Targeted nucleases are powerful tools for mediating genome alteration with high precision. The RNA-guided Cas9 nuclease from the microbial clustered regularly interspaced short palindromic repeats (CRISPR) adaptive immune system can be used to facilitate efficient genome engineering in eukaryotic cells by simply specifying a 20-nt targeting sequence within its guide RNA. Here we describe a set of tools for Cas9-mediated genome editing via nonhomologous end joining (NHEJ) or homology-directed repair (HDR) in mammalian cells, as well as generation of modified cell lines for downstream functional studies. To minimize off-target cleavage, we further describe a double-nicking strategy using the Cas9 nickase mutant with paired guide RNAs. This protocol provides experimentally derived guidelines for the selection of target sites, evaluation of cleavage efficiency and analysis of off-target activity. Beginning with target design, gene modifications can be achieved within as little as 1-2 weeks, and modified clonal cell lines can be derived within 2-3 weeks.

Genome engineering using the CRISPR-Cas9 system

The advent of facile genome engineering using the bacterial RNA-guided CRISPR-Cas9 system in animals and plants is transforming biology. We review the history of CRISPR (clustered regularly interspaced palindromic repeat) biology from its initial discovery through the elucidation of the CRISPR-Cas9 enzyme mechanism, which has set the stage for remarkable developments using this technology to modify, regulate, or mark genomic loci in a wide variety of cells and organisms from all three domains of life. These results highlight a new era in which genomic manipulation is no longer a bottleneck to experiments, paving the way toward fundamental discoveries in biology, with applications in all branches of biotechnology, as well as strategies for human therapeutics.

Objectives : Thus this study attempted to look into the level of community residents' dental health and the state of their dental diseases and estimate the general dental health condition so that these data can be available as references in upcoming public dental health planning, and to provide fundamental data for promotion of the level of residents' dental health by performing dental health education. Methods : Among community residents who visited the department of Dental Hygiene of Shinsung University for scaling from March to May 2009, the data of total 346 subjects of 129 women and 217 men were selected, and DMFT rate, DMFT index, dental health capacity of the first permanent molar, and tooth morality rate were investigated. For statistical analysis, SPSS 14.0 was used, for general characteristics of the subjects, frequency analysis was conducted, for caries experience in the permanent teeth of the subjects, mean and standard deviation were calculated, for caries experience in the permanent teeth by sex and residence, independent t-test was performed, and for caries experience in the permanent teeth by age, one-way ANOVA was conducted. The significance level applied to these analyses was 0.05. Results : As a result of investigation and analysis on caries in the permanent teeth of community residents who visited the department for scaling in this study, the findings are as follows: 1. For sex, DMFT rate and tooth morality rate were found to be higher in women, while dental health capacity of the first permanent molar was found to be higher in men. 2. For age, DMFT rate and DMFT index were found to be highest in 40~49 years old, while DMFT rate, DMFT index, and tooth morality rate were found to be lowest in under 20 years old. Dental health capacity of the first permanent molar was found to be highest in under 20 years old, 20-29 years old, and 30-39 years old and found to be lowest in more than their sixties as 39.78 points(p

A study on caries experience in the permanent teeth of community residents

Purpose: Microsurgery in pediatric patients remains challenging because of technical difficulties in small-vessel anastomosis and flap dissection. Few reports have focused exclusively on the microsurgical reconstruction of traumatic hand defects in children. Herein, we share our experience of posttraumatic hand reconstruction with free tissue transfer in pediatric patients and compare the treatment outcomes with adults.Methods: A single-institution retrospective review of trauma-induced microsurgical hand reconstruction cases was performed. Pediatric patients below 17 years old and adults who underwent microsurgical reconstruction of traumatic hand defects between 2011 and 2021 were included. Patient demographics, flap type, use of vein grafts, operative time, flap survival, and postoperative complications were documented. A subgroup analysis of patients younger than 8 years was also performed. Data of pediatric patients were statistically compared with those of adults who underwent free flap surgery using nearly identical surgical procedures by four senior surgeons at our medical center.Results: Forty-one flaps in 39 pediatric patients and 184 flaps in 184 adult patients were analyzed. Fasciocutaneous flaps were predominantly used in both groups. In pediatric patients, all flaps survived, while 170 adults (92.4%) survived. No statistically significant between-group differences in treatment outcomes were found. However, pediatric patients (22.0%) had significantly fewer secondary operations than adults (67.4%, p<0.001).Conclusion: Microsurgical reconstruction for trauma-induced hand defects in pediatric patients has a high success rate and low complication rate, just as with adults. Pediatric patients may be more resistant to partial necrotic flap changes, thereby requiring fewer secondary operations than adults.

Microsurgical reconstruction for traumatic hand defects in pediatric patients

Atypical hemolytic uremic syndrome (aHUS) is a form of thrombotic microangiopathy (TMA) that can result in end-stage renal disease. Patients with aHUS often have predisposing dysfunction in the complement pathway, and continuous activation of complement proteins can be triggered after transplantation. Here, we report the first successful case of aHUS treatment in a kidney transplant recipient with early use of a C5 inhibitor, eculizumab, in South Korea. The patient was a 32-year-old man, and the donor was his 60-year-old mother. The graft showed immediate good function. On postoperative day (POD) 3, the clinical diagnosis of TMA was made. Persistent renal dysfunction despite 10 plasma exchange (PE) sessions prompted eculizumab treatment on POD 18 under suspicion of aHUS. Next-generation sequencing reported gene mutations classified as variants of unknown significance in coagulation-associated genes. The patient was discharged after three doses of eculizumab with serum creatinine of 1.82 mg/dL. In total, 16 doses of eculizumab were administered. At the last follow-up, 21 months after eculizumab discontinuation, the graft was well functioning. De novo TMA after kidney transplantation can be caused by sustained activation of the complement pathway, and early eculizumab treatment appears important in the successful treatment of aHUS refractory to PE.

The first successful eculizumab rescue therapy of a kidney transplant recipient with atypical hemolytic uremic syndrome in South Korea: a case report

Total degloving amputations present surgeons with unique challenges to preserve function and prevent amputation of the digits. Obviously, the replantation of avulsed skin is the best option; however, there are situations in which vascular networks are severely contused, making it difficult to restore the circulation, due to the crushing mechanism of injury. As an alternative, defatted or meshed skin grafts from the avulsed skin have been proposed. However, those options have not been proven to guarantee viable digits. Moreover, the application of thinned skin grafts usually results in cosmetically and functionally unsatisfactory outcomes due to the lack of sufficient tissue volume. We present a case in which we successfully saved a large volume of the amputated stump with minimal digit amputation by performing a composite graft of the untailored (i.e., without defatting or meshing) avulsed skin on a totally degloved hand.
