Abstract: Doxorubicin (DOX) is a widely used, potent chemotherapeutic agent; however, its clinical application is limited because of its dose-dependent cardiotoxicity. DOX's cardiotoxicity involves increased oxidative/nitrative stress, impaired mitochondrial function in cardiomyocytes/endothelial cells and cell death. Cannabidiol (CBD) is a nonpsychotropic constituent of marijuana, which is well tolerated in humans, with antioxidant, antiinflammatory and recently discovered antitumor properties. We aimed to explore the effects of CBD in a well-established mouse model of DOX-induced cardiomyopathy. DOX-induced cardiomyopathy was characterized by increased myocardial injury (elevated serum creatine kinase and lactate dehydrogenase levels), myocardial oxidative and nitrative stress (decreased total glutathione content and glutathione peroxidase 1 activity, increased lipid peroxidation, 3-nitrotyrosine formation and expression of inducible nitric oxide synthase mRNA), myocardial cell death (apoptotic and poly[ADP]-ribose polymerase 1 [PARP]-dependent) and cardiac dysfunction (decline in ejection fraction and left ventricular fractional shortening). DOX also impaired myocardial mitochondrial biogenesis (decreased mitochondrial copy number, mRNA expression of peroxisome proliferator-activated receptor γ coactivator 1-alpha, peroxisome proliferator-activated receptor alpha, estrogen-related receptor alpha), reduced mitochondrial function (attenuated complex I and II activities) and decreased myocardial expression of uncoupling protein 2 and 3 and medium-chain acyl-CoA dehydrogenase mRNA. Treatment with CBD markedly improved DOX-induced cardiac dysfunction, oxidative/nitrative stress and cell death. CBD also enhanced the DOX-induced impaired cardiac mitochondrial function and biogenesis. These data suggest that CBD may represent a novel cardioprotective strategy against DOX-induced cardiotoxicity, and the above-described effects on mitochondrial function and biogenesis may contribute to its beneficial properties described in numerous other models of tissue injury.

Abstract: Aim Serum α-fetoprotein (AFP) is the most important tumor marker for hepatocellular carcinoma (HCC). Previous reports indicated that HCC was also associated with increased levels of interleukin (IL)-6, IL-10 and hepatocyte growth factor (HGF). This study investigated the role of these cytokines as tumor markers for HCC. Method A total of 128 adults were prospectively enrolled and categorized into four groups: normal subjects (n = 29), chronic hepatitis B or C (n = 50), non-HCC tumors (n = 23) and HCC (n = 26). Serum AFP, IL-6, IL-10 and HGF levels were determined in all subjects. Results The expression of IL-6 or IL-10 (≥3 pg/ml), or high level of HGF (>1000 pg/ml) or AFP (>20 ng/ml) was observed in only 0–3% of normal subjects. Patients with HCC more frequently had higher IL-6 and IL-10 levels (p 5 cm) HCC more often had increased IL-6, IL-10 or AFP levels (p values all Conclusions Serum levels of IL-6 and IL-10 are frequently elevated in patients with HCC but not in benign liver disease or non-HCC tumors. IL-6 and IL-10 may help identify a subset of HCC patients with low AFP level, and may serve as complementary tumor markers in these patients.

Abstract: Purpose: Lyso-thermosensitive liposomal doxorubicin (LTLD) consists of doxorubicin contained within a heat-sensitive liposome. When heated to ≥40°C, LTLD locally releases a high concentration of doxorubicin. We aimed to determine whether adding LTLD improves the efficacy of radiofrequency ablation (RFA) for hepatocellular carcinoma (HCC) lesions with a maximum diameter (dmax) of 3 to 7 cm.Experimental Design: The HEAT Study was a randomized, double-blind, dummy-controlled trial of RFA ± LTLD. The 701 enrolled patients had to have ≤4 unresectable HCC lesions, at least one of which had a dmax of 3 to 7 cm. The primary endpoint was progression-free survival (PFS) and a key secondary endpoint was overall survival (OS). Post hoc subset analyses investigated whether RFA duration was associated with efficacy.Results: The primary endpoint was not met; in intention-to-treat analysis, the PFS HR of RFA + LTLD versus RFA alone was 0.96 [95% confidence interval (CI), 0.79-1.18; P = 0.71], and the OS HR ratio was 0.95 (95% CI, 0.76-1.20; P = 0.67). Among 285 patients with a solitary HCC lesion who received ≥45 minutes RFA dwell time, the OS HR was 0.63 (95% CI, 0.41-0.96; P < 0.05) in favor of combination therapy. RFA + LTLD had reversible myelosuppression similar to free doxorubicin.Conclusions: Adding LTLD to RFA was safe but did not increase PFS or OS in the overall study population. However, consistent with LTLD's heat-based mechanism of action, subgroup analysis suggested that RFA + LTLD efficacy is improved when RFA dwell time for a solitary lesion ≥45 minutes. Abstract: To compare the intraocular pressure (IOP) measurements using the Goldmann applanation tonometer (GAT), noncontact tonometer (NCT), and ocular blood flow tonometer (OBFT), and to evaluate the effects of varying central corneal thickness (CCT) on the readings. Ultrasound pachymetry and tonometry were performed on 170 eyes. Using the corrected GAT values that took CCT into account as the standard, we calculated the NCT and OBFT measurement errors related to the CCT variable by a linear regression model. Group comparisons were performed with the χ2 test and one-way ANOVA test. The correlation between the various tonometer measurements was analysed by Pearson's correlation method. Both the NCT (r=0.872, P<0.001) and OBFT measurements (r=0.861, P<0.001) were highly correlated with the GAT measurements. IOP measurements using the three tonometers were all correlated with CCT (all P<0.001), with the NCT measurements showing the greatest regression coefficient (β=0.063, r=0.650) and the GAT measurements the least (β=0.037, r=0.496). A linear regression model indicated that a 10 μm change in CCT resulted in a NCT measurement deviation of 0.47–0.98 mmHg and an OBFT measurement deviation of 0.29–0.81 mmHg. Pressure readings with the GAT, NCT, and OBFT are all affected by CCT, with the NCT being the one most affected and the GAT the least. Our findings suggest CCT an essential variable to consider in interpreting IOP readings, especially for the NCT measurements.
