Abstract: To determine the effectiveness of school-based interventions that focus on changing dietary intake and physical activity levels to prevent childhood obesity. MEDLINE and EMBASE were searched (January 2006 to September 2007) for controlled trials of school-based lifestyle interventions, minimum duration of 12 weeks, reporting weight outcome. Thirty-eight studies were included; 15 new studies and 23 studies included within the National Institute for Health and Clinical Excellence obesity guidance. One of three diet studies, five of 15 physical activity studies and nine of 20 combined diet and physical activity studies demonstrated significant and positive differences between intervention and control for body mass index. There is insufficient evidence to assess the effectiveness of dietary interventions or diet vs. physical activity interventions. School-based physical activity interventions may help children maintain a healthy weight but the results are inconsistent and short-term. Physical activity interventions may be more successful in younger children and in girls. Studies were heterogeneous, making it difficult to generalize about what interventions are effective. The findings are inconsistent, but overall suggest that combined diet and physical activity school-based interventions may help prevent children becoming overweight in the long term. Physical activity interventions, particularly in girls in primary schools, may help to prevent these children from becoming overweight in the short term.

Abstract: Enhancing cardiovascular fitness can lead to substantial health benefits. High-intensity interval training (HIT) is an efficient way to develop cardiovascular fitness, yet comparisons between this type of training and traditional endurance training are equivocal. Our objective was to meta-analyse the effects of endurance training and HIT on the maximal oxygen consumption (VO2max) of healthy, young to middle-aged adults. Six electronic databases were searched (MEDLINE, PubMed, SPORTDiscus, Web of Science, CINAHL and Google Scholar) for original research articles. A search was conducted and search terms included ‘high intensity’, ‘HIT’, ‘sprint interval training’, ‘endurance training’, ‘peak oxygen uptake’, and ‘VO2max’. Inclusion criteria were controlled trials, healthy adults aged 18–45 years, training duration ≥2 weeks, VO2max assessed pre- and post-training. Twenty-eight studies met the inclusion criteria and were included in the meta-analysis. This resulted in 723 participants with a mean ± standard deviation (SD) age and initial fitness of 25.1 ± 5 years and 40.8 ± 7.9 mL·kg−1·min−1, respectively. We made probabilistic magnitude-based inferences for meta-analysed effects based on standardised thresholds for small, moderate and large changes (0.2, 0.6 and 1.2, respectively) derived from between-subject SDs for baseline VO2max. The meta-analysed effect of endurance training on VO2max was a possibly large beneficial effect (4.9 mL·kg−1·min−1; 95 % confidence limits ±1.4 mL·kg−1·min−1), when compared with no-exercise controls. A possibly moderate additional increase was observed for typically younger subjects (2.4 mL·kg−1·min−1; ±2.1 mL·kg−1·min−1) and interventions of longer duration (2.2 mL·kg−1·min−1; ±3.0 mL·kg−1·min−1), and a small additional improvement for subjects with lower baseline fitness (1.4 mL·kg−1·min−1; ±2.0 mL·kg−1·min−1). When compared with no-exercise controls, there was likely a large beneficial effect of HIT (5.5 mL·kg−1·min−1; ±1.2 mL·kg−1·min−1), with a likely moderate greater additional increase for subjects with lower baseline fitness (3.2 mL·kg−1·min−1; ±1.9 mL·kg−1·min−1) and interventions of longer duration (3.0 mL·kg−1·min−1; ±1.9 mL·kg−1·min−1), and a small lesser effect for typically longer HIT repetitions (−1.8 mL·kg−1·min−1; ±2.7 mL·kg−1·min−1). The modifying effects of age (0.8 mL·kg−1·min−1; ±2.1 mL·kg−1·min−1) and work/rest ratio (0.5 mL·kg−1·min−1; ±1.6 mL·kg−1·min−1) were unclear. When compared with endurance training, there was a possibly small beneficial effect for HIT (1.2 mL·kg−1·min−1; ±0.9 mL·kg−1·min−1) with small additional improvements for typically longer HIT repetitions (2.2 mL·kg−1·min−1; ±2.1 mL·kg−1·min−1), older subjects (1.8 mL·kg−1·min−1; ±1.7 mL·kg−1·min−1), interventions of longer duration (1.7 mL·kg−1·min−1; ±1.7 mL·kg−1·min−1), greater work/rest ratio (1.6 mL·kg−1·min−1; ±1.5 mL·kg−1·min−1) and lower baseline fitness (0.8 mL·kg−1·min−1; ±1.3 mL·kg−1·min−1). Endurance training and HIT both elicit large improvements in the VO2max of healthy, young to middle-aged adults, with the gains in VO2max being greater following HIT when compared with endurance training.

Abstract: For publication in the International Journal of Sports Medicine (IJSM), studies involving human participants or animals must have been conducted in accordance with recognised ethical standards and national/international laws. At the very first submission stage, authors are required to confirm that these standards and laws have been adhered to by reading, and formally citing, this editorial within the methods section of their own manuscript. Authors who do not provide any information regarding ethical approval will have their manuscripts rejected before it enters the peer-review process, without any option to resubmit. In the 21st Century, research opportunities, approaches and environments are in a continual state of flux, and this is also the case for the associated ethical issues. In the original 2009 IJSM editorial [3], we described the ethical considerations embedded into national/international laws and provided specific guidance on the ethical issues which commonly arise in Sports Medicine research. In 2011, this information was updated to recognise the ethical principles of other professional associations and treaties when conducting research involving human participants [4]. Additional information was also provided on the use of Laboratory Animals in research, and on the links between sample size and research ethics. In the second update, published in 2014, we elaborated on the ethical issues relating to the investigation of doping agents; the use of animals for answering research questions that appear to be solely focussed on the enhancement of athletic performance; and sample size in the context of the burden to individual research participants. In this, our new update for 2016 onwards, we; A. Update some of the guidelines to account for the changes made to the Declaration of Helsinki in 2013. B. Cover the use of social media in research C. Provide guidance on how researchers can feed back their incidental and pertinent findings to research participants D. Cover some of the issues relating to studies involving children E. Outline the difference between a full and pilot study in terms of desired number of particpants. F. Describe good practice points on the storage of data
