Abstract: I read with great interest the article by Alvarez et al. In their review, they confirmed that Painful Legs and Moving Toes (PLMT’s) etiology is diverse although the majority of patients had evidence of peripheral nerve involvement or radiculopathy. However, in some patients, namely in patients with Wilson’s disease and myelitis, a structural central nervous system lesion is evoked to be associated with the development of PLMT. We reported a patient with Hashimoto’s disease who developed slight dystonia and bradykinesia and presented PLMT. The patient had any electrophysiological signs of neuropathy or any clinical sign indicative of a root or nerve disorder. MRI brain scan showed several small subcortical white matter fronto-parietal lesions, without diffusion restriction. Discrete and diffuse high signal in periventricular white matter was visible in T2-weighted images. These aspects are frequently found in Hashimoto’s encephalopathy. We also believe that at least in some cases PLMT has a central origin and is composed of a variety of dystonic movements. In our reported case, we think PLMT was associated with Hashimoto’s encephalopathy.

Abstract: Initial therapy is conservative, though this principally takes the form of reassurance and support when it comes to voiding LUTS. Medications for voiding LUTS in men are numerous, comprising alpha blockers, 5-alpha-reductase inhibitors, PDE5 inhibitors, and phytotherapy; these may be used separately or combined with others, including drugs for storage LUTS. Some targets are under evaluation for new medication potential. Interventional therapy depends on whether bladder outlet obstruction is present. If so, there is a range of minimally invasive and surgical options. For underactive detrusor, there is no clear-cut effective treatment, and identifying this problem needs to lead to discussion about the reduced efficacy that may be attached to medications and interventional therapy.

Abstract: CASE A total knee arthroplasty (TKA) was performed on a 35-year-old man with congenital fibular deficiency and a 20° varus and 28° antecurvatum tibial deformity of the left lower limb CONCLUSION One-stage TKA and correction of the extraarticular deformity by means of intraarticular bone resections and a standard soft tissue release were performed to restore the limb's mechanical axis Patients with congenital fibular deficiency present a wide range of limb deformities because of bone deficiencies or treatment sequels, which might require a specific surgical technique and small-sized implants to obtain good results during a TKA
