in conventional hemodynamic monitoring of systemic hemodynamic variables.
Journal ArticleDOI
Gut mucosal atrophy after a short enteral fasting period in critically ill patients.
Glenn Hernandez,Nicolás Velasco,Carol Wainstein,Luis Castillo,Guillermo Bugedo,Alberto Maiz,Francisco López,Sergio Guzmán,Claudio Vargas +8 more
TL;DR: It was found that a short period of enteral fasting was associated with significant duodenal mucosal atrophy and abnormal gut permeability in critically ill patients.
Journal ArticleDOI
The ten pitfalls of lactate clearance in sepsis.
TL;DR: Measurements every 1–2 h would give clinically relevant data about the decrease in lactate levels, and to talk about “lactate clearance” when actually describing a decrease in blood level is wrong and misleading.
