TL;DR: Clinicians should test patients with rash consistent with monkeypox,† regardless of whether the rash is disseminated or was preceded by prodrome, and public health efforts should prioritize gay, bisexual, and other men who have sex with men, who are currently disproportionately affected for prevention and testing.
Journal ArticleDOI
Combined high-resolution genotyping and geospatial analysis reveals modes of endemic urban typhoid fever transmission
Stephen Baker,Kathryn E. Holt,Kathryn E. Holt,Archie C. A. Clements,Abhilasha Karkey,Amit Arjyal,Maciej F. Boni,Sabina Dongol,Naomi Hammond,Samir Koirala,Pham Thanh Duy,Tran Vu Thieu Nga,James Campbell,Christiane Dolecek,Christiane Dolecek,Buddha Basnyat,Gordon Dougan,Jeremy Farrar,Jeremy Farrar +18 more
TL;DR: A longitudinal spatial case–control study for typhoid in Nepal is performed, combining single-nucleotide polymorphism genotyping and case localization via global positioning, and an extensive range of genotypes existing within typhoid clusters, and even within individual households, are demonstrated.
Journal ArticleDOI
The sensitivity of real-time PCR amplification targeting invasive Salmonella serovars in biological specimens
Tran Vu Thieu Nga,Abhilasha Karkey,Sabina Dongol,Hang Nguyen Thuy,Sarah J. Dunstan,Sarah J. Dunstan,Kathryn E. Holt,Le Thi Phuong Tu,James Campbell,James Campbell,Tran Thuy Chau,Nguyen Van Vinh Chau,Amit Arjyal,Samir Koirala,Buddha Basnyat,Christiane Dolecek,Christiane Dolecek,Jeremy Farrar,Jeremy Farrar,Stephen Baker,Stephen Baker +20 more
TL;DR: The data suggest that PCR performed directly on blood samples may be an unsuitable methodology and a potentially unachievable target for the routine diagnosis of enteric fever, whereby the bacterial load of S. Typhi in peripheral blood is low, often below detection by culture and, consequently, below detection by PCR.
Journal ArticleDOI
Variation at HLA-DRB1 is associated with resistance to enteric fever
Sarah J. Dunstan,Sarah J. Dunstan,Nguyen Thi Hue,Nguyen Thi Hue,Buhm Han,Zheng Li,Trinh T. B. Tram,Kar Seng Sim,Christopher M. Parry,Christopher M. Parry,Nguyen Tran Chinh,Ha Vinh,Nguyen Phu Huong Lan,Nga Tran Vu Thieu,Phat Voong Vinh,Samir Koirala,Sabina Dongol,Amit Arjyal,Abhilasha Karkey,Olita Shilpakar,Christiane Dolecek,Jia Nee Foo,Le Thi Phuong,Mai Ngoc Lanh,Tan Do,Tin Aung,Do Nu Hon,Yik Ying Teo,Martin L. Simmons,Stephen Baker,Paul I.W. de Bakker,Buddha Basnyat,Tran Tinh Hien,Jeremy Farrar,Chiea Chuen Khor +39 more
TL;DR: Imputation-based fine-mapping across the extended MHC region showed that the classical HLA-DRB1*04:05 allele could entirely explain the association at rs7765379, thus implicating HLA/DQB1 as a major contributor to resistance against enteric fever, presumably through antigen presentation.
Journal ArticleDOI
The Burden and Characteristics of Enteric Fever at a Healthcare Facility in a Densely Populated Area of Kathmandu
Abhilasha Karkey,Amit Arjyal,Katherine L. Anders,Maciej F. Boni,Sabina Dongol,Samir Koirala,Phan Vu Tra My,Tran Vu Thieu Nga,Archie C. A. Clements,Kathryn E. Holt,Pham Thanh Duy,Jeremy N. Day,James Campbell,Gordon Dougan,Christiane Dolecek,Jeremy Farrar,Buddha Basnyat,Stephen Baker +17 more
TL;DR: The uneven distribution of enteric fever patients within the population suggests local variation in risk factors, such as contaminated drinking water, which is important for initiating a vaccination scheme and improvements in sanitation.
