Abstract: The expression of transferrin receptors (TFR) by normal and neoplastic urothelial cells was studied in “control” patients and in patients with transitional cell carcinoma of the bladder. These tumours were graded independently and consisted of 19 grade I, 30 grade II and 19 grade III lesions. TFRs were identified using a monoclonal antibody specific for TFR (OKT9) in an immunofluorescent or avidin/biotin-immunoperoxidase technique on fresh frozen sections. TFRs were not detected on normal urothelium. However, positive staining was found to increase with increasing pathological grade and stage of the tumours, ranging from 31.6% of grade I to 78.9% of grade III tumours and 51.2% of pTa (mucosa only lesions) to 87.5% of pT2/pT2+ (muscle invasion±deeper) primary urothelial malignancies.

Abstract: Collagen/vicryl (Polyglactin) composite membrane has been used to repair full-thickness defects in the urinary bladder of rabbits. The material has been shown to be biodegradable, prevent leakage of urine, and is readily replaced by collagenous scar tissue lined with a urothelium. Regeneration of smooth nuscle has been observed in the repair area of some animals. The results suggest that such a material may well be of use to urologists wishing to augment contracted bladders or in the repair of bladder fistulae in human subjects, thereby avoiding the use of bowel or other material e.g. omentum.

Abstract: Over two decades, experience with estramustine has provided limited data which support an estrogenic mechanism of action and no data which indicate the nitrogen mustard involvement in the cytotoxic properties of the drug. Consideration of the carbamate-ester portion of estramustine supports the pharmacokinetic evidence that estramustine has a long half life since enzymatic hydrolysis of the carbamate is an uncommon event. Using a variety of immunocytochemical and cellular morphology procedures, estramustine per se has been found to express anticytoskeletal properties through non-covalent binding to microtubule associated proteins (MAP's). In both fish erythrophores and in dividing human prostatic carcinoma cells, estramustine exerts an antimicrotubule effect at micromolar concentrations. Thus, estramustine possesses unique pharmacology and protein binding specificity. As such, it should not be classified as an alkylating agent. The estrogenic effects, while possibly of relevance to clinical administration, are not the primary mechanism by which the drug exerts cytotoxicity.

Abstract: We compared the effect of a variety of drug regimens to lower prostate DHT concentration. Patients with BPH were treated for one week prior to surgery with either tamoxifen, flutamide, megestrol acetate, megestrol acetate plus tamoxifen and megestrol acetate plus DES or ketoconazole. DHT concentration in the surgically resected tissue was compared with tissues obtained from untreated patients. We also obtained prostate tissue at the time of relapse in patients with prostate cancer who had been treated with orchiectomy with or without estrogen therapy. Megestrol acetate plus the mini-dose of DES (0.1 mg) and megestrol acetate plus ketoconazole both decreased DHT concentrations in prostate tissue to levels (0.79 ng/g) significantly below those noted with orchiectomy (1.16 ng/g). The difference between the DHT concentration in the two groups (orchiectomy vs. total androgen blockade) represents the contribution of adrenal androgens to prostate tissue DHT. This small amount of DHT (approximately 0.4 ng/g) may be biologically important in stimulating prostate epithelial cell growth.

Abstract: The concentration of Zn, Cd and Se in unseparated tissues and epithelial and stromal fractions of normal prostate gland, BPH and prostatic carcinomas of different histological grading were determined by flameless AAS. There were distinct differences in the content of Zn, Cd and Se in the epithelial and stromal fractions depending on histology. In all cases the concentration of these elements in the epithelial fractions was higher than in stromal fractions. These differences are discussed.

Abstract: To evaluate influence of age on symptomatology and objective parameters in benign prostatic hypertrophy, preoperative findings in 199 unselected patients during one year were analysed. Symptoms of bladder instability increased with age, as did incidence of uninhibited detrusor contractions and bladder trabeculation, whereas maximum urine flow and obstructive complaints decreased, although prostatic size was the same. In the oldest age groups only was increasing prostatic size associated with increasing bladder trabeculation. Age related changes in neurological control and in the structure of the bladder may be involved, and the present results suggest age to be an important factor in the interpretation of symptoms and objective findings in prostatic hypertrophy.

Abstract: This study is intended to elucidate the process of urinary transport through the pelvi-calyceal system (PCS). PCS-motility of a total of 28 PCS was qualitatively and quantitatively determined in 9 stone patients and 14 control persons by examination of contrast medium excretion. From X-ray videograms recorded by means of Siemens urography, four different urinary transport types could be distinguished as well as the duration of the contraction periods and the intervals between the PCS-contractions. In stone patients physiological types of urinary transport are less common (33%) than in control subjects (81.25%). In stone patients the PCS is often characterized by reduced or even absent motility. Calyceal reflux is usually found in the systolic phase of PCS-contractions, which may apparently be regarded as a physiological process.

Abstract: The effects of a new ultrasonic scalpel were studied in laboratory animals. Tissue heat conduction from the tip of the ultrasonic blade was measured. Tissue damage was assessed by light microscopy of histochemically stained sections. The ultrasonic scalpel incised nonfibrous tissue effectively, with minimal heat conduction, and the incisions healed with no evidence of fibrosis nor of tissue destruction.

Abstract: Development of androgen-independent prostatic cancer cells from androgen-responsive cells can occur by a variety of mechanisms (e.g., environmental adaptation, multifocal origin, or genetic instability). Regardless of the mechanism of development, however, once androgen-independent cancer cells become present within prostatic cancer, the tumor is no longer homogeneous but is now heterogeneous. Once a prostatic cancer is heterogeneously composed of both androgen-dependent and-independent cancer cells, androgen withdrawal therapy, no matter how complete, cannot be curative. In order to produce cures of such heterogeneous prostatic cancers, hormonal therapy must be combined simultaneously with chemotherapy early in the course of the disease so that all the cancer populations (i.e., androgen-dependent and-independent) can be simultaneously affected within an individual patient.

Abstract: A Vicryl (Polyglactin)/Collagen composite membrane has been developed for potential use in urinary tract surgery. The compatability of this membrane, together with its two individual components, collagen film and vicryl mesh, has been tested over a three week period in urine obtained from both healthy and from stone forming patients. The rate of degradation as indicated by changes in the mechanical strength was determined at regular intervals. In addition, absorbable suture materials such as plain catgut, chromic catgut, and vicryl were similarly tested; in the latter case, the breaking stress, and actual loss of weight of the material were compared. The whole series of experiments were then repeated in urine obtained from rabbits, the animal chosen for any future in vivo studies.

Abstract: The effects of prostate fibroblast conditioned medium on two prostate epithelial cell lines (PC-3, LNCaP) and on two non-prostatic cell lines (MCF-7, K562) was investigated. As prostate fibroblast conditioned medium exerts its main effect on DNA synthesis, 3H thymidine incorporation was monitored to measure factor activity. Conditioned media of all prostatic fibroblast lines investigated were inhibitory for PC-3, LNCaP and MCF-7. Conditioned medium of prostatic fibroblasts was clearly stimulatory for K562. Prostate specificity of production of PEIF was demonstrated by the fact that conditioned medium from skin fibroblasts proved to be stimulatory for PC-3. Inhibitory activity from conditioned medium as well as from a BPH homogenate was precipitated by 33–67% ammonium sulfate. These partly purified fractions were respectively five and ten times as active as “crude” conditioned medium. The physical nature of PEIF (protein or macroglycolipid) as well as the possible function (as a signal messenger between stroma and epithelium) is discussed.

Abstract: Innervation of the external urethral sphincter (EUS) was studied in male human subjects. In the region of EUS at the distal end of prostatic urethra, a large axon bundle surrounded by perineurium was evident in the intramural connective tissue gap. Because of the presence of dense core vesicles, the small nonmyelinated axon profiles in the bundle were considered to be adrenergic. After ramifications to smooth musculature, the axons were traced to the EUS. In the EUS, axon bundles containing many nonmyelinated axons were recognized as a sole autonomic nerve among the striated muscle cells. A single or at most two or three axons were surrounded by a Schwann cell, and some possessed dense core vesicles which suggested an adrenergic function. These autonomic adrenergic nerve ends formed surface junctions with the striated muscle of EUS. The clinical relevance of these data are discussed.

Abstract: Human prostatic tissue and expressed prostatic secretions (EPS) from patients with benign prostatic hyperplasia (BPH) and prostate cancer were incubated with (1 beta 3H) androstenedione. The extent of aromatization was determined by measuring the transfer of 3H from the 1 beta position into water. The amount of 3H2O recovered corresponds to the estrogens formed. Tissue from 5 patients with BPH yielded 2.13 (+/- 1.05) pmol/mg protein/h while the EPS from the same patients yielded 727 fmol/mg protein/h. In patients with prostate cancer the mean formation of estrogens was 388 fmol/mg protein/h (+/- 75). 4-hydroxy-androstenedione, an aromatase inhibitor, successfully inhibited aromatization in BPH and prostate cancer 53-98%.

Abstract: A composite membrane produced from collagen and vicryl mesh has been used to cover the kidney surface following partial nephrectomy in rabbits. The membrane readily held sutures, gave satisfactory haemostasis, and prevented leakage of urine. The experiments showed that the prosthesis biodegraded in less than twenty weeks. The only observed long-term effect of the material was thickening of the renal capsule. The results indicate that this membrane may be a suitable reparative materal for use in traumatised kidneys in humans.

Abstract: Male Copenhagen x Fischer F1 rats were implanted with Dunning R3327H prostatic carcinoma, castrated when the tumours became palpable and were then treated with testosterone, testosterone in combination with oestradiol or oestradiol alone for four weeks. Treatment with oestradiol produced the smallest tumours. The testosterone-stimulated growth of tumours was inhibited by oestradiol. The adenocarcinoma was moderately to well-differentiated. Morphometric analysis of the composition of the tumours showed that oestradiol stimulated tumour stroma and inhibited glandular epithelium. These effects were produced concomitantly with decreased overall tumour growth. Testosterone stimulated all cell types of the tumour.

Abstract: Trypsinization (WT) was employed to disaggregate urothelial cells from normal human urinary bladder mucosa for the preparation of primary cultures. Urinary bladders of two male adults both 25 years old were obtained autopsy 1-2 h after death. The mucosa was incubated in HBSS containing 0.25% trypsin at 37 degrees C. Mean cell yield, viability, and attachment were 14.6 X 10(7), 76%, and 42.5% after WT. In histologic sections of treated mucosa, most of the urothelium was removed by WT. Following plating and attachment, cells obtained by WT formed a monolayer of flattened epithelial-like cells. When stained with polyclonal antikeratin antibody using the indirect immunoperoxidase technique, all of the cells were immunoreactive indicating an epithelial origin. In conclusion, based on morphology and immunocytochemistry, WT removed virtually all of the urothelial cells from the mucosa and no contamination by mesenchymal cells resulted from this procedure. Thus, WT is an appropriate technique for the isolation of urothelial cells and initiation of primary cultures of normal human urothelial cells for subsequent study.

Abstract: In elderly males hormonal changes occur, that are believed to cause benign prostatic hyperplasia (BPH). These are decreased testosterone production, an increased testosterone SHBG and a slightly increased estradiol production. Liver cirrhosis in males causes similar endocrine changes. We carried out a post mortem study evaluating the prostates of 51 men who died with liver cirrhosis compared with a similar group without any hepatic disease. The occurrence of BPH in cirrhotic subjects was diminished and delayed compared to total population. Furthermore in cirrhotic men BPH is more common as early nodular hyperplasia (early stage) or stromal hyperplasia (suggesting estrogenic prevalence), while in the general population stromal and epithelial hyperplasia (androgenic stimulation), were almost equally present.

Abstract: Plasminogen activator was recovered from bladder tumors by 30% ammonium sulfate precipitation, acid treatment and concanavalin A-Sepharose affinity chromatography to a purification factor of about 80,000. The pooled fraction from the binding protein to concanavalin A-Sepharose revealed a single enzymatically active band with molecular weight of 55,000, which lost its enzymatic activity in the absence of plasminogen. The enzymatic activity was inactivated by DFP. The purified plasminogen activator reacted with antibody against UK, and not with that against t-PA. The purified plasminogen activator cleaved S-2288 to a greater extent than S-2444, although UK cleaved S-2444 to a greater extent that S-2288. The enzymatic activity was strongly inhibited by basic pancreatic trypsin inhibitor, and benzamidine. These results suggest that the plasminogen activator in bladder tumors may belong to a different category of plasminogen activator.

Abstract: Chemotherapeutic agents were evaluated for effect on the development of urinary bladder tumors induced by N-butyl-N-(4-hydroxybutyl) nitrosamine (BBN) in male Wistar strain rats. Seven hundred and two rats were given 0.05% BBN in drinking water for 8 weeks. After BBN treatment, the animals were divided into 26 groups to follow regimens of single chemotherapy. All drugs were administered intraperitoneally except in one group that was treated orally. In our experimental series, 5-fluorouracil (5-FU), N-(2-tetrahydrofuryl)-5-fluorouracil (FT-207), carbazilquinone (CQ), vincristine (VCR) and cis-diamminedichloroplatinum (CDDP) were effective in inhibiting the incidence of bladder tumor, however, adriamycin (ADM), mitomycin C (MMC), neocarsinostatin (NCS), cyclophosphamide (CPM) and bleomycin (BLM) were not effective.

Abstract: The distribution of tissue polypeptide antigen (TPA), epithelial membrane antigen (EMA) and various cytokeratins was studied with immunohistochemical techniques in malignant, normal and inflamed urothelium. TPA and cytokeratins 8, 18 and 19 were similarly distributed and there was no difference between benign and malignant conditions. All keratins were most abundant in the outer cell layers of the urothelium. EMA was present primarily in the so called umbrella cells. Increased levels of TPA in urine and serum are found in patients with urothelial carcinoma. TPA is reported to be related to the cytoskeletal keratins and its presence definitely does not imply malignant transformation as we have demonstrated it in both benign and malignant urothelium. The increased levels of TPA in urine and serum are probably due to a more rapid turnover and autolysis of cells and may parallel a more aggressive tumor behaviour.

Abstract: Peripheral blood T lymphocyte subsets were quantitated by monoclonal antibodies in 91 patients with urologic malignancies as well as eight age-matched controls. In general, the presence of cancer and increasing stage were associated with a relative decrease in helper and an increase in suppressor T-cell activity as well as a decreased T4/T8 cell ratio. Therapy in the form of androgen depletion reversed these trends and restored helper and suppressor levels as well as the T4/T8 ratio to control values. In a small subset of sixteen patients surgery had only modest immediate (prostate) and long term (kidney) effects on these parameters.

Abstract: The calcium-binding kinetics of chondroitin sulphate C (CS) have been determined using equilibrium analysis including 45Ca. There is a linear relationship between the extent of the Ca binding and the concentration of CS present. 1 μmol CS disaccharide unit binds 0.757 μmol Ca. Scatchard plots of the data have revealed a single constant of dissociation (K D =1429). In the presence of urate ions, and dependent on the pH value, the ability of CS to bind Ca may be impaired by as much as 31%. These measurements have supported the theory that urate ions interact with the GAGs in urine.

Abstract: Microscopic crystals were found by SEM analysis in kidneys of stone forming patients as well as in kidneys of non-stone formers. In most samples calcifications were intratubular in the collecting ducts near the papillary tip. 1–2 μm sized particles lay in a gelatinous substance on the epithelium. The only difference in crystal composition is the higher phosphate content in the kidneys of the nonstone forming group. A correlation between the element concentration of the tissue and the occurrence of microliths is given by the cadmium content of the kidney samples. Both groups showed higher cadmium values in the cortex or medulla when crystals were found in the corresponding papillary samples. Urine supersaturation with respect to calcium oxalate was higher in the stone forming group caused by a higher calcium and oxalate but a lower magnesium excretion.

Abstract: Non-neoplastic prostatic epithelium from 39 patients obtained at transurethral resection for outflow tract obstruction and 5 normal prostates from men under 35 years of age obtained at postmortem were formalin-fixed and paraffin-embedded. The distribution of 8 lectin receptors were studied using a peroxidase anti-peroxidase method and an avidin-biotin method. Con A, WGA, and PNA bound to most epithelial cells. Con A and WGA also showed major stromal binding. Approximately 5% to 10% of cells bound UEA1, GS1, DBA, SBA and BPA. No major differences in lectin receptor expression were observed between normal and hyperplastic epithelium with either of the immunohistochemical techniques except that hyperplastic cells stained more strongly than normal epithelium.

Abstract: Variations of urinary pH and concentrations of calcium, phosphate, oxalate, magnesium and citrate have been produced by 4 different diets given to 19 idiopathic calcium stone formers. The state of saturation towards whewellite and brushite was directly measured in the 76 urine samples by equilibration with the corresponding salts and was compared to chemical constituents by regression analyses. The state of saturation towards calcium oxalate monohydrate was significantly governed only by the urinary oxalate concentration, and a soluble oxalate fraction not contributing to calcium oxalate chelation was demonstrated. The state of saturation towards brushite was exclusively determined by urinary calcium and pH, the latter below 5.5 showing a high influence on brushite solubility.

Abstract: Using a colony-forming technique and 2 human bladder cancer cell lines, T24 and KK-47, the enhanced radioinduced cytotoxicity in combination with 43 degrees C hyperthermia (HPT) or 4 anticancer drugs; bleomycin (BLM), cis-dichlorodiammineplatinum (II) (CDDP), mitomycin C (MMC), carbazilquinone (CQ), has been studied. In the series of both cell lines, the combination of 43 degrees C hyperthermia and irradiation resulted in exceedingly enhanced cytotoxicity. This was characterized by a marked decline of the slope of the radiation dose-survival curve as compared with slope in the combination of each of the anticancer drugs and irradiation. Among the 4 anticancer drugs, BLM was thought to be the most promising agent as a potentiator of the radiotherapy, on the basis of a remarkable decrease in the shoulder portion of the radiation dose-survival curve. The other 3 anticancer drugs showed a certain degree of potentiation of radiosensitivity.

Abstract: Experimental testicular torsion must be used for certain studies of clinical relevance, but most experimental models either do not provide hemodynamic alteration comparable to the clinical situation or cannot guarantee reproducibility. Using a rat model arterial perfusion and hemorrhagic infarction were correlated to the degree of both intra- and extravaginal testicular torsion. Arterial inflow was measured sequentially with radiolabelled microspheres, hemorrhagic infarction was judged by the increase of testicular weight. Maximal hemorrhagic infarction and reproducible values were found when the spermatic cord was twisted togehter with the tunica vaginalis for 360°–540°.

Abstract: Male genial blood flow and its regulation have not been fully clarified. In the present study we employed a radioactive microsphere technique to estimate the blood flow simultaneously in the major urogenital organs of male rats. The recorded genital flow rates varied among the rats. The relative blood perfusion rates of the testicles, seminal vesicles and kidneys did not differ from one animal to another. However, the blood flow in the prostate gland was not correlated to the perfusion of any of the other investigated urogenital organs.

Abstract: Kidneys were perfused either with Euro-Collinssolution or with HTK-solution of Bretschneider. The perfusion pressure as well as the perfusion flow were measured during a six-minute perfusion. The perfusion resistance was higher in Euro-Collins-kidneys than during HTK-perfusion. The venous outflow of the kidney as well as the ureteral outflow was measured during each minute of the perfusion and has analysed for osmolality, and for sodium and potassium concentrations. In Euro-Collins-kidneys a complete “equilibration” of the extracellular space was not achieved, while during HTK-perfusion concentrations in the venous as in the tubular outflow, similar to those in the HTK-solution itself, could be reached. At the end of the different perfusions, tissue was analysed for biochemical parameters such as ATP, ADP, AMP and lactate as well as for morphological features. Lactate had increased and ATP had decreased during perfusion with Euro-Collins-solution, while ATP had not changed and lactate had decreased during perfusion with HTK-solution. Normal glomerular, tubular and dilated vascular structures can be seen after HTK-perfusion, while a glomerular and vascular contraction takes place during Euro-Collins-perfusion.

Abstract: Following a morphological analysis of pelvi-calyceal systems (PCS) of healthy persons and urolithiasis patients, rheological studies from glass and rubber PCS-models are presented. In the PCS of urolithiasis patients morphological and geometrical differences were found, causing a flow pattern which favours calculus formation. In urolithiasis, the PCS is characterized by cavities with significantly reduced flow rates and stagnation of urine. The formation of urinary calculi is often promoted by a hypomotility of the muscles of the PCS, common in urolithiasis patients. Possible therapeutic and diagnostic consequences are discussed.
