Abstract: The first purpose of this review was to assess the impact of implementation on program outcomes, and the second purpose was to identify factors affecting the implementation process. Results from over quantitative 500 studies offered strong empirical support to the conclusion that the level of implementation affects the outcomes obtained in promotion and prevention programs. Findings from 81 additional reports indicate there are at least 23 contextual factors that influence implementation. The implementation process is affected by variables related to communities, providers and innovations, and aspects of the prevention delivery system (i.e., organizational functioning) and the prevention support system (i.e., training and technical assistance). The collection of implementation data is an essential feature of program evaluations, and more information is needed on which and how various factors influence implementation in different community settings.

Abstract: Communities have the potential to function effectively and adapt successfully in the aftermath of disasters. Drawing upon literatures in several disciplines, we present a theory of resilience that encompasses contemporary understandings of stress, adaptation, wellness, and resource dynamics. Community resilience is a process linking a network of adaptive capacities (resources with dynamic attributes) to adaptation after a disturbance or adversity. Community adaptation is manifest in population wellness, defined as high and non-disparate levels of mental and behavioral health, functioning, and quality of life. Community resilience emerges from four primary sets of adaptive capacities—Economic Development, Social Capital, Information and Communication, and Community Competence—that together provide a strategy for disaster readiness. To build collective resilience, communities must reduce risk and resource inequities, engage local people in mitigation, create organizational linkages, boost and protect social supports, and plan for not having a plan, which requires flexibility, decision-making skills, and trusted sources of information that function in the face of unknowns.

Abstract: If we keep on doing what we have been doing, we are going to keep on getting what we have been getting. Concerns about the gap between science and practice are longstanding. There is a need for new approaches to supplement the existing approaches of research to practice models and the evolving community-centered models for bridging this gap. In this article, we present the Interactive Systems Framework for Dissemination and Implementation (ISF) that uses aspects of research to practice models and of community-centered models. The framework presents three systems: the Prevention Synthesis and Translation System (which distills information about innovations and translates it into user-friendly formats); the Prevention Support System (which provides training, technical assistance or other support to users in the field); and the Prevention Delivery System (which implements innovations in the world of practice). The framework is intended to be used by different types of stakeholders (e.g., funders, practitioners, researchers) who can use it to see prevention not only through the lens of their own needs and perspectives, but also as a way to better understand the needs of other stakeholders and systems. It provides a heuristic for understanding the needs, barriers, and resources of the different systems, as well as a structure for summarizing existing research and for illuminating priority areas for new research and action.

Abstract: The pathways and processes through which empowering community settings influence their members, the surrounding community and the larger society are examined. To generate the proposed pathways and processes, a broad range of studies of community settings were reviewed, in the domains of adult well-being, positive youth development, locality development, and social change. A set of organizational characteristics and associated processes leading to member empowerment across domains were identified, as well as three pathways through which empowering settings in each domain contribute to community betterment and positive social change. The paper concludes with an examination of the ways that community psychology and allied disciplines can help increase the number and range of empowering settings, and enhance the community and societal impact of existing ones.

Abstract: Capacity is a complex construct that lacks definitional clarity. Little has been done to define capacity, explicate components of capacity, or explore the development of capacity in prevention. This article represents an attempt to operationalize capacity and distinguish among types and levels of capacity as they relate to dissemination and implementation through the use of a taxonomy of capacity. The development of the taxonomy was informed by the capacity literature from two divergent models in the field: research-to-practice (RTP) models and community-centered (CC) models. While these models differ in perspective and focus, both emphasize the importance of capacity to the dissemination and sustainability of prevention innovations. Based on the review of the literature, the taxonomy differentiates the concepts of capacity among two dimensions: level (individual, organizational, and community levels) and type (general capacity and innovation-specific capacity). The proposed taxonomy can aid in understanding the concept of capacity and developing methods to support the implementation and sustainability of prevention efforts in novel settings.

Abstract: This paper describes the development, application, and results of an implementation monitoring component of the Communities That Care (CTC) prevention framework used in the Community Youth Development Study (CYDS) to ensure high-fidelity prevention program implementation. This system was created based on research that community-based implementation of evidence-based prevention programs often includes adaptations in program design, content, or manner of delivery (Gottfredson and Gottfredson, Journal of research in crime and delinquency, 39, 3–35, 2002; Hallfors and Godette, Health Education Research, 17, 461–470, 2002; Wandersman and Florin, American Psychologist, 58, 441–448, 2003). A lack of fidelity to the implementation standards delineated by program designers is one indicator of a gap between prevention science and practice which can lessen the likelihood that communities will realize the positive participant effects demonstrated in research trials. By using the CTC model to select and monitor the quality of prevention activities, the 12 CYDS communities replicated 13 prevention programs with high rates of adherence to the programs’ core components and in accordance with dosage requirements regarding the number, length, and frequency of sessions. This success indicates the potential of the CTC program implementation monitoring system to enhance community Prevention Delivery Systems (Wandersman et al. American Journal of Community Psychology, this issue) and improve the likelihood of desired participant changes.

Abstract: This paper reviews empirical studies on whether participating in mutual help groups for people with mental health problems leads to improved psychological and social functioning. To be included, studies had to satisfy four sets of criteria, covering: (1) characteristics of the group, (2) target problems, (3) outcome measures, and (4) research design. The 12 studies meeting these criteria provide limited but promising evidence that mutual help groups benefit people with three types of problems: chronic mental illness, depression/anxiety, and bereavement. Seven studies reported positive changes for those attending support groups. The strongest findings come from two randomized trials showing that the outcomes of mutual help groups were equivalent to those of substantially more costly professional interventions. Five of the 12 studies found no differences in mental health outcomes between mutual help group members and non-members; no studies showed evidence of negative effects. There was no indication that mutual help groups were differentially effective for certain types of problems. The studies varied in terms of design quality and reporting of results. More high-quality outcome research is needed to evaluate the effectiveness of mutual help groups across the spectrum of mental health problems.

Abstract: The Interactive Systems Framework (ISF) for Dissemination and Implementation (Wandersman et al. 2008) elaborates the functions and structures that move evidence-based programs (EBPs) from research to practice. Inherent in that process is the tension between implementing programs with fidelity and the need to tailor programs to fit the target population. We propose Planned Adaptation as one approach to resolve this tension, with the goal of guiding practitioners in adapting EBPs so that they maintain core components of program theory while taking into account the needs of particular populations. Planned Adaptation is a form of capacity building within the Prevention Support System that provides a framework to guide practitioners in adapting programs while encouraging researchers to provide information relevant to adaptation as a critical aspect of dissemination research, with the goal of promoting wider dissemination and better implementation of EBPs. We illustrate Planned Adaptation using the JOBS Program (Caplan et al. 1989), which was developed for recently laid-off, working- and middle-class workers and subsequently implemented with welfare recipients.

Abstract: Communities are increasingly being required by state and federal funders to achieve outcomes and be accountable, yet are often not provided the guidance or the tools needed to successfully meet this challenge. To improve the likelihood of achieving positive outcomes, the Getting To Outcomes (GTO) intervention (manual, training, technical assistance) is designed to provide the necessary guidance and tools, tailored to community needs, in order to build individual capacity and program performance. GTO is an example of a Prevention Support System intervention, which as conceptualized by the Interactive Systems Framework, plays a key role in bridging the gap between prevention science (Prevention Synthesis and Translation System) and prevention practice (Prevention Delivery System). We evaluated the impact of GTO on individual capacity and program performance using survey- and interview-based methods. We tracked the implementation of GTO and gathered user feedback about its utility and acceptability. The evaluation of GTO suggests that it can build individual capacity and program performance and as such demonstrates that the Prevention Support System can successfully fulfill its intended role. Lessons learned from the implementation of GTO relevant to illuminating the framework are discussed.

Abstract: This study of 598 7th grade students of Mexican origin examined the role of traditional cultural values as a mediator of the effects of immigrant status, Mexican cultural orientation and Anglo cultural orientation on adolescent externalizing behavior and academic engagement. Immigrant status of adolescents and their maternal caregivers uniquely predicted increased Mexican cultural orientation and decreased Anglo cultural orientation, and both Mexican and Anglo cultural orientation related positively to adolescents’ endorsement of traditional cultural values. Endorsement of traditional cultural values related, in turn, to decreased externalizing behaviors and increased academic engagement and these findings were replicated across adolescent and teacher report of these two outcomes. Tests of mediation provided further evidence to support these pathways. Findings support the central importance of traditional cultural values as a protective resource that explains why immigrant youth exhibit fewer externalizing problems and increased academic engagement when compared to their second and third generation peers.

Abstract: Recent research has suggested that the sexual identity development of lesbian, gay, and bisexual (LGB) youths may not follow a single pattern, but may follow a variety of pathways. Although some research documenting variability in identity development exists, unclear are the potential individual and social contexts that predict these different patterns, as well as the contexts that predict changes in identity integration over time. This report longitudinally examined these issues in an ethnically diverse sample of 156 LGB youths (ages 14–21) in New York City. Cluster analytic techniques identified two patterns of sexual identity formation (i.e., early and more recent), and three patterns of sexual identity integration (i.e., high, middling, and low). Gender and sexual abuse were found to predict identity formation; and, gay-related stress, social support, negative social relationships, sexual orientation, sexual identity, and gender were found to predict patterns of sexual identity integration and changes in integration over 1 year. These findings document different patterns of sexual identity development and identify potential contextual barriers and facilitating factors that may be used to develop interventions to promote healthy LGB identity development.

Abstract: Youth mentoring programs are in the limelight. Over three million young people have a Big Brother, a Big Sister, or a similar adult volunteer involved in their lives–a sixfold increase from just a decade ago–and generous federal funding continues to fuel new initiatives. This expansion speaks volumes about the faith our society places in one-on-one relationships between vulnerable young people and caring adults. But what do we know about the effectiveness of this intervention strategy? A better understanding of the research evidence for youth mentoring, including findings from reviews, evaluations, and meta-analyses, provides a basis for a more informed, practically applicable approach to strengthening youth mentoring interventions.

Abstract: The roles of social support and coping as intervening processes between exposure to community violence and internalizing symptoms were examined longitudinally among a community sample of 667 middle school students in the inner city. After controlling for potential confounders (e.g., social desirability, victimization and witnessing of family violence, guardian’s psychological symptomatology), internalizing symptoms at Year 2 were predicted by hypothesized changes over 1 year, such that increased community violence exposure, decreased guardian and peer support, and increased use of defensive and confrontational behavioral coping were related to more internalizing symptoms of anxiety, depression and PTSD, although some of these relations varied by gender. The relations between internalizing symptoms at Year 3 and increased changes in exposure to community violence over 2 years were moderated by social support and/or coping, such that decreased guardian support and increased use of defensive and confrontational coping were generally associated with more symptoms for boys exposed to community violence. Girls who witnessed increased community violence and who increased their use of defensive or confrontational coping experienced more internalizing symptoms. The findings underscore the importance of developmental and contextual considerations in the design and implementation of interventions.

Abstract: Members of racial/ethnic minority groups are less likely than Caucasians to access mental health services despite recent evidence of more favorable attitudes regarding treatment effectiveness. The present study explored this discrepancy by examining racial differences in beliefs about how the natural course and seriousness of mental illnesses relate to perceived treatment effectiveness. The analysis is based on a nationally representative sample of 583 Caucasian and 82 African American participants in a vignette experiment about people living with mental illness. While African Americans were more likely than Caucasians to believe that mental health professionals could help individuals with schizophrenia and major depression, they were also more likely to believe mental health problems would improve on their own. This belief was unrelated to beliefs about treatment effectiveness. These findings suggest that a belief in treatment effectiveness may not increase service utilization among African Americans who are more likely to believe treatment is unnecessary.

Abstract: This experiment compared the effectiveness of an unlocked, mental health consumer-managed, crisis residential program (CRP) to a locked, inpatient psychiatric facility (LIPF) for adults civilly committed for severe psychiatric problems. Following screening and informed consent, participants (n = 393) were randomized to the CRP or the LIPF and interviewed at baseline and at 30-day, 6-month, and 1-year post admission. Outcomes were costs, level of functioning, psychiatric symptoms, self-esteem, enrichment, and service satisfaction. Treatment outcomes were compared using hierarchical linear models. Participants in the CRP experienced significantly greater improvement on interviewer-rated and self-reported psychopathology than did participants in the LIPF condition; service satisfaction was dramatically higher in the CRP condition. CRP-style facilities are a viable alternative to psychiatric hospitalization for many individuals facing civil commitment.

Abstract: DEBI, or the Diffusion of Effective Behavioral Interventions is the largest centralized effort to diffuse evidence-based prevention science to fight HIV/AIDS in the United States. DEBI seeks to ensure that the most effective science-based prevention interventions are widely implemented across the country in community-based organizations. Thus, this is a particularly timely juncture in which to critically reflect on the extent to which known principles of community collaboration have guided key processes associated with the DEBI rollout. We review the available evidence on how the dissemination of packaged interventions is necessary but not sufficient for ensuring the success of technology transfer. We consider additional principles that are vital for successful technology transfer, which were not central considerations in the rollout of the DEBI initiative. These issues are: (1) community perceptions of a top-down mode of dissemination; (2) the extent to which local innovations are being embraced, bolstered, or eliminated; and (3) contextual and methodological considerations that shape community preparedness. Consideration of these additional factors is necessary in order to effectively document, manage, and advance the science of dissemination and technology transfer in centralized prevention efforts within and outside of HIV/AIDS.

Abstract: The principles and processes for engaging youth-adult partnerships (Y-AP) in organizational and community decision making have often been articulated from developmental and social justice perspectives. A broad empirical foundation for Y-AP has been established. Y-AP remains an innovative idea in the United States, however. The belief that youth and adults can, and should, collaborate on issues of importance runs counter to prevailing policies, institutional structures, and community norms. 4-H Youth Development is one public system that is actively seeking to disseminate and implement Y-AP. 4-H Youth Development seeks to integrate Y-AP into its own governance structures as well as those of local government and community coalitions. Through qualitative analysis of the efforts in one Midwestern state, this study examines the contextual challenges faced by county staff—the providers of program support within 4-H Youth Development—and the ways in which county staff respond to these obstacles. This project identifies the goals, leverage points, and strategies through which county staff seek to integrate Y-AP into established forums of decision making. Implications for the dissemination and implementation of principle and process-based innovation are offered, with special attention to the role of the program support system.

Abstract: Research demonstrates that the well-being of migrants is a multilevel, dynamic, and value dependent phenomenon. It is multilevel because risk and protective factors are present at various ecological planes, from the individual to the social sphere. It is dynamic because objective and subjective risk and protective factors interact across ecological levels, creating favorable or unfavorable conditions for migrants to adjust to the new surroundings. Finally, it is value dependent because access to resources is conditioned by norms of justice prevalent in the host society. Findings from this special section on acculturation are combined with previous literature to support the three claims advanced in this article.

Abstract: Child behavior problems have been identified as being responsible for the greatest reduction in quality of life for children between ages 1 and 19. In this study, we examine whether neighborhood social processes are associated with differences in child behavior problems in an economically and racially diverse sample of 405 urban-dwelling first grade children and whether parenting behavior mediates and/or moderates the effects of neighborhoods. Furthermore, we examine whether neighborhood social processes play the same role with regards to child behavior problems at differing levels of neighborhood economic impoverishment. Results of multivariate multilevel regression analyses indicate that a high negative social climate is associated with greater internalizing problems. High potential for community involvement for children in the neighborhood was associated with fewer behavior problems, but only in economically impoverished neighborhoods. Differences in parenting behavior did not appear to mediate neighborhood effects on behavior problems, and parenting characterized by a high degree of positive involvement was associated with fewer behavior problems in all types of neighborhoods.

Abstract: Family interventions in schizophrenia have shown positive effects on patients but little attention has been paid to their effects on family members, particularly those in non-Western countries. This randomized controlled trial evaluated the effectiveness of a bi-weekly, 12-session, family-led mutual support group for Chinese caregivers of schizophrenia sufferers over 6 months compared with standard psychiatric care. It was conducted with 76 families of outpatients with schizophrenia in Hong Kong of whom 38 were assigned randomly to either a mutual support group or standard care. Families’ psychosocial health status and patients’ symptom severity and length of re-hospitalizations at recruitment, one-week and 12-month post-intervention were compared between groups. Results of repeated-measures mixed model indicated that the mutual support group experienced significantly greater improvements in families’ burden, functioning and number of support persons and length of patients’ re-hospitalizations at two post-tests. The findings provide evidence that mutual support groups can be an effective family-initiated, community-based intervention for Chinese schizophrenia sufferers.

Abstract: This study examined whether street homelessness, sheltered homelessness, and the severity of psychological symptoms predicted non-violent and violent crime among 207 mentally ill participants who were homeless at baseline. Participants were interviewed at 9 time points over 4 years. Hierarchical linear modeling (HLM) was used to examine whether changes in homelessness status and symptom severity predicted changes in criminal activity over time. Results indicated that homelessness both on the streets and in shelters and psychological symptom severity predicted increases in non-violent crime. Sheltered homelessness and symptom severity predicted increases in violent crime, although street homelessness did not. A separate mediational analysis with 181 participants showed that the relationship between diagnosis of a psychotic disorder and both non-violent and violent criminal activity was partially mediated through the severity of psychotic symptoms. Implications for research and intervention are discussed.

Abstract: Theory and research suggest that there is a basic and irreducible human yearning for a relationship with something that transcends ourselves, something sacred. The sacred can be understood not only in individual terms, but also in terms of relationships, settings, and communities. Empirical studies indicate that the sacred has powerful implications for human behavior; it can be an organizing force and a resource to people in their most difficult times, yet it can also be a source of seemingly intractable problems. This paper reviews several of the promising steps psychologists have taken to learn about the sacred, learn from spiritual communities, and collaborate with these communities in efforts to better the world. Research and practice in this area is enriching our understanding of the meaning of community and the meaning of spirituality.

Abstract: This research investigated youth leadership experiences of adolescent girls who participated in a comprehensive feminist-based leadership program. This qualitative study utilized a grounded theory approach to understand changes that occurred in 10 female adolescent participants. The words of the participants revealed that initially they viewed leadership in traditional terms and were hesitant to identify themselves as leaders or to see themselves included within their concepts of leadership. Following the program their view of leadership expanded and diversified in a manner that allowed for inclusion of themselves within it. They spoke with greater strength and confidence and felt better positioned and inspired to act as leaders. Participants identified having examples of women leaders, adopting multiple concepts of leadership, and participating in an environment of mutual respect and trust as factors that contributed to their expanded conceptualization.

Using the domestic violence movement as a case study, this study undertakes a close analysis of advocates' narratives about the phenomenon of domestic violence. This analysis sheds light on the current status of the movement as a social change movement attempting to promote alternative understandings of domestic violence as a social, rather than individual, problem. Study findings provide some evidence that the domestic violence movement has become increasingly de-politicized by documenting a range of narratives that convey an apolitical, degendered, individual-level analysis of domestic violence.

Abstract: This article illustrates ideas for bridging science and practice generated during the Division of Violence Prevention's (DVP) dissemination/implementation planning process. The difficulty of moving what is known about what works into broader use is near universal, and this planning process pushed us to look beyond the common explanations (e.g., providers were resistant/unwilling to change practice) and think about the multiple layers and systems involved. As part of this planning process, the Interactive Systems Framework for Dissemination and Implementation (ISF) was developed and then applied to the fields of child maltreatment and youth violence prevention. Challenges for each of the three systems in the ISF are discussed as well as and action and research ideas to address the challenges. Also described are actions taken by DVP in response to the planning process to illustrate how a funder can use the ISF to bridge science and practice.

Abstract: In the field of teen pregnancy prevention many efficacious prevention programs are available but adoption of these programs is slow at the community level. In this article, we present a multi-site, capacity-building effort called the Promoting Science-based Approaches to Teen Pregnancy Prevention project (PSBA) as a case example of a proactive application of the Interactive System Framework (ISF) for dissemination and implementation. The ISF is a multi-system model leading to dissemination and implementation of science-based prevention programming through the work of three interactive systems: The “Prevention Delivery,” “Prevention Support,” and “Prevention Synthesis & Translation” Systems. This article describes the proactive use of the ISF to conceptualize and bolster the PSBA program’s goal of assisting local prevention partners in the use of science-based approaches (SBA) to prevent teen pregnancy. PSBA uses all three systems of the ISF to facilitate practice improvements and offers valuable research opportunities to investigate factors related to dissemination and implementation processes across these systems. Describing our application of this framework highlights the feasibility of actively using the ISF to build prevention infrastructure and to guide large-scale prevention promotion strategies in the area of teen pregnancy prevention. The program’s ongoing evaluation is presented as an example of early efforts to develop an evidence base around the ISF. Research implications are discussed.

Abstract: There has been a vast amount of research on the changes experienced by immigrants, but little is known about the changes experienced by host individuals. This article focuses on the role of host individuals in the networks of relations between immigrant populations and the communities from the dominant culture, as well as the changes experienced by host individuals because of their continuous contact with immigrants. This research applied a network approach to the study of the acculturation of host individuals. Two independent studies were carried out: a systematic analysis of the personal networks of Argentinean (n = 67), Ecuadorian (n = 59), Italian (n = 37) and German (n = 37) residents in Seville and Cadiz (Spain) (Study 1); and an ethnographic study with human service workers for Latin American immigrants in Boston (USA) (Study 2). With two different strategies, the role of host individuals in personal networks of foreigners in the United States and Spain was analyzed. The results show that host individuals tend to have less centrality than compatriots, showing an overall secondary role in the personal networks of immigrants. The lowest average centrality was observed in recent and temporal migrants, whereas the highest corresponded to the individuals with more time of residence in Spain. The personal networks of human service providers in the United States vary in ethnic composition and in their structural properties, and therefore shape different types of integrative bridges for immigrants.

Abstract: Qualitative case study findings are presented. We examined whether public participation in a hazardous waste dispute manifested in ways consistent with theories of social power; particularly whether participatory processes or participants’ experiences of them were consistent with the three-dimensional view of power (Gaventa, Power and powerlessness: quiescence and rebellion in an appalacian valley, 1980; Lukes, Power: A radical view, 1974; Parenti, Power and the powerless, 1978). Findings from four data sources collected over 3 years revealed that participatory processes manifested in ways consistent with theories of power, and participants’ experiences reflected this. Results illustrated how participation was limited and how citizen influence could be manipulated via control of resources, barriers to participation, agenda setting, and shaping conceptions about what participation was possible. Implications for community research and policy related to participation in hazardous waste disputes are discussed.

Abstract: Efforts to promote systems change frequently involve the creation of councils, coalitions, and other collaborative settings. However, research, to date, reports limited empirical evidence that they achieve desired outcomes (Roussos and Fawcett, Annu Rev Public Health 21:369–402, 2000). The precise nature of this evidence base has received less attention. In particular, formal investigations into council effectiveness (a) rarely highlight the specific nature of collaborative efforts; (b) emphasize fairly distal markers as the “gold standard” for effectiveness; (c) focus largely on formative “outcomes” (e.g., action plan quality); and (d) utilize primarily quantitative research approaches. The current study extends previous research by employing a qualitative approach to investigate the particular activities and proximal outcomes of 41 domestic violence coordinating councils. Study findings suggest that councils engage in six primary activities: discussing issues, sharing information, identifying weaknesses in the system’s response, providing training for key stakeholders, engaging in public/community education, and lobbying key stakeholders who are not council members. Three proximal outcomes were consistently identified in council efforts: the promotion of knowledge, relationships, and institutionalized change. Attending more directly to proximal outcomes and concrete activities in our research has important implications for conceptualizing and researching the effectiveness of councils and collaborative settings.
