Abstract: The incidence of heart failure (HF) is increasing while numerous pharmacological breakthroughs have been accomplished in the last two decades. Mortality remains high and is comparable to mortality due to oncologic disease. Coronary artery disease and hypertension are the primary causes of heart failure in Western societies while valvular heart disease is a significant cause in economically underdeveloped societies. While there are many pharmacological therapies for systolic disfunction, little is known about the optimal way to treat diastolic dysfunction. Although William Withering described the properties of digitalis in 1785, two hundred years passed before a scientific trial documented its benefit in HF. Skilled dissection and probing of the molecular basis of cardiac muscle contraction led to the development of numerous compounds postulated to be more effective than digitalis, only to disappoint repeatedly in numerous trials. In contrast, ACE inhibitors (ACEI) revolutionized the management of HF and it is unlikely that another class of agents will show dramatic improvements like the ACEI. Therefore, the pharmacological therapy of HF is proving to be more an art than a science, as numerous agents have to be tuned against each other to produce an overall effect that is beneficial.

Abstract: In the reviewed period, articles on peripheral eye movement disorders covered interesting aspects. Localizing value of associated signs, repetitive presentations of palsies, and classical quotations are stressed for the oculomotor nerve. The superior oblique is correlated to central nervous system disorders when overacting in pediatric patients or when ocular torsion is matched to the perceived vertical tilt. The family "pseudo" brought two of its members: "pseudo" myasthenia and "pseudo" myotonia. Mitochondrial citopathies with ocular manifestations can overlap among the different clinical types, eg, Kearns-Sayre, MELAS (mitochondrial encephalopathy-lactic acidosis and strokelike episodes), MERFF (myoclonic epilepsy and ragged red fibers). The diagnostic value of DNA mutations is emphasized in those syndromes. Imaging of the carotid arteries provides useful hints in cases where the lumen is narrowed due to internal processes or external compression; its interpretation is not only of diagnostic but of prognostic value. Certain otorhinolaryngology surgical procedures can damage the orbital muscles and produce serious inconvenience to the ocular motility. Analyzing the involved structures the therapeutic gesture can be determined. Diplopia after cataract surgery or retinal detachment repair is due to different factors, anesthetics, or implant location and is implied in every case.

Abstract: The purpose of this study was to retrospectively assess patients treated with modular, non-cemented, tapered, fluted, distal fixation stems. We included patients with 24 months minimum follow-up. Diagnosis that led to revisions were described. The radiographic analysis was made with preoperative, immediate postoperative and last control postoperative X-rays. Sixty-seven patients met inclusion criteria. We observed 59,7% (n=40) Osteointegration, 34,3% of Stable Fibrosis and 5,97% Unstable fibrosis. Stress Shielding was registered with 10,44% and Subsidence was observed in 34,3% of the patients. Modular, fluted, tapered, distal fixation stems have an excellent survival rates.
