Abstract: Objective Empirical beta-lactam monotherapy has become the standard therapy in febrile neutropenia. The aim of this study was to compare the efficacy and safety of piperacillin-tazobactam versus carbapenem therapy with or without amikacin in adult patients with febrile neutropenia. Methods In this prospective, open, single-center study, 127 episodes were randomized to receive either piperacillin-tazobactam (4 x 4.5 g IV/day) or carbapenem [meropenem (3 x 1 g IV/day) or imipenem (4 x 500 mg IV/day)] with or without amikacin (1 g IV/day). Doses were adjusted according to renal function. Clinical response was determined during and at completion of therapy. Results One hundred and twenty episodes were assessable for efficacy (59 piperacillin-tazobactam, 61 carbapenem). Mean duration of treatment was 14.8 +/- 9.6 days in the piperacillin-tazobactam group and 14.7 +/- 8.8 days in the carbapenem group (P > 0.05). Mean days of fever resolution were 5.97 and 4.48 days for piperacillin-tazobactam and carbapenem groups, respectively (P > 0.05). Similar rates of success without modification were found in the piperacillin-tazobactam (87.9%) and in the carbapenem groups (75.4%; P > 0.05). Fungal infection occurrence rates were 30.5 and 18% in piperacillin-tazobactam and carbapenem groups, respectively (P = 0.05). Antibiotic modification rates were 30.5 and 13.1% (P = 0.02) and the addition of glycopeptides to empirical antibiotic regimens rates were 15.3 and 44.3% for piperacillin-tazobactam and carbapenem groups, respectively (P = 0.001). The rude mortality rates were 14% (6/43) and 29.3% (12/41) in piperacillin-tazobactam and carbapenem groups, respectively (P = 0.08). Conclusions The effect of empirical regimen of piperacillin-tazobactam regimen is equivalent to carbapenem in adult febrile neutropenic patients.

Abstract: Purpose: To evaluate the neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) levels in patients with dry eye disease (DED).Methods: The white blood cell, neutrophil, platelet, and lymphocyte counts were performed in 78 dry eye patients and 60 controls. The NLR was calculated by dividing neutrophil count by lymphocyte count and the PLR was calculated by dividing platelet count by lymphocyte count.Results: The mean age was 53.4 ± 3.8 years in the DED group and 52.7 ± 3.4 years in the control group. The mean NLR was 2.6 ± 1.2 and the mean PLR was 138.4 ± 62.6 in the DED group and the mean NLR was 1.84 ± 0.5 and the mean PLR was 118.5 ± 64.7 in the control group. A significant difference was found in the NLR and PLR between the DED and the controls (p = 0.032 and p = 0.026, respectively).Conclusion: The NLR and PLR values were found higher in patients with dry eye than in healthy subjects.

Abstract: Continuous quality and reliability improvement of computers as well as their widespread applica- tion in design of furniture industry encourage the elaboration of optimization algorithms of furniture construc- tions, in which rigidity characteristics of the applied joints would be taken under consideration. In the last four decades, the fi nite element method, FEM, has become the dominant technique used for analyzing physical phe- nomena in the fi eld of structural, solid and fl uid mechanics as well as for the solution of fi eld problems. This paper gives a bibliographical review of the FEM applied in the analysis of furniture products constructed with wood. The following topics are included: Wood as a furniture construction material and its mechanical properties. It is, therefore, reasonable to replace statistical methods of furniture optimization, which have been applied so far, by gradient methods which allow obtaining much more precise results.

Abstract: Manganese was removed from naturally polluted river sediment by applying an electrokinetic remediation technique. The sediment was alkaline and had 20% clay, which was mainly illite. The electrokinetic remediation experiments were performed by controlling pHs in the electrode cells and reverse electroosmotic flows were observed, i.e., water moved from cathode towards anode. Manganese accumulated in areas closer to cathode, however, other metals, such as copper, zinc and lead were mostly observed in the middle section of the sediment. As a result of reverse electroosmotic flow, the removal efficiencies of metals were low and the highest removal efficiencies of manganese, copper and lead, were evaluated as 18%, 20% and 12%, respectively. Almost no removal of zinc was observed in all electrokinetic remediation experiments.

TL;DR: Etiology of cervical LAPs was diagnosed in 41.3% patients and lymphoma was the most common malignancy, and predictive factors for malignancies were having LAP larger than 30mm, rubbery lymph node, high serum CRP and LDH values, no hilum in ultrasonography, and enlargement of lymph node in follow-up.
