The Diagnostic and Statistical Manual of Mental Disorders, fourth edition, text revision (DSM-IV-TR) (American Psychiatric Association [APA], 2000) is a compendium of mental disorders, a listing of the criteria used to diagnose them, and a detailed system for their definition, organization, and classification. Put simply, it is the primary diagnostic manual for mental health professionals in the United States and much of the Western world. Diagnosis refers to the identification and labeling of a mental disorder by examination and analysis. Mental health professionals diagnose individuals based on the symptoms that they report experiencing and the signs of illness with which they present. The DSM-IV-TR aids professionals in understanding and diagnosing mental disorders through its provision of explicit diagnostic criteria and an official classification system.

The assumption that dynamic risk factors (DRF) are causal concepts as well as predictive ones is apparent in their dual roles in practice and in the theories explored in the previous chapter; an assumption that we argue is mistaken. In this chapter we examine a number of conceptual and empirical problems with DRF which we view as arising from their composite nature. Specifically, the conceptual problems of incoherence, lack of specificity or reference, the grain problem, and lack of factualness or normativity. We will then briefly summarize the state of empirical research which has looked at the statistical relationship between DRF and behavior. We suggest that these many flaws mean that they cannot play meaningful explanatory or treatment planning roles, at least not in their current state.

Treatment of Research and Sexual Abuse: A Journal

A great deal of research has focused on attempting to investigate the cognitions of child sexual offenders. However, a greater emphasis is required on beliefs, actions, values and their interactions. The Sex Offender Relationship Frame model, derived by grounded theory using a sample of adult males incarcerated for child sexual offences, represents a useful framework for the role of values in the reasoning and decision-making of offenders, and offers important theoretical and treatment implications. This article employs a case study methodology utilising four descriptive cases in order to comprehensively illustrate each of the four relationship frames identified within this model. The detailed cases demonstrate the importance of considering the role of values held by offenders in furthering both our understanding of child sexual offending and also our treatment practices.

Sexual offenders with major mental disorder are a heterogeneous population, presenting with co-morbid psychiatric disorders (Laws and O’Donohue. Sexual deviance: Theory, assessment, and treatment (2nd edn). New York: Guilford Press, 2008), personality difficulties (Blake & White, 2002), cognitive impairments (Sturmey and McMurran. Forensic case formulation. Chichester, UK: Wiley-Blackwell, 2011) and substance misuse (Olver and Wong. Psychology, Crime & Law, 19(5–6), 477–492, 2013). They are subjected to double stigma and dual minority status, due to their membership of two highly marginalized groups within society (Kelley and Thornton. Journal of Aggression. Conflict and Peace Research, 7, 258–274, 2016). Additionally, the interplay between forensic mental health and criminal justice systems necessitates that forensic practitioners actively manage ethical dilemmas or dual relationship problems, which requires one to provide treatment and simultaneously protect the wider community (e.g. custody vs. care – Ward. Aggression and Violent Behavior, 18, 92–100, 2013). Historically, mentally disordered sexual offenders (MDSOs) have been underrepresented within the sexual offender research and practice literature, owing to an overreliance on correctional study samples. However, a recent focus on MDSOs within the forensic arena has raised a number of important theoretical questions and practical considerations regarding the psychological assessment, treatment and risk management of this complex offender group. This chapter will provide an overview and explore each of these domains and conclude by considering the key research and practice issues for MDSO populations.
