Abstract: . A study utilizing a split-mouth design in 18 subjects and with four operators was performed in order to compare the effect of hand and ultrasonic instrumentation of periodontal pockets. Contralateral pairs of teeth with pockets of comparable depth were treated by either hand or ultrasonic instruments. Instrumentation was repeated after 4 weeks. Pocket depth, bleeding on probing and gingival fluid measurements were used for evaluation. A gradual reduction of pocket depth and number of bleeding points took place throughout the 8-week experimental period. Clinically significant differences were not observed comparing the effect of hand instruments to ultrasonics for any of the operators. The amounts of gingival fluid were similar for both methods of instrumentation at the final examination. Thus, the present study failed to demonstrate any significant differences between the effect of ultrasonic root debridement and hand instrumentation in the treatment of 4–6 mm deep periodontal pockets.

Abstract: Two isoelectric electroencephalograms obtained 24 hours apart support a clinical diagnosis of brain death in prolonged coma. Without documentation of electrocerebral silence, physicians are reluctant to discontinue vital support systems. A radionuclide bolus technique has been developed that documents the absence of cerebral blood flow in suspected brain death and supplements the flat EEG. In a recent review of this technique, all 27 adults who had EEG activity maintained the integrity of their cerebral blood flow. This contrasts to our studies of 5 children, all of whom demonstrated persistent EEG activity but had no evidence of cerebral blood flow by either the isotope bolus technique (5 patients) or cerebral angiography (4 patients). These children (mean age, 7 months) lacked cephalic reflexes and were maintained on assisted ventilation for an average of 15 days. Multiple electroencephalographic tracings persistently demonstrated low-voltage cortical activity over this time. Despite the lack of cerebral blood flow, all patients were continued on respiratory support. At autopsy, extensive brain liquefaction necrosis was noted. In comatose children, EEG monitoring may be of limited value while cerebral blood flow measurements can provide more practical and prognostic information.

Abstract: The relationship between various experimental concentrations of CO2 and calcification in Bossiella orbigniana (Decaisne) was studied by measuring Ca-45 incorporation into the crystalline matrix. Air containing CO2 at partial pressures (PCO2) of 0.04 to 5.5% was bubbled through synthetic seawater in incubation vessels. The resultant pH values in the presence of plants ranged from 6.5 to 8.7. The maximum calcification rate appears to lie between 0.11 and 1.05% PCO2. The data suggest that calcification is controlled by a biological process that may be sensitive to pH and/or to the relative bicarbonate concentration. The data also suggest that a severalfold increase in CO2 over the present atmospheric level might lead to increased calcification in this marine alga.

Abstract: The effect of a calcium and vitamin D dietary supplement on postextraction alveolar bone resorption is reported. Mean alveolar bone loss for patients receiving the supplement was 36% less than that for patients receiving a placebo medication in a 1-year double-blind study of 46 immediate denture patients. Relationships between alveolar bone resorption, osteoporosis, and adequacy of dietary calcium-phosphorus ratios were discussed, and clinical implications of recent research findings were presented.

Abstract: Although postoperative constrictive pericarditis is rare, the diagnosis should be considered when unexplained right-sided heart failure develops after cardiac surgery. Within a 6 week interval, evidence of constrictive pericarditis developed in three patients who had recently undergone myocardial revascularization. One patient presented with biventricular failure, pericardial effusion and suspected tamponade. Severe constrictive pericarditis was demonstrated at subsequent operation. An apparent postpericardiotomy syndrome preceded evidence of right heart failure in the other two patients. Etiologic considerations include the possibility that pericardial irrigation with povidone-iodine (Betadine) solution may have contributed to subsequent fibrosis.

Abstract: In reconstituted casein systems, complete sub-micelles were previously observed only under extreme conditions of ionic strength, namely a NaCl concentration of 2 M or greater. However, studies with the analytical ultracentrifuge on phosphate-containing casein systems revealed that under certain conditions, a stable sub-micelle-micelle equilibrium was established. Conditions which were standard throughout were 7.5 mg/ml protein, 0.04 M-NaCl, pH 6.6 and 37 °C. The concentrations of added CaCl2 (Ca2+) and inorganic phosphate (P1) were variable. With no P1 present, Ca-sensitive caseins precipitated or formed micelles when k-casein was present, between 6 and 7 mM-Ca2+. With 10–20 mM-P1 precipitation or micelle formation began at about 4 mM-Ca2+ and was complete at about 5 mM-Ca2+. Over this interval, during micelle formation, a sub-micelle peak with s20, w of about 13 S was observed in equilibrium with the broad micelle peak. In a system containing a 2:2:1 weight ratio of ±sI-:²-:k-casein, this sub-micelle was isolated at 4 mM-Ca2+ and 2.5 mM-P1. The mol. wt was 760000 and it thus contained approximately 33 monomer caseins. The reconstituted micelle system formed in the absence of P1 was quite temperature sensitive, forming at 37 °C but disappearing upon cooling. In the presence of P1 the micelles formed at 37 °C but were stable to cooling as are natural micelles. Evidently, a combination of hydrophobic and electrostatic interactions are involved in natural micelle formation, probably with the production of salt bridges of Ca and phosphate ions between sub-micelles.

Abstract: Calcitonin-containing cells in serial, 6-micrometer sections of the thyroid glands of Swiss Webster mice, at 1 day, 2 weeks, 4 weeks and 8 weeks of age, were demonstrated by an immunoperoxidase method, using antiserum to human calcitonin. C-cell nuclei were counted in every sixth section of both left and right lobes. The average number of C-cells counted in the thyroid glands of 8-week-old animals was 18-fold, 5.5-fold and 2.5-fold greater than the number observed in 1-day, 2-week and 4-week-old animals, respectively. C-cell concentration was found to be greatest in 4-week-old mice. Mitoses of C-cells were observed in animals which were 1 day, 2 weeks and four weeks old. No mitotic figures were seen in 8-week-old animals. A few C-cells were seen in close association with neurons. The volume of the thyroid glands of 8-week-old animals was about 14-, 4- and 3-fold greater than the volume in the 1-day-old, 2-week-old and 4-week-old mice, respectively. These changes in the C-cell population during development provide a model for the study of C-cell proliferation and storage of calcitonin.

Abstract: MEDUSA (MEdical Dose Uniformity SAmpler), a 16 plane multi-wire proportional chamber, has been built to accurately measure beam profiles. The large number of planes allows for reconstruction of highly detailed beam intensity structures by means of Fourier convolution reconstruction techniques. This instrument is being used for verification and tuning of the Bevalac radiotherapy beams, but has potential applications in many beam profile monitoring situations.

Abstract: It has been suggested that recovery of motor and sensory function in the site distal to a peripheral nerve lesion should be improved if the nerve bundles (fasciculi) are matched and individually sutured. Three parameters are proposed to provide quantitative data: the count of the nerve fibers that regenerate, the number of functional regenerated nerve fibers, and a measurement of end organ reinnervation. A thin cross section of a transected and repaired sciatic nerve of a mongrel cat is fixed, stained, photographed, and digitized through a microscope 6 months following nerve repair. The data arrays are then subjected to four basic processing routines: edge enhancing, thresholding, template matching, and peak detection. Finally, the peaks are counted and provide an estimate of the number of nerve fibers in the nerve under study. Comparing counts of nerve fibers proximal and distal to the transection site of the nerve provide data on the proportion of regeneration present at various times. The content of this paper is, to a large extent, describing the implementation of the needed image-processing algorithms for automated counting on the Multi-MiniComputer System (MMCS). Optimal use of the AP-120B array processor and the pipeline processing provided by using the Eclipse 200s and the Nova 3 make a marked improvement in overall throughput.
