Abstract: We compared the diagnostic accuracy of conventional asynchronous teledermatology ('store-and-forward' [SAF]) with a combined technique, in which SAF methodology was used first, followed by a videoconference using low-cost Web cameras. The study involved 228 patients with 242 lesions. For each case, two independent teledermatologists (A and B) evaluated digital images and clinical information by the conventional SAF method and gave a single diagnosis. Then, each communicated with the patient via Web cameras and gave a single diagnosis (combined method). Finally, teledermatologist A performed a face-to-face examination of each patient and established the gold standard diagnosis. With the conventional SAF method, the diagnostic accuracy of teledermatologist A was 81%, while that for teledermatologist B was 75%. With the combined method, the corresponding values were 90% and 82% (P<0.001 for both). There was no significant difference in the interobserver agreement between the two methods. Use of Web camera videoconferencing improved patient satisfaction with teledermatology. This method of teledermatology may be a useful alternative to the SAF method alone.

Abstract: Background and Purpose Successful recanalization after endovascular stroke therapy (EVT) did not translate into a good clinical outcome in randomized trials The goal of the study was to identify the predictors of a good outcome after mechanical thrombectomy with stent retrievers Methods A retrospective analysis of a prospectively collected database included consecutive patients treated with stent retrievers We evaluated the influence of risk factors for stroke, baseline NIHSS score, Alberta Stroke Program Early CT (ASPECT) score, recanalization rate, onset-to-recanalization and onset-to-groin puncture time, and glucose levels at admission on good outcomes The number of stent passes during procedure and symptomatic hemorrhage rate were also recorded A modified Rankin Scale (mRS) score of 0–2 at 90 days was considered as a good outcome Results From January 2011 to 2014, 70 consecutive patients with an acute ischemic stroke underwent EVT with stent retrievers The absence of a medical history of diabetes was associated with good outcomes Apart from diabetes, the baseline demographic and clinical characteristics of patients were similar between subjects with poor outcome versus those with good outcomes Median time from onset to recanalization was significantly shorter in patients with good outcomes 245 (IQR: 216–313 min) compared with poor outcome patients (315 (IQR: 240–360 min); ) Symptomatic intracranial hemorrhage was observed in eight (216%) of 37 patients with poor outcomes and no symptomatic hemorrhage was seen in patients with good outcomes () In multivariate stepwise logistic regression analysis, a favorable ASPECT score (ASPECT > 7) and successful recanalization after EVT were predictors of good outcomes Every 10-year increase was associated with a 360-fold decrease in the probability of a good outcome at 3 months The probability of a good outcome decreases by 143-fold for each 20 mg/dL increase in the blood glucose at admission Conclusion To achieve a good outcome after EVT with stent retrievers, quick and complete recanalization and better strategies for patient selection are warranted We need randomized trials to identify the significance of tight blood glucose control in clinical outcome during or after EVT

Abstract: As a consequence of the expanded use of long-term hemodialysis and extended life spans, complications of chronic renal failure are encountered with an increased frequency among uremic patients. Such patients may develop many thoracic and extrathoracic problems--most frequently uremic pleuritis and pericarditis, uremic pneumonia, infection, and metastatic pulmonary calcification. We retrospectively analyzed the medical records of 257 patients who had received long-term hemodialysis between 1990 and 2006 to better understand the incidence, causes, and clinical features of pleural effusions in this population. The incidence of pleural effusion in hospitalized patients receiving long-term hemodialysis was 20.2% (n=52; mean age, 55.83 +/- 16.56 years; male-to-female ratio, approximately 3:2). Pleural effusion resulted from hypervolemia in 61.5% and was bilateral in 68.8% of patients. Unilateral effusion was present in 25 of 52 (48%) patients. The most frequent causes of unilateral effusion were hypervolemia (n=9) and parapneumonic effusion (n=5). Thoracenteses were performed in 14 of the 52 patients in the study group. Of thoracenteses performed, 64.3% of the patients had transudative pleural effusion and 35.7% had exudative effusion. Transudative pleural effusion resulted from hypervolemia in 66.7% and heart failure in 22.2%. Of the patients with transudative effusion, 85.7% were bilateral. The most frequent cause of exudative pleural effusion was uremic pleuritis, which occurred in 40% of the patients. The most common symptom was dyspnea, which occurred in 53.8% of patients. In conclusion, pleural effusions are common in patients receiving chronic hemodialysis. Thoracentesis may be performed in patients with unilateral pleural effusion. Since hypervolemia was the most common cause of pleural effusion, this complication should not be considered an obstacle in renal transplant recipients.

Abstract: This study determines the frequency of the spousal domestic violence among 1,178 married women who applied to some first level medical institutions located in Ankara, the capital of Turkey. 77.9% (n = 918) of the women who participated in the study have stated that they have been exposed to at least one of the types of spousal violence during their lifetime. The most reported type of domestic violence suffered by women throughout their lives is economic violence (60.4%). The prevalence of controlling behaviors, emotional violence, physical and sexual violence are 59.6%, 39.7%, 29.9% and 31.3% respectively.
