Abstract: Allergic rhinitis (AR) and asthma represent global health problems for all age groups. Asthma and rhinitis frequently coexist in the same subjects. Allergic Rhinitis and its Impact on Asthma (ARIA) was initiated during a World Health Organization workshop in 1999 (published in 2001). ARIA has reclassified AR as mild/moderate-severe and intermittent/persistent. This classification closely reflects patients' needs and underlines the close relationship between rhinitis and asthma. Patients, clinicians, and other health care professionals are confronted with various treatment choices for the management of AR. This contributes to considerable variation in clinical practice, and worldwide, patients, clinicians, and other health care professionals are faced with uncertainty about the relative merits and downsides of the various treatment options. In its 2010 Revision, ARIA developed clinical practice guidelines for the management of AR and asthma comorbidities based on the Grading of Recommendation, Assessment, Development and Evaluation (GRADE) system. ARIA is disseminated and implemented in more than 50 countries of the world. Ten years after the publication of the ARIA World Health Organization workshop report, it is important to make a summary of its achievements and identify the still unmet clinical, research, and implementation needs to strengthen the 2011 European Union Priority on allergy and asthma in children.

Abstract: Solitary plasmacytoma (SP) is characterized by a mass of neoplastic monoclonal plasma cells in either bone (SBP) or soft tissue without evidence of systemic disease attributing to myeloma. Biopsy confirmation of a monoclonal plasma cell infiltration from a single site is required for diagnosis. The common presentation of SBP is in the axial skeleton, whereas the extramedullary plasmacytoma (EMP) is usually seen in the head and neck. The ratio of SP seen at males to females is 2 : 1 and the median age of patients is 55 years. The incidence rate of SP in black race is approximately 30% higher than the white race. Incidence rate increases exponentially by advancing age. SBP has a significant higher risk for progression to myeloma, and the choice of treatment is radiotherapy (RT) that is applied with curative intent at min. 4000 cGy. By only RT application, long-term disease-free survival (DFS) is possible for approximately 30% of patients with SBP and 65% of patients with EMP.

Abstract: Silicate (13-93) and borate (13-93B3) bioactive glass scaffolds were created by robotic deposition (robocasting) of organic solvent-based suspensions and evaluated in vitro for potential application in bone repair. Suspensions (inks) were developed, characterized, and deposited layer-by-layer to form three-dimensional scaffolds with a grid-like microstructure (porosity ≈50%; pore width 420 ± 30 μm). The mechanical response of the scaffolds was tested in compression, and the conversion of the glass to hydroxyapatite (HA)-like material in a simulated body fluid (SBF) was evaluated. As fabricated, the 13-93 scaffolds had a compressive strength 142 ± 20 MPa, comparable to the strength of human cortical bone, while the strength of the 13-93B3 scaffolds (65 ± 11 MPa), was far higher than that for trabecular bone. When immersed in SBF, the borate 13-93B3 scaffolds converted faster than the silicate 13-93 scaffolds to an HA-like material, but they also showed a sharper decrease in strength with immersion time. Based on their high compressive strength and bioactivity, the scaffolds fabricated in this work by robocasting could have potential application in the repair of load-bearing bone.

Abstract: The aim of this study was to evaluate the effects of 820-nm diode laser on osteoclastic and osteoblastic cell proliferation-activity and RANKL/OPG release during orthodontic tooth movement. Thirty-eight albino Wistar rats were used for this experiment. Maxillary incisors of the subjects were moved orthodontically by a helical spring with force of 20 g. An 820-nm Ga-Al-As diode laser with an output power of 100 mW and a fiber probe with spot size of 2 mm in diameter were used for laser treatment and irradiations were performed on 5 points at the distal side of the tooth root on the first, second, and 3rd days of the experiment. Total laser energy of 54 J (100 mW, 3.18 W/cm(2), 1717.2 J/cm(2)) was applied to group II and a total of 15 J (100 mW, 3.18 W/cm(2), 477 J/cm(2)) to group III. The experiment lasted for 8 days. The number of osteoclasts, osteoblasts, inflammatory cells and capillaries, and new bone formation were evaluated histologically. Besides immunohistochemical staining of PCNA, RANKL and OPG were also performed. No statistical difference was found for the amount of tooth movement in between the control and study groups (p > 0.05). The number of osteoclasts, osteoblasts, inflammatory cells, capillary vascularization, and new bone formation were found to be increased significantly in group II (p < 0.05). Immunohistochemical staining findings showed that RANKL immunoreactivity was stronger in group II than in the other groups. As to OPG immunoreactivity, no difference was found between the groups. Immunohistochemical parameters were higher in group III than in group I, while both were lower than group II. On the basis of these findings, low-level laser irradiation accelerates the bone remodeling process by stimulating osteoblastic and osteoclastic cell proliferation and function during orthodontic tooth movement.

Abstract: Concepts of disease severity, activity, control and responsiveness to treatment are linked but different. Severity refers to the loss of function of the organs induced by the disease process or to the occurrence of severe acute exacerbations. Severity may vary over time and needs regular follow-up. Control is the degree to which therapy goals are currently met. These concepts have evolved over time for asthma in guidelines, task forces or consensus meetings. The aim of this paper is to generalize the approach of the uniform definition of severe asthma presented to WHO for chronic allergic and associated diseases (rhinitis, chronic rhinosinusitis, chronic urticaria and atopic dermatitis) in order to have a uniform definition of severity, control and risk, usable in most situations. It is based on the appropriate diagnosis, availability and accessibility of treatments, treatment responsiveness and associated factors such as comorbidities and risk factors. This uniform definition will allow a better definition of the phenotypes of severe allergic (and related) diseases for clinical practice, research (including epidemiology), public health purposes, education and the discovery of novel therapies.

Abstract: Background: The aim of the present study is to investigate gingival crevicular fluid (GCF) and plasma acute-phase cytokines, interleukin-1β (IL-1β), interleukin-6 (IL-6), interleukin-11 (IL-11), oncostatin M (OSM), and leukemia inhibitory factor (LIF) levels in patients with different periodontal diseases.Methods: Eighty individuals were included in this study; 20 with chronic periodontitis (CP), 20 with generalized aggressive periodontitis (GAgP), 20 with gingivitis, and 20 classified as healthy (H). Probing depth, clinical attachment level, plaque index, and papilla bleeding index were recorded. Plasma and GCF IL-1β, IL-6, IL-11, OSM, and LIF levels were analyzed by enzyme-linked immunosorbent assay.Results: CP and GAgP groups had significantly higher GCF IL-1β, IL-6, and IL-11 levels when compared with the H group (P <0.05). Abstract: Recovery of social functioning is a largely unattained goal in schizophrenia rehabilitation. In the recent past, new neurocognitive and social cognitive training approaches have been introduced to improve functioning in various domains of patients' social life. These programs have neglected, to some degree, the social environment in which the training takes place. Accordingly, the present study sought to examine if family-assisted social cognitive training could improve quality of life, social functioning and social cognition in schizophrenia patients as compared to a social stimulation approach. In a randomized, controlled, parallel group trial design with two groups, one receiving family-assisted social cognitive training once a week (F-SCIT) and the other, social stimulation once every three weeks (SS), both for 14-weeks period, patients were assessed at baseline, before randomization and 16weeks after randomization. Participants were recruited from Celal Bayar University Psychosis Unit and were in a clinically stable condition. Patients who received F-SCIT significantly improved in quality of life, social functioning and social cognition, whereas the SS group worsened in nearly all outcome variables. Family-assisted SCIT is effective in improving quality of life, social functioning and social cognition.

Abstract: PURPOSE The aim of this study was to evaluate serum levels of leptin, ghrelin, and adiponectin in obese and non-obese children with asthma and in healthy non-asthmatic children, and analyze their relationships with clinical outcomes. METHODS This study enrolled 40 obese and 51 non-obese children with asthma and 20 healthy children. Body mass index and serum leptin, ghrelin, and adiponectin levels were determined in all children. Asthma symptom scores and lung function test results were recorded for subjects with asthma. RESULTS Serum leptin levels (11.8±7.9, 5.3±6.8, and 2.1±2.4 ng/mL in the obese asthmatic, non-obese asthmatic, and control groups, respectively) and adiponectin levels (12,586.2±3,724.1; 18,089.3±6,452.3; and 20,297.5±3,680.7 ng/mL, respectively) differed significantly among the groups (P<0.001 for all). Mean ghrelin levels were 196.1±96.8 and 311.9±352.8 pg/mL in the obese and non-obese asthmatic groups, respectively, and 348.8±146.4 pg/mL in the control group (P=0.001). The asthma symptom score was significantly higher in the obese children with asthma than in the non-obese children with asthma (P<0.001). Leptin and adiponectin levels were correlated with the asthma symptom score in non-obese children with asthma (r=0.34 and r=-0.62, respectively). CONCLUSIONS Obesity leads to more severe asthma symptoms in children. Moreover, leptin, adiponectin, and ghrelin may play important roles in the inflammatory pathogenesis of asthma and obesity co-morbidity.

Abstract: Objectives The aim of the study was to assess the symptoms of women and to describe the relationship between women's attitude towards menopause and quality of life.Methods This study was conducted from November 2010 to April 2011 at Celal Bayar University Faculty of Medicine and 494 women agreed to participate in the study. Women's attitudes towards menopause and quality of life were evaluated by Menopause-specific Quality of Life (MENQOL) questionnaires.Results Women with no education had the highest scores for vasomotor, psychosocial and physical cluster symptoms (4.1 ± 2.0, 3.1 ± 1.6, 3.2 ± 1.4, respectively). Vasomotor (4.0 ± 1.9) and physical quality-of-life scores (3.2 ± 1.40) were significantly higher among housewives. Psychosocial (3.6 ± 1.8) and physical scores (3.5 ± 1.5) of women who had no health insurance were found to be higher. Abstract: A simple and effective biodegradable material known as chitosan-clay composite beads were prepared to remove Pb(II) ions from aqueous solution. For this purpose, various important parameters such as contact time, pH and temperature were examined on the adsorption of Pb(II) ions onto crosslinked chitosan-clay composite beads. Maximum adsorption capacity of Pb(II) was observed at pH 4.5 and 25°C and calculated as 7.93 mg/g according to Langmuir isotherm model. Thermodynamic parameters namely ΔG°, ΔH° and ΔS° of the Pb(II) adsorption process have been calculated as 7.889 kJ/mol, −15.131 kJ/mol and −0.0785 kJ/molK respectively. EDTA was the best eluent for the desorption of Pb(II) ions from the crosslinked chitosan-clay beads. Scanning electron microscope (SEM) was used to characterize the surface morphology of the crosslinked chitosan-clay beads.

Abstract: Fossil fuel reserves (coal, oil, natural gas, etc.) diminish day by day. In addition, rapid advancement of technology causes an increase in the amount of energy needed. This emerging situation resulted in ever increasing importance of renewable energy sources, and various systems are being developed to utilize these renewable energy sources effectively. In this study, information about Aydin–Salavatli geothermal field’s features and working principles of the 2 power plants (DORA 1 and DORA 2) in the region are given. Power plants’ energy and exergy efficiencies were calculated with the emphasis on the effects of thermal fluids used in power plants. Also, in this study, some correlations were developed. Power plants’ sections which cause exergy losses were identified. Accordingly, improvement suggestions were presented in this study. Additionally, exergoeconomic analyses were conducted while power plants’ investment costs and equipment maintenance costs were taken into consideration.

Abstract: This paper presents records extending or confirming the distribution of Mediterranean species. Three alien algae are included, namely Codium taylorii reported for the first time from the Aegean and Turkey (Izmir Gulf), Caulerpa racemosa var. cylindracea (Karpathos and Chalki Isl., Aegean Sea) and Ganonema farinosum (Karpathos Isl., Aegean Sea). As far as animals are concerned, Litarachna divergens (Acari: Hydrachnidia) was recorded (Side, Eastern Mediterranean) and represents a new amendment at genus level for Turkish fauna. Other invertebrates include alien species such as the crabs Dyspanopeus sayi (Lago Fusaro, SW Italy), Percnon gibbesi (Larnaca, Cyprus; Karpathos and Chalki Isl., Aegean Sea) and Callinectes sapidus (Voda estuary, NW Greece), the nudibranch Aplysia dactylomela (Boka Kotorska Bay, Montenegro), the gastropod Conomurex persicus (Karpathos and Chalki Isl., Aegean Sea) and the bryozoan Electra tenella (Livorno harbour and Messina Straits area). The alien fish Siganus luridus, Siganus rivulatus, Fistularia commersonii, Sphyraena chrysotaenia and Sargocentron rubrum are also reported from the islands of Karpathos and Chalki, and Pteragogus pelycus from Heraklion Bay, Crete. In addition, new localities for four rare Mediterranean inhabitants are given: the cephalopod Thysanoteuthis rhombus (NW Sardinia) and the fish: Lampris guttatus (Calabria, S Italy), Petromyzon marinus (Gokova Bay) and Remora australis (Saronikos Gulf), while the opisthobranch gastropod Cerberilla bernadettae is reported for the first time from the E Mediterranean (Cyprus). Finally, three species of the Aegean ascidiofauna are recorded for the first time: Lissoclinum perforatum, Ciona roulei and Ecteinascidia turbinata . Furthermore, it was established that Phallusia nigra has extended its distributional range to the north of the Aegean Sea.

Abstract: The role of dietary supplemental mannan oligosaccharide (MOS) and an essential oil blend (EOB) on performance of laying hens, and susceptibility of egg yolk and hen liver to lipid oxidation were examined. Four hundred and thirty-two 52-week old Lohmann laying hens were divided into three groups and fed a basal diet containing no antioxidant as control (CNT), basal diet plus 1 g/kg MOS and basal diet with 24 mg/kg EOB, for a 10-week experimental period. Supplementation of diet with MOS and EOB improved egg production rate and eggshell weight, but did not influence other performance or egg quality traits. MOS and EOB provided higher antioxidant activity in egg yolk than the control regimen at all storage time periods. EOB also retained the oxidative stability of liver by measuring malondialdehyde (MDA) levels. Liver antioxidant enzymes, including superoxide dismutase (SOD) and glutathion peroxidase (GSH-Px), were higher in birds fed the additives. MOS and EOB tended to increase serum glucose concentration (6.2% and 8.8%, respectively) while they slightly decreased triglycerides (11.0% and 4.8%, respectively) without affecting cholesterol level. Relative weight of pancreas and spleen were not affected by dietary treatments whereas diet supplemented with EOB significantly increased liver weight. The findings of this study suggest that EOB and MOS could act as free radical scavengers that enhance performance and also increase eggshell weight.

Abstract: A web-based application patient follow-up program was developed to create a registry of patients with ankylosing spondylitis (AS) by the Turkiye Romatizma Arastirma Savas Dernegi (TRASD) AS Study Group. This study describes the methodological background and patient characteristics. The patient follow-up program is a web-based questionnaire, which contains sections on socio-demographic data, anamnesis, personal and family history, systemic and musculoskeletal examination, laboratory and imaging data and treatment. Between October 1, 2007 and February 28, 2009, 1,381 patients from 41 centers were included in the registry (1,038 males [75.2%]; mean age 39.5 ± 10.7 years). Mean disease duration was 12.1 ± 8.5 years, and mean time from initial symptom to diagnosis was 5 ± 6.8 years (median 2 years). HLA-B27 positivity was detected in 73.7% of 262 patients tested. Manifestations of extraarticular involvement were anterior uveitis (13.2%), psoriasis and other skin and mucous membrane lesions (6%) and inflammatory bowel disease (3.8%). The prevalence of peripheral arthritis was 11.2%. In 51.7% of patients, the Bath AS Disease Activity Index was ≥4. But since our patients consisted of the ones with more severe disease who referred to the tertiary centers and needed a regular follow-up, they may not represent the general AS population. Disease-modifying anti-rheumatic drugs were being used by 41.9% of patients, with 16.4% using anti-TNF agents. TRASD-IP (Izlem Programi: Follow-up program) is the first AS registry in Turkey. Such databases are very useful and provide a basis for data collection from large numbers of subjects. TRASD-IP gives information on the clinical and demographic profiles of patients, and the efficacy and safety of anti-TNF drugs, examines the impact on quality of life, and provides real-life data that may be used in cost-effectiveness analyses.

Abstract: There are not enough studies that describe the isokinetic strength of professional soccer players at high angular velocities. The purpose of this study was to evaluate the seasonal changes in isokinetic strength of Turkish professional soccer players (n=14) over the course of a 24-week soccer season. The isokinetic strength of players who underwent usual soccer training and weekly competition throughout the soccer season was assessed by means of the Biodex System 3 dynamometer with the knee attachment. The peak torque of knee extensor and flexor muscles were measured at angular velocities of 60°/s, 300°/s and 500°/s. Players were tested at the beginning and end of the competitive season. While the first- and second-test measurements did not show significant changes at 60°/s and 300°/s angular velocities, at the end of the training period, players’ knee strength changed significantly at 500°/s angular velocities. In addition, the H/Q ratio improved significantly for the dominant as well as non-dominant leg at 500°/s. Significant bilateral strength improvements for knee flexors were also observed at 500°/s. The findings of this study suggest that usual daily soccer training (technical, tactical, power, strength, endurance, flexibility, etc.) and weekly competition might produce changes in knee strength at high angular velocities.
