Assessment of Cardiac Structure and Left Atrial Appendage Functions in Primary Antiphospholipid Syndrome A Transesophageal Echocardiographic Study

Abstract: Background and Purpose— Although thromboembolic events are the major complication of primary antiphospholipid syndrome (PAPS), cardiac involvement is commonly present. Left atrial appendage (LAA) is recognized as an important source for thrombus formation and thromboembolism. The purpose of the study was to assess the structure and function of LAA with transesophageal echocardiography (TEE) in PAPS patients. Methods— Thirty-one PAPS patients (22 women, mean age 36±9 years) in sinus rhythm and 31 (17 women, mean age 37±7 years) controls with normal TEE examination were investigated. Results— Eighty-four percent of the PAPS patients had functional and structural valvular defect predominantly in the mitral valve. Valvular lesions were especially frequent in PAPS patients with a history of cerebrovascular events, patients with history of arterial thrombosis (91.6%), and patients with high titers of IgG anticardiolipin antibodies (100%). Calibration transfer in temperature modulated gas sensor arrays

Abstract: Shifts in working temperature are an important issue that prevents the successful transfer of calibration models from one chemical instrument to another. This effect is of special relevance when working with gas sensor arrays modulated in temperature. In this paper, we study the use of multivariate techniques to transfer the calibration model from a temperature modulated gas sensor array to another when a global change of temperature occurs. To do so, we built 12 identical master sensor arrays composed of three different types of commercial Figaro sensors and acquired a dataset of sensor responses to three pure substances (ethanol, acetone and butanone) dosed at 7 concentrations. The master arrays are then shifted in temperature (from −50 to 50 °C, ΔT = 10 °C) and considered as slave arrays. Data correction is performed for an increasing number of transfer samples with 4 different calibration transfer techniques: Direct Standardization, Piece-wise Direct Standardization, Orthogonal Signal Correction and Generalized Least Squares Weighting. In order to evaluate the performance of the calibration transfer, we compare the Root Mean Square Error of Prediction (RMSEP) of master and slave arrays, for each instrument correction. Best results are obtained from Piece-wise Direct standardization, which exhibits the lower RMSEP values after correction for the smaller number of transfer samples.

The role of closed-suction drainage in preventing epidural fibrosis and its correlation with a new grading system of epidural fibrosis on the basis of MRI

Abstract: In this study we aimed to evaluate the role of closed-suction drainage on the extent of epidural fibrosis (EF) after lumbar disc surgery and to define a new grading system of epidural fibrosis in these patients, based on magnetic resonance imaging. Seventy-nine patients (34 women, 45 men) with a unilateral, single-level lumbar disc herniation were included in this study. Forty-one patients in whom closed-suction drainage was implanted were compared with 38 patients in whom the drain was not implanted. We have used a new grading system for the extent of epidural fibrosis, on the basis of follow-up magnetic resonance imaging findings. Pain intensity was evaluated by visual analog scale (VAS), and the patients’ function and working ability were measured according to the Prolo functional-economic scale. We conclude that, in patients operated on for unilateral, single-level lumbar disc hernias, implantation of closed-suction drainage into the operation site results in less formation of EF radiologically and yields better clinical outcome.
