Nitroglycerin improves the hemodynamic response to vasopressin in portal hypertension

TLDR
Investigation of the effect of nitroglycerin on splanchnic and systemic hemodynamics in cirrhotic patients and portal hypertensive dogs and in dogs provides evidence that the addition of nitreglycerin to an i.v. infusion of vasopressin reversed the detrimental effects of vasipressin while preserving the beneficial effects.

UK guidelines on the management of variceal haemorrhage in cirrhotic patients

TL;DR: These updated guidelines on the management of variceal haemorrhage have been commissioned by the Clinical Services and Standards Committee (CSSC) of the British Society of Gastroenterology (BSG) under the auspices of the liver section of the BSG.
Journal ArticleDOI

Complications of cirrhosis. I. Portal hypertension

TL;DR: Recent studies suggest that early measurement of HVPG during variceal bleeding may be used as a guide for therapeutic decisions in the treatment of patients with acute varices, and the efficacy of more aggressive techniques should be further tested against beta-blockers in patients with a high risk of bleeding.

Effects of somatostatin on hepatic and systemic hemodynamics in patients with cirrhosis of the liver: Comparison with vasopressin

TL;DR: It is shown that somatostatin effectively reduces hepatic blood flow and portal pressure in patients with cirrhosis and severe portal hypertension, without altering the systemic circulation.

Wedged and free hepatic venous pressure measured with a balloon catheter

TL;DR: The balloon technique offers many advantages over the conventional approach, including the ease of achieving and demonstrating the wedged position from the femoral approach, the ability to measure the free and wedged pressure repeatedly without manipulating the catheter, and the measurement of pressure in a larger, more representative segment of the liver.

Intraarterial Vasopressin in the Treatment of Upper Gastrointestinal Hemorrhage: A Prospective, Controlled Clinical Trial

TL;DR: The failure of cessation of hemorrhage to improve survival is thought to be due to the degree of advancement of the underlying disease, to the torrential nature of the hemorrhage, and to the use of intraarterial vasopressin in some patients in the conventional treatment group in whom conventional therapy had failed.

Measurement of portal pressure and its clinical application

TL;DR: It is still uncertain whether portal pressure is useful in predicting variceal bleeding, but two groups of investigators have recently shown that wedged hepatic vein pressure and portal vein pressure are essentially identical in alcoholic liver disease and probably in cryptogenic cirrhosis and chronic active hepatitis.

Effect of intravenously administered posterior pituitary extract on hemorrhage from bleeding esophageal varices. A controlled evaluation.

TL;DR: A controlled study of pituitary extract in patients with cirrhosis and the effects of both esophageal balloon tamponade and emergency surgery in these patients have inherent dangers and are not always effective.
