XI. STRATEGIES FOR IMPROVING DIABETES CARE D iabetes is a chronic illness that requires continuing medical care and patient self-management education to prevent acute complications and to reduce the risk of long-term complications. Diabetes care is complex and requires that many issues, beyond glycemic control, be addressed. A large body of evidence exists that supports a range of interventions to improve diabetes outcomes. These standards of care are intended to provide clinicians, patients, researchers, payors, and other interested individuals with the components of diabetes care, treatment goals, and tools to evaluate the quality of care. While individual preferences, comorbidities, and other patient factors may require modification of goals, targets that are desirable for most patients with diabetes are provided. These standards are not intended to preclude more extensive evaluation and management of the patient by other specialists as needed. For more detailed information, refer to Bode (Ed.): Medical Management of Type 1 Diabetes (1), Burant (Ed): Medical Management of Type 2 Diabetes (2), and Klingensmith (Ed): Intensive Diabetes Management (3). The recommendations included are diagnostic and therapeutic actions that are known or believed to favorably affect health outcomes of patients with diabetes. A grading system (Table 1), developed by the American Diabetes Association (ADA) and modeled after existing methods, was utilized to clarify and codify the evidence that forms the basis for the recommendations. The level of evidence that supports each recommendation is listed after each recommendation using the letters A, B, C, or E.

Because of new evidence on several diagnostic and therapeutic aspects of hypertension, the present guidelines differ in many respects from the previous ones. Some of the most important differences are listed below: 
1. Epidemiological data on hypertension and BP control in Europe. 
2. Strengthening of the prognostic value of home blood pressure monitoring (HBPM) and of its role for diagnosis and management of hypertension, next to ambulatory blood pressure monitoring (ABPM). 
3. Update of the prognostic significance of night-time BP, white-coat hypertension and masked hypertension. 
4. Re-emphasis on integration of BP, cardiovascular (CV) risk factors, asymptomatic organ damage (OD) and clinical complications for total CV risk assessment. 
5. Update of the prognostic significance of asymptomatic OD, including heart, blood vessels, kidney, eye and brain. 
6. Reconsideration of the risk of overweight and target body mass index (BMI) in hypertension.

Ezétimibe et hypercholestérolémie familiale : une limite de l’adage « lower is better » ?

L’efficacité et la sécurité de la chirurgie bariatrique chez les sujets présentant une obésité hypothalamique secondaire à un craniopharyngiome (OHCP) sont mal connues à long terme. L’objectif de notre travail était d’évaluer les effets à 5 ans de la chirurgie bariatrique dans cette population. Il s’agit d’une étude rétrospective multicentrique française comparant des sujets présentant une OHCP à des sujets ayant une obésité commune (OC) ayant été opérés il y a au moins 5 ans d’une sleeve gastrectomie (SG) ou d’un bypass gastrique Roux-en-Y (BPY). Vingt-trois sujets OHCP ont été inclus (9 opérés de SG, 14 de BPY, âge médian 35 ans [quartile range : 25 ; 43], indice de masse corporelle médian 44,2 kg/m 2 [40,7 ; 51], 8/23 diabétiques). La perte de poids était significativement inférieure dans le groupe OHCP que dans le groupe OC (−17,8 % vs −26,2 % [ p = 0,003]). Cela était encore plus vrai parmi les sujets opérés de BPY (OHCP : −13,2 % vs OC : −31,6 % [ p = 0,001]). Le taux de rémission du diabète de type 2 était comparable entre les patients OHCP et OC. Aucune différence sur le risque de complications postopératoires n’a été observée. Aucune décompensation aiguë d’un déficit hypophysaire n’a été rapportée dans le groupe OHCP. La chirurgie bariatrique permet une perte de poids significative à 5 ans chez les sujets OHCP, mais elle est moindre que les sujets présentant une OC. 

Résultats pondéraux et métaboliques à 5 ans d’une chirurgie bariatrique chez des sujets présentant une obésité hypothalamique secondaire à un craniopharyngiome : une étude cas-contrôle du réseau FORCE

La chirurgie bariatrique est actuellement le traitement le plus efficace dans la prise en charge de l’obésité sévère. La perte pondérale majeure induite en postopératoire permet une amélioration des comorbidités métaboliques, dont la stéatose hépatique. Cependant, une cytolyse précoce, parfois persistante, a été décrite dans la littérature en dehors de toute hépatopathie connue. L’objectif de notre étude est de décrire la proportion de patients présentant une cytolyse après chirurgie bariatrique et d’identifier des facteurs de risque conduisant à cette situation clinique. Tous les patients ayant eu une chirurgie bariatrique entre 2016 et 2019 au sein de notre service de nutrition ont été inclus dans l’étude. Les paramètres cliniques et biologiques pertinents ont été recueillis en pré- et postopératoire à 3 et 6 mois. L’élévation des transaminases était définie par l’augmentation des ASAT et/ou ALAT de 50 % à 3 et 6 mois par rapport aux valeurs préopératoires. Nous avons analysé séparément notre population suivant un seuil > ou < 35 UI/L. Parmi les 569 patients opérés, près de 10 % (n = 54) ont eu une augmentation d’un facteur 1,5 des transaminases en postopératoire. Il s’agissait majoritairement de femmes, âgées de 45 (±11) ans, ayant une obésité plus sévère en préopératoire et à 6 mois postopératoire que le reste de la population opérée. Le type de chirurgie bariatrique n’intervenait pas dans l’évolution des transaminases. De la même façon, l’intensité de la perte pondérale n’était pas associée à l’évolution des transaminases. Une amélioration non significative entre la phase préopératoire et à 6 mois a été observée pour les valeurs mesurées au fibroscan : pour la stéatose estimée par le CAP (305 ± 63 puis 274 ± 72 dB/m, p = 0,19) et l’élasticité estimée (8,6 ± 6 puis 6,2 ± 2 kPa, p = 0,27). Aucun patient n’a développé d’insuffisance hépatocellulaire après la chirurgie. Dans cette étude rétrospective monocentrique incluant des patients obèses opérés de chirurgie bariatrique, nous avons observé une majoration des transaminases dans le semestre postopératoire pour 10 % de notre cohorte. Les facteurs de risque identifiés sont le sexe féminin et l’importance de l’obésité préopératoire. Il n’a pas été retrouvé d’association avec le type de chirurgie ni l’intensité de la perte de poids. Cette élévation est transitoire avec une résolution rapide du bilan biologique sans impact clinique significatif. Les mécanismes sous-jacents de la détérioration de la fonction hépatique restent mal compris et de nombreux éléments physiopathologiques ont été proposés dont la dénutrition ou l’inflammation portale. Une surveillance biologique hépatique systématique ne semble pas nécessaire et devrait être réservée aux patients à haut risque, notamment avec une maladie hépatique préalable.

Le surpoids et l’obesite ont vu leur prevalence augmenter dans la population feminine en âge de procreer et sont de plus en plus frequents lors des consultations pour infertilite. Ils constituent un facteur non negligeable de baisse de la fertilite naturelle. La graisse abdominale, par le biais de l’insulinoresistance, l’hyperinsulinisme et l’hyperandrogenie, favorise sur un terrain predispose a la survenue de troubles de l’ovulation. Le depistage et la prise en charge du syndrome metabolique doivent etre systematiques dans ce cadre. L’obtention d’une perte de poids meme moderee suffit a ameliorer l’ovulation spontanee ou l’efficacite des traitements inducteurs de l’ovulation. L’alimentation en elle-meme affecte egalement l’ovulation. L’instauration d’un regime de type mediterraneen et d’une activite physique reguliere sont des mesures simples a mettre en place. Elles doivent imperativement preceder la mise en route des therapeutiques specifiques et peuvent parfois suffire, a elles seules, a l’obtention de la grossesse desiree. Chez les patientes ne presentant pas de troubles de l’ovulation, surpoids et obesite augmentent le delai necessaire pour concevoir, diminuent les chances de succes des traitements et augmentent le risque de fausse couche. La aussi, une alimentation de type mediterraneenne et la pratique d’une activite physique reguliere sont associees a de meilleures chances de succes des traitements d’AMP.La prise en compte du style de vie et la correction des facteurs defavorables par des conseils appropries ou une prise en charge specifique devraient donc etre systematiques, tout comme la prescription d’acide folique, chez toute femme desirant concevoir un enfant.
