2015 ESC/ERS Guidelines for the diagnosis and treatment of pulmonary hypertension: The Joint Task Force for the Diagnosis and Treatment of Pulmonary Hypertension of the European Society of Cardiology (ESC) and the European Respiratory Society (ERS): Endorsed by: Association for European Paediatric and Congenital Cardiology (AEPC), International Society for Heart and Lung Transplantation (ISHLT).

This paper presents a numerical investigation of the ignition and combustion process of a primary reference fuel in a partially premixed light-duty internal combustion (PPC) engine. Partially pre-mixed combustion is achieved by employing a multiple injection strategy with three short injection events of fuel pulses. The timing of the first two fuel pulses, 48 and 22 crank angle degrees before top dead center, are chosen with the purpose to stratify the fuel and air charge, whereas the third injection, at five crank angle degrees before top dead center, serves as an actuator of the main heat release. In addition to this baseline injection, three alternative injection strategies are studied, including a split-fuel two-injection strategy and modified triple-injection strategies. Large eddy simulations are employed utilizing a skeletal chemical kinetic mechanism for primary reference fuel capable of capturing the low-temperature ignition and the high temperature combustion. The large eddy simulation (LES) results are compared with experiments in an optical accessible engine. The results indicate that the first ignition sites are in the bowl region where the temperature is relatively higher, and the reaction fronts thereafter propagate in the swirl direction and towards the centerline of the cylinder. The charge from the first two injections initially undergoes low-temperature reactions and thereafter high-temperature reservoirs are formed in the bowl region. The main heat-release is initiated in the engine when the fuel from the third injection reaches the high-temperature reservoirs. Finally, the remaining fuel in the lean mixtures from the first two injections is oxidized. By variation of the injection strategy, two trends are identified: (1) by removing the second injection a higher intake temperature is required to enable the ignition of the charge, and (2) by retarding second injection, a longer ignition delay is identified. Both can be explained by the stratification of fuel and air mixture, and the resulting reactivity in various equivalence ratio and temperature ranges. The LES results reveal the details of the charge stratification and the subsequent heat release process. The present results indicate a rather high sensitivity of partially premixed combustion process to the injection strategies.

The role of multiple injections on combustion in a light-duty PPC engine

BACKGROUND
This study examined the effects of regulated and controlled supramaximal high-intensity interval training (HIT) adapted for older adults, compared to moderate-intensity training (MIT), on cardiorespiratory fitness; cognitive, cardiovascular, and muscular function; and quality of life.


METHODS
Sixty-eight nonexercising older adults (66-79 years, 44% males) were randomized to 3 months of twice-weekly HIT (20-minute session including 10 × 6-second intervals) or MIT (40-minute session including 3 × 8-minute intervals) on stationary bicycles in an ordinary gym setting. Individualized target intensity was watt controlled with a standardized pedaling cadence and individual adjustment of the resistance load. Primary outcomes were cardiorespiratory fitness (V̇o2peak) and global cognitive function (unit-weighted composite).


RESULTS
V̇o2peak increased significantly (mean 1.38 mL/kg/min, 95% CI [0.77, 1.98]), with no between-group difference (mean difference 0.05 [-1.17, 1.25]). Global cognition did not improve (0.02 [-0.05, 0.09]), nor differed between groups (0.11 [-0.03, 0.24]). Significant between-group differences in change were observed for working memory (0.32 [0.01, 0.64]), and maximal isometric knee extensor muscle strength (0.07 N·m/kg [0.003, 0.137]), both in favor of HIT. Irrespective of the group, there was a negative change in episodic memory (-0.15 [-0.28, -0.02]), a positive change in visuospatial ability (0.26 [0.08, 0.44]), and a decrease in systolic (-2.09 mmHg [-3.54, -0.64]) and diastolic (-1.27 mmHg [-2.31, -0.25]) blood pressure.


CONCLUSIONS
In nonexercising older adults, 3 months of watt-controlled supramaximal HIT improved cardiorespiratory fitness and cardiovascular function to a similar extent as MIT, despite half the training time. In favor of HIT, there was an improvement in muscular function and a potential domain-specific effect on working memory.

Effects of Controlled Supramaximal High-Intensity Interval Training on Cardiorespiratory Fitness and Global Cognitive Function in Older Adults: The Umeå HIT Study-A Randomized Controlled Trial.

AIMS The aim of this cross-sectional study was to examine the prevalence of the multidimensional phenomenon of fatigue in adults with congenital heart disease. BACKGROUND Adults with congenital heart disease are a growing population, and patient-reported outcomes can provide valuable information about the patient's experience of living with CHD. Fatigue is a multidimensional phenomenon that can be described as an overwhelming feeling of exhaustion with a reduced capacity of mental and physical work. Fatigue can be observed clinically in adults with congenital heart disease, but the actual prevalence is unknown. METHODS Fatigue was assessed by the Multidimensional Fatigue Inventory which enables the respondent to report the presence of fatigue according to five dimensions: "general fatigue," "physical fatigue," "mental fatigue," "reduced motivation," and "reduced activity." The questionnaire was sent to 463 patients in Lund and Umea. Four groups with complex CHD and two groups with moderately complex CHD were included. The reliability (internal consistency) of the Multidimensional Fatigue Inventory was tested for all dimensions and groups of diagnosis. RESULTS The response rate was 56.6% (n= 262). In patients with complex CHD, 40.0-59.4% reported severe to very severe general fatigue, and patients with a single ventricle reported the highest prevalence (59.4%). Among patients with complex CHD, 29.2-40.0% reported severe to very severe mental fatigue. The Multidimensional Fatigue Inventory had a high reliability measured with Cronbach's alpha. CONCLUSIONS The study findings show a relatively high prevalence of fatigue in adults with congenital heart disease, and general fatigue was the most prevalent. Further studies are needed regarding fatigue and its causes and consequences in adults with congenital heart disease. RELEVANCE TO CLINICAL PRACTICE The Multidimensional Fatigue Inventory proved to be an instrument with high reliability and low internal loss, which suggests that the instrument may be suitable to use as a patient-reported outcome in the care of adults with congenital heart disease, preferably at repeated occasions.
