So far in this course we have dealt entirely with the evolution of characters that are controlled by simple Mendelian inheritance at a single locus. There are notes on the course website about gametic disequilibrium and how allele frequencies change at two loci simultaneously, but we didn’t discuss them. In every example we’ve considered we’ve imagined that we could understand something about evolution by examining the evolution of a single gene. That’s the domain of classical population genetics. For the next few weeks we’re going to be exploring a field that’s actually older than classical population genetics, although the approach we’ll be taking to it involves the use of population genetic machinery. If you know a little about the history of evolutionary biology, you may know that after the rediscovery of Mendel’s work in 1900 there was a heated debate between the “biometricians” (e.g., Galton and Pearson) and the “Mendelians” (e.g., de Vries, Correns, Bateson, and Morgan). ESC Guidelines for the Management of Acute Myocardial Infarction in Patients Presenting With ST-Segment Elevation

Background Classic Fabry disease (FD) is caused by GLA mutations that result in enzymatic deficiency of alpha-galactosidase A (AGAL), lysosomal storage of globotriaosylceramide, and a resulting multisystemic disease. In non-classic later-onset FD, patients have some preserved AGAL activity and a milder disease course, though female carriers may also be affected. While FD pathogenesis has been mostly attributed to catalytic deficiency of mutated AGAL, lysosomal storage and impairment of lysosomal functions, other pathogenic factors may be important, especially in non-classic later-onset FD. Methods We characterized the clinical, biochemical, genetic, molecular, cellular and organ pathology correlates of the p.L394P AGAL variant that was identified in six individuals with end-stage kidney disease by the Czech national screening program for FD and by further screening of 25 family members. Results Clinical findings revealed a milder clinical course with ~15% residual AGAL activity. Laboratory investigations documented intracellular retention of mutated AGAL with resulting ER stress and the unfolded protein response (UPR). Kidney biopsies did not show lysosomal storage. We observed similar findings of ER stress and UPR with several other classic and non-classic FD missense GLA variants. Conclusions We identified defective proteostasis of mutated AGAL resulting in chronic ER stress and UPR of AGAL expressing cells (hereafter referred to as AGALopathy) as an important contributor to FD pathogenesis. These findings provide insight into non-classic later-onset FD and may better explain clinical manifestations with implications for pathogenesis, clinical characterization and treatment of all FD forms. Significance statement Catalytic deficiency of mutated AGAL is responsible for classicFabry disease (FD) pathogenesis but does not fully explain the findings in non-classic later-onset FD, in which affected individuals and female carriers develop clinical manifestations despite some AGAL activity and variably mitigated lysosomal storage. In this investigation of individuals with the p.L394P AGAL variant, we identified defective proteostasis of mutated AGAL resulting in chronic endoplasmic reticulum stress and the unfolded protein response as significant contributors to pathogenesis of non-classic later-onset FD. Similar effects were documented also in other AGAL variants identified in classic and non-classicFD. Endoplasmic reticulum stress and the unfolded protein response therefore play an important role in FD.

Plicni embolie je relativně casto se vyskytujici onemocněni s nezanedbatelnou mortalitou, nepravem podceňovane, ktere je ale při 
vcasne diagnoze dobře lecitelne, a je-li adekvatni lecba zahajena vcas, býva dobře ucinna. Zavažnost plicni embolie je urcena předevsim 
jejim hemodynamickým dopadem na funkci prave komory, a tedy zachovanim dostatecneho dopředneho výdeje a organových funkci. 
Nicmeně, samotna „zavažnost“ tohoto onemocněni může být ovlivněna i potencialem k rozvoji dalsich komplikaci, jako je recidiva 
embolie, embolizace trombů do plicniho nebo systemove řecistě, rozvoj respiracni insuficience a výskyt zavažných komplikaci použite 
lecby, předevsim krvacivých.

Podrobne studie a sledovani nemocných s předcasným výskytem akutniho infarktu myokardu (AIM) jsou větsinou kazuisticka nebo 
zahrnuji jen male pocty pacientů sledovaných v ramci jednotlivých center. Mladsi nemocni s AIM, kteři větsinou bývaji definovani 
jako nemocni, kteři prodělali svůj prvni AIM u mužů do věku 45 a u žen do 55 let, reprezentuji pouze 2 až 6 % vsech nemocných 
s AIM. Tato skupina mladsich nemocných je přitom v řadě aspektů odlisna od nemocných, kteři prodělali svůj prvni infarkt myokardu 
až ve věku nad 60 let. V tomto sděleni jsme se soustředili na popis přitomných rizikových faktorů a zajimavých nalezů, ktere provazely 
anebo se etiologicky podilely na vzniku infarktu myokardu u mladých nemocných. Na casne manifestaci ICHS se nemusi podilet jen 
zname a běžně zjisťovane rizikove faktory, ale i jine zakladni choroby. Mladi nemocni s AIM totiž tvoři poměrně specifickou skupinu 
pacientů s ICHS.

Akutní infarkt myokardu u mladých nemocných - výsledky pilotního sledování

Kouřeni patři k uznavaným rizikovým faktorům aterosklerozy, jeho aktivni i pasivni formy se podileji na iniciaci a progresi aterosklerozy i na rozvoji nestabilnich forem - akutnich koronarnich syndromů. Jak vyplýva z ceských Doporuceni pro lecbu zavislosti na tabaku, je kouřeni v Evropě i v Ceske republice přicinou každeho pateho umrti, což u nas představuje každý rok přibližně 18 000 umrti. Vliv kouřeni na rozvoj kardiovaskularnich onemocněni je zprostředkovan řadou mechanismů. Uplatňuji se geneticka predispozice, změny vasomotorických funkci endotelu, strukturalni poskozeni endotelu, změny poddajnosti cevni stěny, změny spektra krevnich lipidů, změny funkce krevnich desticek a trombofilie, působeni volných kyslikových radikalů, sniženi koncentraci antioxidantů, přitomnost zanětlivých změn a infekce v cevni stěně. V přehledu jsou uvedeny zakladni poznatky o vlivu kouřeni na rozvoj aterosklerozy a jejich komplikaci, uvedených změn. Zminěna je take zavažnost kouřeni v populaci děti s přihlednutim k tomu, že z patnactiletých děti v Ceske republice kouři 25 % divek a 23 % chlapců (druhe a ctvrte misto na světě, data z roku 2009). Ceska kardiologicka spolecnost se proto teto problematice v nasledujicich letech bude věnovat systematicky a ve větsim rozsahu, než tomu bylo dosud.
