Guidelines for the Management of Adult Acute and Acute-on-Chronic Liver Failure in the ICU: Cardiovascular, Endocrine, Hematologic, Pulmonary, and Renal Considerations.
Rahul Nanchal,Ram M. Subramanian,Constantine J. Karvellas,Steven M. Hollenberg,William Peppard,Kai Singbartl,Jonathon D. Truwit,Ali Al-Khafaji,Alley Killian,Mustafa Alquraini,Khalil Ibrahim Alshammari,Fayez Alshamsi,Emilie P. Belley-Côté,Rodrigo Cartin-Ceba,Joanna C. Dionne,Dragos Galusca,David T. Huang,Robert C. Hyzy,Mats Junek,Prem A. Kandiah,Gagan Kumar,Rebecca L. Morgan,Peter E. Morris,Jody C. Olson,Rita Sieracki,Randolph H. Steadman,Beth Taylor,Waleed Alhazzani +27 more
TL;DR: Multidisciplinary international experts were able to formulate evidence-based recommendations for the management acute or acute on chronic liver failure in the ICU, acknowledging that most recommendations were based on low-quality indirect evidence.
Journal ArticleDOI
Prognosis of cirrhotic patients admitted to intensive care unit: a meta-analysis
Delphine Weil,Eric Levesque,Marc McPhail,Rodrigo Cavallazzi,Eleni Theocharidou,Evangelos Cholongitas,Arnaud Galbois,Heng Chih Pan,Constantine J. Karvellas,Bertrand Sauneuf,René Robert,Jérome Fichet,Gaël Piton,Thierry Thevenot,Gilles Capellier,Vincent Di Martino +15 more
TL;DR: Prognosis of cirrhotic patients admitted to ICU is poor since only a minority undergo liver transplant, and the prognostic performance of general ICU scores decreases over time, unlike the Child–Pugh and MELD scores, even recorded in the context of organ failure.
Journal ArticleDOI
Pathophysiology of Acute Liver Failure
TL;DR: Multiple aspects of nutrition also need to be considered as part of the overall pathophysiology of ALF, as the liver is a tremendously important metabolic organ involved in energy metabolism, protein synthesis, fat metabolism, and glycemic control.
Journal ArticleDOI
Therapeutic hypothermia in acute liver failure: a multicenter retrospective cohort analysis.
Constantine J. Karvellas,R. Todd Stravitz,Holly Battenhouse,William M. Lee,Michael L. Schilsky +4 more
TL;DR: Although young ALF patients in the US Acute Liver Failure Study Group with grade III or IV hepatic encephalopathy may benefit, TH did not consistently affect 21‐day survival and complications in ALf patients at high risk for cerebral edema.
Journal ArticleDOI
Liver transplantation in the critically ill: a multicenter Canadian retrospective cohort study
Constantine J. Karvellas,Thomas Lescot,Peter Goldberg,Michael D. Sharpe,Juan J. Ronco,Eberhard L. Renner,Hina Vahidy,Zafrina Poonja,Prosanto Chaudhury,Norman M. Kneteman,Markus Selzner,Earl Francis Cook,Sean M. Bagshaw +12 more
TL;DR: SOFA appears poor at predicting 90-day survival in critically ill cirrhosis patients after LT, but higher SOFA score and elevated lactate 48 hours after ICU admission are associated with a lower probability receiving LT.
