Abstract: Encephalitis and polyneuritis occurring after rabies vaccination are believed to be immunologically mediated. We studied antibody responses to neural antigens in 36 patients with major neurologic complications, 25 with minor complications, and 39 with no complications after immunization with a brain-derived, Semple rabies vaccine. Patients with major complications had significantly elevated levels of antibody to brain white matter as compared with the other groups (P<0.001). Assays for antibody to selected central nervous system antigens showed that high levels of serum and cerebrospinal fluid antibody to myelin basic protein correlated with the presence of major neurologic complications (both central and peripheral nervous systems). The level of antibody to cerebroside correlated best with the number of injections of vaccine, but like antibody to myelin basic protein, the antibody to cerebroside was present in the cerebrospinal fluid of patients with major complications. Abstract: We studied 61 patients with complications of Semple-type postexposure rabies immunization. Thirty-six had neurologic signs, and 25 had only fever, headache, or myalgia. Thirty-two patients had CNS complications, and 4 had an acute peripheral neuropathy. Disease was acute and monophasic in 33, but 3 patients had progressive disease, including 1 patient with a relapsing-remitting course. No clinical features, including CSF content of myelin basic protein, were prognostic indicators. In three of six patients with encephalomyelitis, lymphocytes showed a proliferative response to myelin.

Abstract: • From 1977 to 1984, Salmonella typhi was isolated from 85% and Salmonella paratyphi A was Isolated from 15% of 192 Thai children with enteric fever at children's Hospital, Bangkok. Children with enteric fever presented with sudden onset of fever and gastrointestinal symptoms. Diarrhea occurred in 62% of children with paratyphoid fever and 36% of children with typhoid fever. Rose spots were seen in 15% of patients with typhoid and 7% of patients with paratyphoid fever. There were no deaths. Bronchitis and pneumonia occurring In 11% of patients were the most common complications. Acute hemolysis occurred in 3% of the patients with typhoid fever who had thalassemia or glucose-6-phosphate dehydrogenase deficiency. Chloram-phenicol-resistant S typhi , which accounted for 70% of the isolates in 1977, has since 1982 accounted for less than 2% of isolates. ( AJDC 1987;141:862-865)

Abstract: During 5 h of acute heat exposure (41 °C), there were increases in the heart rate from 43±2 (S.E.) to 51 ± 1 beats/min, respiratory rate from 26±4 to 86± 16 breaths/min and rectal temperature from 38·5 ± 0·1 °C to 39·7 ± 0·2 °C. The flow rate of liquid from the rumen and body water turnover significantly increased while biological half life of chromium-51 ethylenediaminetetra-acetate in the rumen and tritiated water decreased from 12·9 ± 2·5 and 87·7 ± 6·8 h to 9·3 ± 2·0 and 49·2 ± 5·7 h respectively. An increase in blood volume during acute heat stress occurred with an increase of both plasma and cell volume. An elevation of plasma water coincided with an increase in plasma protein and glucose. There is evidence that the increase in plasma water during heat exposure came from extravascular tissue space and/or from the digestive tract.

Abstract: Three patients are reported whose cranial dura mater and bones were penetrated by intracranial glioblastomas in the absence of previous craniotomy or radiotherapy. The gliomatous nature of the tumors was confirmed by localization of cytoplasmic glial fibrillary acidic protein (GFAP) of the neoplastic cells. Review of the literature disclosed only 15 glioblastomas, including the three cases, spontaneously penetrating the cranial dura mater and bones. These patients ranged in age from 3.5 to 70 years with an average age of 40 years. The male/female ratio was 5/8. Five glioblastomas were in the temporal lobes, three were in the frontal lobes, three were in the frontotemporal regions, two were in the occipital lobes, one was in the frontoparietal region, and one was in the temporoparietooccipital region. Six glioblastomas also had spontaneous distant metastases. In the absence of previous craniotomy and radiotherapy, rapid growth of the glioblastomas may promote such spontaneous penetration into the cranial dura mater and bones.

Abstract: Nine children; 4 males and 5 females, aged ranging from 2 1/2 to 13 years presented with signs and symptoms of poor tissue perfusion associated with dengue shock syndrome. All these 9 patients were subjected to the therapeutic trial of high dose methyl prednisolone (MP; 9/9) and mannitol (M; 6/9) after their failure to the saline and plasma replacement. Following the high dose MP and M, a significant increment in the effective circulatory blood volume as reflected by the sustained increment in CVP, widening of PP and declining in PR as well as improvement in clinical tissue perfusion were established in 7 of these nine patients so treated.

Abstract: Paramichelia baillonii has been used by the natives of Northern Thailand for medicinal purposes. Four components have been isolated from the bark of this plant and their structures determined by spectroscopic means. Three of the components had been reported previously: the germacranolide epoxides (--)-dihydroparthenolide [1] and (--)-parthenolide [2] and the oxoaporphinoid alkaloid liriodenine [4]. The fourth component is an unusual new germacranolide alkaloid which has been named (--)-bisparthenolidine [3] because it was presumably formed in the plant from ammonia and two molecules of parthenolide. Parthenolide was reported previously to possess anti-tumor activity, and in the present study we report on the significant activity of the new alkaloid 3 in the KB cell culture assay.

Abstract: A parapharyngeal ectomesenchymoma consisting of mixed malignant fibrous histiocytoma and primitive neuroectodermal tumour with neuroglial differentiation occurred in a 36-year-old woman. Immunohistochemical and electron microscopic studies verified the combined mesenchymal and neuroectodermal components within the tumour. Only 9 similar cases, including the present one, are on record. The patients ranged in age from 6 months to 49 years, with an average age of 18 years. The male: female ratio was 4:5. The tumour location was widespread. All reported ectomesenchymomas were histopathologically malignant. Recurrence and/or metastasis was/were common. The neural crest has been suggested as the origin of the ectomesenchymomas.

Abstract: Six edible Thai mushroom species were analysed for protein, in vitro protein digestibility, amino acids and major and trace elements. The results for protein and amino acids were discussed relative to the contents in Thai rice. The contents of mineral elements showed great variation, in agreement with literature data. Two samples (Hunukao and Hom) were rich in calcium and one (Phang) was rich in iron and copper. For comparison results for samples of edible mushrooms obtained in Norway were included.

Abstract: Specific IgE antibody levels in the serum of patients with proven gnathostomiasis and in those with intermittent cutaneous migratory swellings were determined by the enzyme-linked immunosorbent assay (ELISA) using aqueous extracts of Gnathostoma spinigerum infective larvae as antigen. There was not only an elevation of specific IgE antibody levels but also a marked increase of total IgE in the serum of these patients. The mean levels of specific IgE antibody in both groups of patients were significantly higher than that of healthy controls (P<0·01). Only one of the 50 serum specimens tested had an ELISA reading that fell within the mean + 2 standard deviations of the control group, suggesting that the method would be useful with a high degree of reliability in confirming a clinical diagnosis of gnathostomiasis in humans. Compared with healthy controls, there was almost a 10-fold rise (P<0·01) in the total IgE in the serum of these patients, indicating that G. spinigerum infection potentiates IgE production similarly to many other nematode parasites.

Abstract: A coagglutination test for detection of group A streptococci in throat samples was evaluated in a multicentre study and found to be about 95 % sensitive when applied to swabs taken from symptomatic patients which yielded more than 100 colonies per plate on culture. The sensitivity of the test dropped significantly when it was applied to swabs giving fewer than 100 colonies on culture. The specificity of the test was high regardless of colony count on conventional media.

Abstract: The genetic polymorphism of factor B (BF) in four Asian populations was studied by immunofixation after agarose gel electrophoresis (AGE) in both veronal buffer (VB) and Tris/glycine/veronal buffer (TGVB). A suballele of the BF*F and three rare alleles were detected in a Chinese population. The subtype and the rare variants were further characterized by using isoelectric focusing in polyacrylamide gels (PAGIF) and by zymosan treatment of the serum samples. The subtype was identified as BFFb1, first defined in the Japanese and found to occur at a polymorphic frequency. One of the rare variants exhibited an intermediate mobility between BFF and BFS and seemed to be similar to BFF025. PAGIF, however, could clearly discriminate the two variants that had a similar mobility on AGE. The results obtained in this study suggest that conventional electrophoresis is not enough to characterize a new variant and to discriminate it from previously reported variants. The findings obtained on immunofixation after AGE/TGVB and in PAGIF must be added to those obtained by the standard method.
