Mineralocorticoid-resistant renal hyperkalemia without salt wasting (type II pseudohypoaldosteronism): role of increased renal chloride reabsorption.

TL;DR: It is suggested that the primary abnormality in this syndrome increases the reabsorptive avidity of the distal nephron for chloride, which limits the sodium and mineralocorticoid-dependent voltage driving force for potassium and hydrogen ion secretion, resulting in hyperkalemia and acidosis and augments distal sodium chloride reabsorption resulting inhyperchloremia, volume expansion, hyporeninemia, and hypertension.

A comparison of plasma vasopressin measurements with a standard indirect test in the differential diagnosis of polyuria.

TL;DR: It is concluded that the incorporation of a vasopressin assay improves accuracy in the differential diagnosis of polyuria and may be explained by increased sensitivity to low concentrations of vasoppressin in the former disorder and a reduction of maximal concentrating ability in both.

The use of hypertonic saline infusions in the differential diagnosis of diabetes insipidus and psychogenic polydipsia.

TL;DR: Hare and his associates in a series of studies on normal dogs have shown that the concentration of chlorides in the tubular reabsorbate (R), the ratio of R to plasma chlorides (R/P) and the urine flow, are all decreased following infusions of hypertonic saline, concomitantly with an increase of antidiuretic substance in the urine.

A water deprivation test for the differential diagnosis of polyuria.

TL;DR: A relatively simple, standardized 6 1/2 hour water deprivation test is described which adequately differentiates disorders of the neurohypophyseal-hypothalamic system from primary polydipsia and patients with diabetes insipidus demonstrate defective urinary concentrating capacity.
