TL;DR: Combining the MEST score with cross-sectional clinical data at biopsy provides earlier risk prediction in IgAN than current best methods, which requires clinical data over 2 years of follow-up.

TL;DR: Limitations in current classification criteria and definitions for vasculitis have been identified and suggestions provided for improvement and it is proposed that, in combination with the updated evidence, these should form the basis of future attempts to develop and validate revised criteria and definition of vasulitis.

TL;DR: Allocation of immunosuppression to high-risk patients with IgAN can be substantially improved using the Prediction Tool compared to using proteinuria, which had a negative net benefit and was harmful.
