Acute kidney injury (AKI) is a complex disorder for which currently there is no accepted definition. Having a uniform standard for diagnosing and classifying AKI would enhance our ability to manage these patients. Future clinical and translational research in AKI will require collaborative networks of investigators drawn from various disciplines, dissemination of information via multidisciplinary joint conferences and publications, and improved translation of knowledge from pre-clinical research. We describe an initiative to develop uniform standards for defining and classifying AKI and to establish a forum for multidisciplinary interaction to improve care for patients with or at risk for AKI. Members representing key societies in critical care and nephrology along with additional experts in adult and pediatric AKI participated in a two day conference in Amsterdam, The Netherlands, in September 2005 and were assigned to one of three workgroups. Each group's discussions formed the basis for draft recommendations that were later refined and improved during discussion with the larger group. Dissenting opinions were also noted. The final draft recommendations were circulated to all participants and subsequently agreed upon as the consensus recommendations for this report. Participating societies endorsed the recommendations and agreed to help disseminate the results. The term AKI is proposed to represent the entire spectrum of acute renal failure. Diagnostic criteria for AKI are proposed based on acute alterations in serum creatinine or urine output. A staging system for AKI which reflects quantitative changes in serum creatinine and urine output has been developed. We describe the formation of a multidisciplinary collaborative network focused on AKI. We have proposed uniform standards for diagnosing and classifying AKI which will need to be validated in future studies. The Acute Kidney Injury Network offers a mechanism for proceeding with efforts to improve patient outcomes.

Acute Kidney Injury Network: report of an initiative to improve outcomes in acute kidney injury

Summary Background Since December, 2019, Wuhan, China, has experienced an outbreak of coronavirus disease 2019 (COVID-19), caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Epidemiological and clinical characteristics of patients with COVID-19 have been reported but risk factors for mortality and a detailed clinical course of illness, including viral shedding, have not been well described. Methods In this retrospective, multicentre cohort study, we included all adult inpatients (≥18 years old) with laboratory-confirmed COVID-19 from Jinyintan Hospital and Wuhan Pulmonary Hospital (Wuhan, China) who had been discharged or had died by Jan 31, 2020. Demographic, clinical, treatment, and laboratory data, including serial samples for viral RNA detection, were extracted from electronic medical records and compared between survivors and non-survivors. We used univariable and multivariable logistic regression methods to explore the risk factors associated with in-hospital death. Findings 191 patients (135 from Jinyintan Hospital and 56 from Wuhan Pulmonary Hospital) were included in this study, of whom 137 were discharged and 54 died in hospital. 91 (48%) patients had a comorbidity, with hypertension being the most common (58 [30%] patients), followed by diabetes (36 [19%] patients) and coronary heart disease (15 [8%] patients). Multivariable regression showed increasing odds of in-hospital death associated with older age (odds ratio 1·10, 95% CI 1·03–1·17, per year increase; p=0·0043), higher Sequential Organ Failure Assessment (SOFA) score (5·65, 2·61–12·23; p Interpretation The potential risk factors of older age, high SOFA score, and d-dimer greater than 1 μg/mL could help clinicians to identify patients with poor prognosis at an early stage. Prolonged viral shedding provides the rationale for a strategy of isolation of infected patients and optimal antiviral interventions in the future. The invention relates to a method for constructing a blood vessel, in particular to an injection type vascularized adipose tissue and a construction method thereof. In the invention, autologous cells are used for simulating a natural vascular tissue, and a natural biological material is used for packaging tissue cells to form microballoons; smooth muscle cells and endothelial cells are compoundedto form a blood vessel structure by utilizing gaps of the microballoons as blood vessel forming channels to obtain an artificial tissue with a blood vessel structure; by simulating the natural vascular tissue, adipose stem cells are packaged into the natural material to prepare the microballoons; and a vascularized factor slow release system is combined to construct the injection type vascularized adipose tissue. According to the invention, the injection type vascularized adipose tissue can be constructed in vitro or in vivo and used for repairing small and large soft tissues, can stably exist in the human body for a long time, has small operational injury and quick postoperative recovery and also can be used for an in vitro biochemically or pharmacally researched model. The method for constructing the blood vessel and the artificial tissue also can be used for other artificial tissues, such as liver tissues, cartilaginous tissues, and the like.

Objective: To investigate the effects of stellate ganglion block (SGB) on the peri-operative vasomotor cytokine content and intrapulmonary shunt in patients with esophagus cancer who underwent thoracotomy. Materials and Methods: Forty patients undergoing elective resection of esophageal cancer patients who had Ⅰ~Ⅱ American Society of Anesthesiologist (ASA) were randomly divided into total intravenous anesthesia group (group N, n=20) and total intravenous anesthesia combined with SGB group (group S, n=20, 0.12 mL/kg 1% lidocaine was used for SGB 10 min before induction). Heart rate (HR), mean arterial pressure (MAP), central venous pressure (CVP), mean pulmonary arterial pressure (MPAP) and continuous cardiac output (CCO) were continuously monitored. The blood from internal jugular vein was drawn respectively before induction (T 0 ), and 30 min (T 1 ), 60 min (T 2 ) and 120 min (T 3 ) after one-lung ventilation (OLV), and 30 min (T4) after two-lung ventilation. The contents of plasma endothelin (ET), nitric oxide (NO) and calcitonin gene-related peptide (CGRP) were detected with enzyme linked immunosorbent assay (ELISA). Meanwhile, arterial and mixed venous blood samples were collected for determination of blood gas and calculation of intrapulmonary shunt fraction (Qs/Qt). Results: During OLV, ET contents were increased significantly in two groups ( P 0.05). NO content in group S was obviously higher than in group N at T3 (P<0.05), whereas CGRP content in group N was markedly lower than in group S at each time point (P<0.05). Qs/Qt was significantly increased in both groups after OLV, but there was no statistical significant regarding the Qs/Qt at each time point between two groups. Conclusions: Total intravenous anesthesia combined with SGB is conducive to regulation of perioperative vasomotor cytokines in thoracotomy, and has little effect on intrapulmonary shunt at the time of OLV.

Effects of stellate ganglion block on the peri-operative vasomotor cytokine content and intrapulmonary shunt in patients with esophagus cancer.

This paper presents a quantitative comparison of the flow fields of a radial turbine between real gas and perfect gas models for the internal combustion engine (ICE) organic Rankine Cycle (ORC) application. Three-dimensional turbulent Navier-Stokes simulations are carried out using CFD code NUMECA FINE TM /TURBO, which is linked to an accurate thermodynamic model for organic working fluid R123 in the form of thermodynamic tables. Four turbine operating conditions including the design point and three part-load points, the inlet compressibility factors of which are 0.82-0.89, are analyzed to discuss the differences of flow fields. Obvious derivations of thermodynamic parameters are investigated in the turbine flow fields. The derivations of speed of sound and density at the nozzle inlet are about 15-20%. There exist about 10m/s value differences in the nozzle outlet velocity evaluation, and furthermore a difference of 10 degrees in the rotor inlet incidence angle comparison. The derivations of relative Mach number are about 20-35% in the rotor outlet near the shroud surface. More than 30% differences are shown in the comparison of turbine total temperature drops. Other thermodynamic parameters show much smaller derivations. The differences of thermodynamic parameters lead to a 1-3% larger in mas flow rate, 1-2% larger in isentropic efficiency and 6-8% smaller in specific power comparison. However, there do not exist obvious differences on thermodynamic parameters distributions in the flow fields. The similar flow fields provide a suggestion that perfect gas model may be an acceptable model for turbine preliminary design and one-dimensional analysis in this gas thermodynamic region, and also the real gas flow fields simulated can be used as a start point to refine the turbine design.
