Abstract: AIM The aim of the present study was to determine the prevalence of the metabolic syndrome using Adult Treatment Panel III (ATP III) criteria in Adana, a southern province of Turkey. METHODS The randomly selected study population included 1637 adults who were 20-79 yr of age. The presence of > or = 3 of components like hypertension (defined as blood pressure > or = 130/> or = 85 mmHg on two separate examinations, or usage of antihypertensive agents), visceral obesity (waist circumference >88 cm in females and >102 cm in men), low HDL cholesterol level ( or = 1.7 mmol/l), fasting glucose level > or = 6.1 mmol/l, or usage of antihyperglycaemic drugs, indicated the metabolic syndrome. RESULTS The prevalence of the metabolic syndrome was 33.4% and more common in women than in men (39.1 vs 23.7%; p 0.05), but were markedly higher among women in rural than urban areas (44.5 vs 31.2%; p<0.0001). CONCLUSIONS Developing countries like Turkey also need to start action to prevent and treat the components of the metabolic syndrome. Prevention of the modifiable risk factors, such as obesity and physical inactivity, and blood pressure control should be the key strategy for avoiding mortality and financial costs of the healthcare system, especially in view of limited resources.

Abstract: Oral isotretinoin is the most effective choice in the treatment of severe acne. Application of isotretionin to acne has been expanded to treat those patients with less severe but scarring acne who are responding unsatisfactorily to conventional therapies. However, its use is associated with many side effects, some of which can result in very disastrous consequences. Data related with intermittent isotretinoin therapy is still limited. Our aim was to asses the efficacy and tolerability of two different intermittent isotretinoin courses and compare them with conventional isotretinoin treatment. In this multicenter and controlled study, 66 patients with moderate to severe cases were randomized to receive either isotretionin for the first 10 days of each month for 6 months (group 1), or each day in the first month, afterwards the first 10 days of each month for 5 months (group 2) or daily for 6 months (group 3). The drug dosage was 0.5 mg/kg/day in all groups. Patients were followed-up for 12 months. Efficacy values were evaluable for 22 patients in group 1, 19 patients in group 2, and 19 patients in group 3. Acne scores in each group were significantly lower at the end of treatment and follow-up periods (P < 0.001). When patients were evaluated separately as moderate (n = 31) and severe (n = 29), no statistically significant differences were obtained among the treatment protocols in patients with moderate acne. However, there was a significant difference between groups 1 and 3 to the response of the treatments in severe acne patients at the end of follow-up period (P = 0.013). The frequency and severity of isotretionin-related side effects were found to be lower in groups 1 and 2 compared with group 3. Intermittent isotretinoin may represent an effective alternative treatment, especially in moderate acne with a low incidence and severity of side effects. The intermittent isotretinoin can be recommended in those patients not tolerating the classical dosage.

Abstract: Objective: To test the hypotheses that (1) there is no significant difference between the effects of two burs on the surface roughness of enamel after orthodontic debonding, and (2) there is no difference between resin removal times of the two burs. Materials and Methods: The crowns of 20 premolars were embedded in acrylic blocks, and the buccal surfaces were subjected to atomic force microscopy (AFM), with measurement of initial roughness values. The brackets were bonded with a light-cured adhesive and were debonded with a debonding plier. In half of samples, adhesive remnants were removed with a tungsten carbide bur, whereas a fiber-reinforced composite bur was used in the other half. The second AFM measurements were made after resin removal. Duration of removal procedures was also recorded. Results of roughness and duration measurements were analyzed with the use of repeated measurements analysis of variance and independent t-tests, respectively.
