Journal ArticleDOI
Acute-on-chronic liver failure is a distinct syndrome that develops in patients with acute decompensation of cirrhosis
Richard Moreau,Rajiv Jalan,Pere Ginès,Marco Pavesi,Paolo Angeli,Juan Córdoba,François Durand,Thierry Gustot,Faouzi Saliba,Marco Domenicali,Alexander L. Gerbes,Julia Wendon,Carlo Alessandria,Wim Laleman,Stefan Zeuzem,Jonel Trebicka,Mauro Bernardi,Vicente Arroyo +17 more
TL;DR: Diagnostic criteria for ACLF was established and showed that it is distinct from AD, based not only on the presence of organ failure(s) and high mortality rate but also on age, precipitating events, and systemic inflammation.
Journal ArticleDOI
Development and validation of a prognostic score to predict mortality in patients with acute-on-chronic liver failure
Rajiv Jalan,Faouzi Saliba,Marco Pavesi,Alex Amoros,Richard Moreau,Pere Ginès,Eric Levesque,François Durand,Paolo Angeli,Paolo Caraceni,Corinna Hopf,Carlo Alessandria,Ezequiel Rodríguez,Pablo Solís-Muñoz,Wim Laleman,Jonel Trebicka,Stefan Zeuzem,Thierry Gustot,Rajeshwar P. Mookerjee,L. Elkrief,Germán Soriano,J. Córdoba,Filippo Morando,Alexander L. Gerbes,Banwari Agarwal,Didier Samuel,Mauro Bernardi,Vicente Arroyo +27 more
TL;DR: The CLIF-C ACLFs is a clinically relevant, validated scoring system that can be used sequentially to stratify the risk of mortality in ACLF patients and is superior to the MELDs and MELD-Nas in predicting mortality.
Journal ArticleDOI
Clinical Course of acute-on-chronic liver failure syndrome and effects on prognosis
Thierry Gustot,Javier Fernández,Elisabet Garcia,Filippo Morando,Paolo Caraceni,Carlo Alessandria,Wim Laleman,Jonel Trebicka,Laure Elkrief,Corinna Hopf,Pablo Solís-Muñoz,Faouzi Saliba,Stefan Zeuzem,A. Albillos,Daniel Benten,José Luis Montero-Álvarez,Maria Teresa Chivas,Mar Concepción,Juan Córdoba,A. McCormick,Rudolf E. Stauber,Wolfgang Vogel,Andrea De Gottardi,Tania M. Welzel,Marco Domenicali,A. Risso,Julia Wendon,Carme Deulofeu,Paolo Angeli,François Durand,Marco Pavesi,Alexander L. Gerbes,Rajiv Jalan,Richard Moreau,Pere Ginès,Mauro Bernardi,Vicente Arroyo +36 more
TL;DR: Assessment of ACLF patients at 3‐7 days of the syndrome provides a tool to define the emergency of LT and a rational basis for intensive care discontinuation owing to futility.
Journal ArticleDOI
The CLIF Consortium Acute Decompensation score (CLIF-C ADs) for prognosis of hospitalised cirrhotic patients without acute-on-chronic liver failure
Rajiv Jalan,Marco Pavesi,Faouzi Saliba,Alex Amoros,Javier Fernández,Peter Holland-Fischer,Rohit Sawhney,Rajeshwar P. Mookerjee,Paolo Caraceni,Richard Moreau,Pere Ginès,François Durand,Paolo Angeli,Carlo Alessandria,Wim Laleman,Jonel Trebicka,Didier Samuel,Stefan Zeuzem,Thierry Gustot,Alexander L. Gerbes,Julia Wendon,Mauro Bernardi,Vicente Arroyo,Canonic Study Investigators +23 more
TL;DR: The new CLIF-C AD score is more accurate than other liver scores in predicting prognosis in hospitalised cirrhotic patients without ACLF and may be used to identify a high-risk cohort for intensive management and a low-risk group that may be discharged early.
Journal ArticleDOI
Addressing Profiles of Systemic Inflammation Across the Different Clinical Phenotypes of Acutely Decompensated Cirrhosis
Jonel Trebicka,Alex Amoros,Carla Pitarch,Esther Titos,José Alcaraz-Quiles,Robert Schierwagen,Robert Schierwagen,Carmen Deulofeu,Javier Fernandez-Gomez,Salvatore Piano,Paolo Caraceni,Karl Oettl,Elsa Solà,Wim Laleman,Jane McNaughtan,Rajeshwar P. Mookerjee,Minneke J. Coenraad,Tania M. Welzel,Christian J. Steib,Rita Garcia,Thierry Gustot,Miguel Ma Rodriguez Gandia,Rafael Bañares,Agustín Albillos,Stefan Zeuzem,Victor Vargas,Faouzi Saliba,Frederik Nevens,Carlo Alessandria,Andrea De Gottardi,Heinz Zoller,Pere Ginès,Tilman Sauerbruch,Alexander L. Gerbes,Rudolf E. Stauber,Mauro Bernardi,Paolo Angeli,Marco Pavesi,Richard Moreau,Joan Clària,Rajiv Jalan,Vicente Arroyo +41 more
TL;DR: Investigation of a battery of markers of systemic inflammation in patients with acutely decompensated cirrhosis found that baseline systemic inflammation was more intense in those who had poor 28-day outcomes (ACLF, death) than those who did not experience these outcomes.
