Journal ArticleDOI
Lower tidal volume strategy (≈3 ml/kg) combined with extracorporeal CO2 removal versus 'conventional' protective ventilation (6 ml/kg) in severe ARDS: the prospective randomized Xtravent-study.
Thomas Bein,Steffen Weber-Carstens,Anton Goldmann,Thomas Müller,Thomas Staudinger,Jörg Brederlau,Ralf Michael Muellenbach,Rolf Dembinski,Bernhard M. Graf,Marlene Wewalka,Alois Philipp,Klaus-Dieter Wernecke,Matthias Lubnow,Arthur S. Slutsky +13 more
TL;DR: The use of very low VT ventilation combined with extracorporeal CO2 removal has the potential to further reduce VILI compared with a ‘normal’ lung protective management.
Journal ArticleDOI
Extracorporeal membrane oxygenation: evolving epidemiology and mortality.
Christian Karagiannidis,Daniel Brodie,Stephan Strassmann,Erich Stoelben,Alois Philipp,Thomas Bein,Thomas Müller,Wolfram Windisch +7 more
TL;DR: In a high-income country like Germany, the use of ECMO has been rapidly increasing since 2007 for both respiratory and cardiac support, with a recent plateau in vv-ECMO use.
Journal ArticleDOI
Extracorporeal Life Support Organization Coronavirus Disease 2019 Interim Guidelines: A Consensus Document from an International Group of Interdisciplinary Extracorporeal Membrane Oxygenation Providers.
Kiran Shekar,Jenelle Badulak,Giles J. Peek,Udo Boeken,Heidi J. Dalton,Lovkesh Arora,Bishoy Zakhary,Kollengode Ramanathan,Joanne P. Starr,Bindu Akkanti,M. Velia Antonini,Mark Ogino,Mark Ogino,Lakshmi Raman,Nicholas Barret,Daniel Brodie,Alain Combes,Roberto Lorusso,Graeme MacLaren,Thomas Müller,Matthew L. Paden,Vincent Pellegrino +21 more
TL;DR: These guidelines are not meant to replace sound clinical judgment or specialist consultation but rather to strengthen provision and clinical management of ECMO specifically, in the context of the COVID-19 pandemic.
Journal ArticleDOI
Extracorporeal membrane oxygenation for respiratory failure in adults.
TL;DR: The latest generation of ECMO systems is more biocompatible, better performing and longer lasting, and although recent studies suggested that veno-venous ECMO might improve the outcomes of patients with ARDS, indications for ECMO use remain uncertain.
Journal ArticleDOI
Extracorporeal lung support in trauma patients with severe chest injury and acute lung failure: a 10-year institutional experience.
Michael Ried,Thomas Bein,Alois Philipp,Thomas Müller,Bernhard M. Graf,Christof Schmid,David Zonies,Claudius Diez,Hans-Stefan Hofmann +8 more
TL;DR: Pumpless and pump-driven ELS systems are an excellent treatment option in severe thoracic trauma patients with ALF and facilitate survival in an experienced trauma center with an interdisciplinary treatment approach and are encouraged to use vv-ECMO due to reduced complication rates, better oxygenation and best short-term outcome.
