TL;DR: It is suggested that SRH is a frequent accompanying symptom of epileptic seizures causing major impairment in daily life, and migraine is an important comorbidity of epilepsy, affecting the incidence and characteristics of SRH.
Abstract: Purpose The purpose of this study is to investigate the frequency and characteristics of migraine and seizure-related headache (SRH) according to the criteria of the International Headache Society. Materials and Methods A questionnaire was undertaken at the initial evaluation of newly referred patients from 32 epilepsy clinics. Results Of a total of 597 patients, 74 (12.4%) patients had migraine. Age at the onset of epilepsy was lower in patients with migraine than in those without. Twenty-six (4.4%), nine (1.5%), and 146 (24.5%) patients experienced prodromal, ictal, and postictal SRH, respectively (n = 169, 28.3%). A pain intensity of prodromal and postictal SRH was 6.1 ± 1.5 (SD) and 6.3 ± 1.9 (SD) on the visual analogue scale, and their duration was 12.6 ± 26.7 (SD) hours and 9.0 ± 17.4 (SD) hours, respectively. Age at the onset of epilepsy was lower in patients with SRH than in those without, and the risk of occurrence of SRH was significantly greater in patients with longer epilepsy duration. SRH could be classified as a type of migraine in 46.2% of patients with prodromal SRH and in 36.3% of patients with postictal SRH. Prodromal SRH occurred more frequently and was more likely to be a migraine-type in patients with migraine compared with those without. Postictal SRH occurred more frequently and was more likely to be a migraine-type in patients with migraine. Conclusion This study suggests that SRH is a frequent accompanying symptom of epileptic seizures causing major impairment in daily life, and migraine is an important comorbidity of epilepsy, affecting the incidence and characteristics of SRH.

TL;DR: Virreous infiltration without uveoretinal involvement can be an unusual manifestation of intraocular NK/T-cell lymphoma and Clinician awareness of possible ocular involvement may assist in the diagnosis of disseminated NK/ T- cell lymphoma.
Abstract: Purpose To report an unusual presentation of disseminated, intraocular, extranodal natural killer/T-cell lymphoma, nasal type (NK/T-cell lymphoma), originating from nasal NK/T-cell lymphoma. Methods Case report. Results A 63-year-old woman who had been treated with systemic chemotherapy and radiotherapy for NK/T-cell lymphoma in the nasal cavity presented with vitreous haze of the right eye. Despite anti-inflammatory therapy, the right eye showed poor clinical response and received diagnostic vitrectomy. The vitreous opacity resembled a thin sheet, with no sign of subretinal infiltrate or vascular sheathing. The vitreous specimen contained many large, pleomorphic lymphoma cells. The malignant cells were positive for CD3, CD8, and granzyme B. Systemic workups showed no involvement of other organs. A diagnosis of T-cell lymphoma in the vitreous was made; the tumor likely originated from nasal NK/T-cell lymphoma. The patient was treated with intrathecal chemotherapy and intravitreal methotrexate injection. The eye was clinically clear of malignant cells after the injections. Conclusions Vitreous infiltration without uveoretinal involvement can be an unusual manifestation of intraocular NK/T-cell lymphoma. Clinician awareness of possible ocular involvement may assist in the diagnosis of disseminated NK/T-cell lymphoma.

TL;DR: This study identified the items of the asthma guidelines preferred by physicians in Korea and identified the most effective strategies for encouraging the use of the guidelines through continuing medical education and discussions with colleagues.
Abstract: Purpose Patient care based on asthma guidelines is cost-effective and leads to improved treatment outcomes. However, ineffective implementation strategies interfere with the use of these recommendations in clinical practice. This study investigated physicians' preferences for asthma guidelines, including content, supporting evidence, learning strategies, format, and placement in the clinical workplace.

TL;DR: A 54-year-old man was admitted to hospital with intermittent lower back pain and the tumor was confirmed to be clear cell ependymoma by histopathologic examination and his symptom was relieved after surgery.
Abstract: The authors present a rare case of clear cell ependymoma that developed in the cauda equina. A 54-year-old man was admitted to hospital with intermittent lower back pain. A neurological examination conducted on admission revealed no sensory or motor disturbance. Deep tendon reflexes in both lower extremities were normal. Magnetic resonance images demonstrated a 1.0 cm-sized intradural mass at the filum terminale. Gross total resection was performed via total laminectomy of L1 and L2. The tumor was confirmed to be clear cell ependymoma by histopathologic examination. His symptom was relieved after surgery.
