Abstract: Background: Aluminium phosphide is a solid fumigant pesticide and it has currently aroused interest with increasing number of poisoning cases in past three decades. Over the time it has become an effective and widely used medium for suicide and homicide. Aluminium phosphide has highly effective killing power. Death is due to its direct toxic effect over the heart leading to peripheral circulatory failure .Aluminium phosphide poisoning affects the most of the organs. Early symptoms include nausea, vomiting, retrosternal and epigastric pain, dyspnea, anxious, agitation and smell of garlic breath. Methods: 50 patients, who were admitted in MICU, following consumption of aluminium phosphide tablets were included in the study. Patient’s clinical evaluation was done as per standard. PaO 2 was noted with pulse oxymeter. Troponin I, ECG and echo of the patients was done to study the cardiac toxicities and pathological reports were correlated. Results: Out of 50 cases, ECG was normal in 20 cases and abnormal in 30 cases-sinus tachycardia was most common finding (24%) while heart block was least (4%). On echo, hypokinesia was seen in 40% cases, decreased ejection fraction in 8% cases and normal echo was seen in 12% cases. Serum troponin test was positive in 26%. Out of 50 cases, 19 died and rest 31 cases survived. Conclusions: AgNO 3 test, ECG and pulse oxymetry are bed side, cost effective tools and should be done in all cases. Echocardiography is a useful tool to evaluate cardiac function and cardiac anatomy. Whenever it is available, it must be done in cases of aluminium phosphide poisoning. Serum troponin-I test should be done to assess significant myocardial damage. Thus, proper clinical work out along with relevant investigations and management as per standard protocol may save many more lives.

Abstract: We report the first case of intraocular gnathostomiasis from Central India. A 29-year-old male from Indore, Madhya Pradesh, presented with pain and redness of the right eye since 1 month. Slit lamp examination revealed anterior uveitis, multiple iris atrophic patches, and a live worm hooked on iris. The worm was removed through a small sclerocorneal tunnel. Microscopy confirmed Gnathostoma spinigerum. The patient was treated with oral albendazole and steroids. The case is reported because of its rarity.

Abstract: Background: Trichoscopy is a reliable instrument for diagnosis and for tracking therapy-related changes in female pattern hair loss (FPHL). Videodermoscopic diagnosis of FPHL has been established, which requires fine measurements of hair-related parameters; the method requires an expensive equipment/digital program. Aim: To determine whether a low-cost, simple USB dermoscope can ascertain the hair-related changes in early FPHL. Methods: An age-matched, cross-sectional study was performed over 3 years on subjects with less than 6-month history of hair loss and without an obvious broadening of midline hair parting. Trichoscopic analysis of the frontal and occipital scalp of the study subjects were performed, using a USB-connected dermoscope. The subjects were analyzed for the presence of microscopic hair changes in the form of anisotrichosis, vellus-like hair, single hair follicle unit, peri-pilar sign and yellow dots. Results: A total of 230 cases and 230 controls were analyzed. The dermoscopic hair changes were found to be significantly associated with the frontal scalp zone of cases. Limitations: Histopathological evaluation of the cases was not done. Conclusion: Microscopic changes recorded with the help of a simple USB dermoscope are helpful in establishing a diagnosis of FPHL even in early disease.

Abstract: Background: Amniotic fluid index (AFI) is kind of an estimate of the amount of amniotic fluid. It is an index for the fetal well-being. The aim was to study fetal and maternal out come in cases of low AFI and normal AFI. Methods: This was a case control prospective comparative study performed on 200 randomly selected low risk pregnant patients at term (37-40 weeks of gestation) admitted in Obstetrics and Gynaecology Department. 100 patients with AFI <5 cm (cases) and 100 patients with AFI 8-20 cm (control). Results: Increased number of LSCS in cases that is 35 while only 10 in control and almost equal incidences of MSL and FD in both the groups, while in cases 17 women were planned for elective LSCS for various indications in expectation of better fetal outcome. There was significant low APGAR score in babies of cases, but clinically we refute this. In present study almost double the no. of babies in cases was IUGR or FGR. Significant association between low AFI and congenital anomalies in babies. Most of the anomalies were of urinary tract system. Conclusions: An AFI ≤5 cm detected at term that was at or after 37 completed weeks of gestation in a low risk pregnancy was an indicator of poor perinatal outcome. Oligohydramnios was being detected more frequently now-a-days due to ready availability of ultrasonography these days.

Abstract: Mucosal melanoma of the nasal cavity is a rare tumour. With the extended used of nasal endoscopes and advent of CT scan, early diagnosis of nasal melanoma and accurate assessment of its extent is possible. This allows endoscopic excision of tumour, avoiding unnecessary removal of healthy tissue. Here we present a case where endonasal endoscopic excision with postoperative radiotherapy achieved disease free survival till follow up (nearly 2 years).
