Mild cognitive impairment

This article attempts to bring together basic and complex information which has been gathered on cellulose structure, principally that of native cellulose, over the last few decades. Even though advances have been made in the field of crystallography, powder crystallography cannot yield a definitive cellulose structure and single crystal diffraction is not possible due to the lack of suitable crystals. Knowledge obtained on the biosynthesis of native cellulose and on the polymorphy of cellulose and its derivatives help our understanding of ultrastructure. Many inconsistencies between early crystallographic studies of native cellulose have been clarified by the discovery that two polymorphs (α and β) of cellulose I exist. Models of the possible ultrastructural arrangements within native cellulose have been put forward over the decades; with advancement in technology, computer simulations of small and large systems are being created to test the viability of these ultrastructural models. It is hoped that this review will aid in the understanding of the complexity and uncertainties that still exist in this subject

Cellulose: the structure slowly unravels

Summary Early diagnosis of Alzheimer's disease (AD) is needed to initiate symptomatic treatment with acetylcholinesterase inhibitors, and will be of even greater significance if drugs aimed at slowing down the degenerative process, such as vaccination regimes and β-secretase and γ-secretase inhibitors, prove to affect AD pathology and to have clinical effect. However, there is no clinical method to determine in which patients mild cognitive impairment (MCI) will progress to AD with dementia, and in which patients MCI is benign. Hence, there is a great clinical need for biomarkers to identify incipient AD in patients with MCI. The CSF biomarkers total tau protein, phosphorylated tau protein, and the 42 amino-acid residue form of amyloid-β may, if put in the right clinical context, prove to have high enough diagnostic accuracy to meet this challenge.

The present invention makes it possible to improve the energy efficiency of an entire train on the move. A train-car control device in one embodiment is used in a train comprising a coupled plurality of hybrid train cars each driven by electric power supplied by a primary power supply and/or electric power supplied by an electricity-storage device, wherein each electricity-storage device can be charged, during braking, by regenerated electric power from a regenerative brake. The aforementioned train-car control device is provided with the following: a communication means for communicating with that hybrid train car; an acquisition means for acquiring electricity-storage information consisting of the state of each electricity-storage device as detected by each hybrid train car via communication performed by the aforementioned communication means; and a control means that, on the basis of the acquired electricity-storage information from each hybrid train car, controls load sharing among the hybrid train cars, when the train is on the move, so as to even out the states of the electricity-storage devices.

安城更生病院の外来がん化学療法室は，外来患者のために安全かつ有効的な化学療法を導入することを目的として，2006年8月に開設された。しかしながら，外来化学療法における安全性および有効性はまだ十分に検証されていない。今回われわれは，当院においてがん外来化学療法を導入することに対する有効性を明らかにするためにこの研究を行なった。調査項目として外来化学療法を受ける患者数の推移，在院日数の変化，救急車で受け入れた患者数の変化，病床稼働率の変化および使用頻度の高いレジメンの推移を含めた。 この結果，2006年8月に外来がん化学療法室開設以来，外来がん化学療法室へ訪れる患者の増加に伴って，救急車によって受け入れられた患者数は明らかに増加した。そして在院日数も徐々に短縮した。加えて，病床稼働率は外来化学療法室の開設によって，明らかな影響を受けなかった。また，高頻度に使用される化学療法レジメンは9つであった (全レジメンの64.2%を占めた)。 これらの結果は，今後当院で外来がん化学療法を促進していくにあたり，有用な知見を与えた。

Study on Effective and Safe Cancer Chemotherapy for Outpatients (Part1): Influence of Establishment of the Outpatient Cancer Chemotherapy Unit on Effective Usage of Hospital Beds

PROBLEM TO BE SOLVED: To provide a wireless apparatus capable of transmitting a millimeter wave signal without deteriorating the transmission characteristic even under a bad propagation environment. SOLUTION: The wireless apparatus modulates transmission data with a baseband signal by the FSK modulation system and applies frequency modulation to a high frequency signal generated by a local oscillator 122 on the basis of the baseband signal and transmits the resulting signal by a circularly polarized wave when the propagation environment is bad. The wireless apparatus modulates the transmission data with the base band signal by the QPSK modulation system and uses first and second mixers 124, 125 to frequency-mix the baseband signal and the high frequency signal generated by the local oscillator 122 to transmit the resulting signal by a linearly polarized wave when the propagation environment is good. Wireless apparatus, and polarized wave switching method

A redecking method for damaged reinforced concrete slab is proposed. The prefabricated steel deck of battledeck floor type, which consists of a deck plate, longitudinal ribs and small transverse ribs, is used and some cross beams are added to the existing bridge to support the deck. Static loading tests of large scale models were carried out and the structural details were developed. Fatigue tests of the models were also conducted to check the durability. Applying this method, a bridge was widened and reinforced. Two loading tests of the bridge were executed, i.e. during the construction and after the completion. This paper presents the outline of these tests and the reconstruction work.

An Experimental Study on Redecking Method with Prefabricated Steel Deck of Battledeck Floor Type

Background/Purpose The Frey procedure, the coring out of the pancreatic head and longitudinal pancreaticojejunostomy, is a safe, easy, and reliable method to solve most of the problems associated with chronic pancreatitis. During long-term follow up, unexpected relapse in the pancreatic tail was encountered. The pattern of failure and the rationale for a new procedure to treat or prevent such relapse were investigated. Methods From 1992 to 2008, 71 patients with chronic pancreatitis underwent the Frey procedure at Tohoku University Hospital. The etiology was alcoholic in 92.6% of them, followed in incidence by idiopathic and hereditary chronic pancreatitis. In the primary operation, besides the Frey procedure, combined resection of the pancreatic tail was performed in three patients, and choledochoduodenostomy was performed in one patient. The follow-up rate was 92.9%, with a median period of 46 months. Results The incidence of early postoperative complications was 18.4%, with one reoperation for gastrointestinal bleeding from the splenic artery. Pain control was achieved in all patients and there was no operative mortality. During the long-term follow up of 62 patients with the Frey procedure, eight patients had relapse of inflammation and required reoperation. Five of these eight patients had a pseudocyst in the pancreatic tail and underwent distal pancreatectomy (DP). Conclusions Relapse occurred in alcoholic middle-aged male patients, and in the patients with hereditary and idiopathic pancreatitis. Frey-DP and Frey-spleen-preserving DP (SPDP) procedures can be performed safely and effectively to treat the relapse and to prevent relapse in the pancreatic tail.
