XI. STRATEGIES FOR IMPROVING DIABETES CARE D iabetes is a chronic illness that requires continuing medical care and patient self-management education to prevent acute complications and to reduce the risk of long-term complications. Diabetes care is complex and requires that many issues, beyond glycemic control, be addressed. A large body of evidence exists that supports a range of interventions to improve diabetes outcomes. These standards of care are intended to provide clinicians, patients, researchers, payors, and other interested individuals with the components of diabetes care, treatment goals, and tools to evaluate the quality of care. While individual preferences, comorbidities, and other patient factors may require modification of goals, targets that are desirable for most patients with diabetes are provided. These standards are not intended to preclude more extensive evaluation and management of the patient by other specialists as needed. For more detailed information, refer to Bode (Ed.): Medical Management of Type 1 Diabetes (1), Burant (Ed): Medical Management of Type 2 Diabetes (2), and Klingensmith (Ed): Intensive Diabetes Management (3). The recommendations included are diagnostic and therapeutic actions that are known or believed to favorably affect health outcomes of patients with diabetes. A grading system (Table 1), developed by the American Diabetes Association (ADA) and modeled after existing methods, was utilized to clarify and codify the evidence that forms the basis for the recommendations. The level of evidence that supports each recommendation is listed after each recommendation using the letters A, B, C, or E.

Aggregation-induced emission (AIE) refers to a photophysical phenomenon shown by a group of luminogenic materials that are non-emissive when they are dissolved in good solvents as molecules but become highly luminescent when they are clustered in poor solvents or solid state as aggregates. In this Review we summarize the recent progresses made in the area of AIE research. We conduct mechanistic analyses of the AIE processes, unify the restriction of intramolecular motions (RIM) as the main cause for the AIE effects, and derive RIM-based molecular engineering strategies for the design of new AIE luminogens (AIEgens). Typical examples of the newly developed AIEgens and their high-tech applications as optoelectronic materials, chemical sensors and biomedical probes are presented and discussed.

Thermokarst lakes result from the thawing of ice-rich permafrost and are widespread across northern landscapes. These waters are strong emitters of methane, especially in permafrost peatland regions, where they are stained black by high concentrations of dissolved organic matter (DOM). In the present study, we aimed to structurally characterize the DOM from a set of peatland thermokarst lakes that are known to be intense sites of microbial decomposition and methane emission. Samples were collected at different depths from three thermokarst lakes in the Sasapimakwananisikw (SAS) River valley near the eastern Hudson Bay community of Kuujjuarapik–Whapmagoostui (Nunavik, Canada). Samples were analyzed by spectrofluorometry, Fourier-transform infrared spectroscopy (FTIR), nuclear magnetic resonance spectroscopy (NMR), and elemental analysis. Fluorescence analyses indicated considerable amounts of autochthonous DOM in the surface waters of one of SAS 1A, indicating a strong bioavailability of labile DOM, and consequently a greater methanogenic potential. The three lakes differed in their chemical composition and diversity, suggesting various DOM transformations phenomena. The usefulness of complementary analytical approaches to characterize the complex mixture of DOM in permafrost peatland waters cannot be overlooked, representing a first step towards greater comprehension of the organic geochemical properties of these permafrost-derived systems.

Structural Characterization of Dissolved Organic Matter in Permafrost Peatland Lakes

A new synthesis of 2,3-diarylacridin-9(10 H)-onesand ( E)-2-phenyl-4-styrylfuro[3,2- C]quinolines is described. Thiswas accomplished by the Heck reaction of ( E)-3-iodo-2-styrylquinolin-4(1 H)-ones with styrene, leading to ( E, E)-2,3-distyrylquinolin-4(1 H)-ones, which when heated at high temperatures,cyclise in two different ways. Electrocyclisation and further insitu oxidation leads to 2,3-diarylacridin-9(10 H)-onesand tautomerisation, cyclisation by nucleophilic addition and furtherin situ oxidation produces ( E)-2-phenyl-4-styrylfuro[3,2- C]quinolines.
