TL;DR: In this article, the authors examined the epidemiology, demographics, clinical presentation, and treatment of inflammatory bowel disease (IBD) in a predominantly Hispanic cohort in Los Angeles (LA) County.
Abstract: Previous studies have suggested that inflammatory bowel disease (IBD) occurs at higher rates among non-Hispanic Whites (NHWs) compared to other ethnicities; however, Hispanics as the largest minority in the United States remain underrepresented in IBD research and we hypothesize that they have similar rates of IBD. We examined the epidemiology, demographics, clinical presentation, and treatment of IBD in a predominantly Hispanic cohort in Los Angeles (LA) County. This was a retrospective cohort study based at Olive View-UCLA Medical Center, one of the three major safety-net hospitals in LA County. Electronic medical records from 2015 to 2018 were queried, and biopsy-proven cases of IBD (n = 170) were identified. Outcomes included the incidence and prevalence of IBD, disease distribution, treatment, and IBD-related surgery. The incidence of IBD among Hispanics was 175 (95% confidence interval [CI] 127–240) and 113 (95% CI 62–200) for NHWs per 100,000 person-years. Prevalence of IBD per 100,000 people was 418 (95% CI 341–512) for Hispanics and 557 (95% CI 431–739) for NHWs. Notably, the proportion of Hispanic IBD patients with a history of smoking was 21.5% vs 50.8% in NHWs (p = 0.011). There were no significant differences between the two groups with regard to Montreal classification, pharmacotherapy, or IBD-related surgery. In one of the largest US studies of Hispanics with IBD, and the only one to have both clinical and histopathologic confirmation as inclusion criteria, we found the incidence and prevalence of IBD among Hispanics to be higher than previously recognized and comparable to NHWs. Additionally, Hispanic IBD patients had lower rates of smoking compared to NHWs.

TL;DR: In this article, the authors present a quality improvement process for the adaptation and application of the MeNTS tool to gynecologic surgical services beyond the initial COVID response and into recovery of surgical services.
Abstract: Health systems are struggling to manage a fluctuating volume of critically ill patients with COVID-19 while continuing to provide basic surgical services and expand capacity to address operative cases delayed by the pandemic As we move forward through the next phases of the pandemic, we will need a decision-making system that allows us to remain nimble as clinicians to meet our patient's needs while also working with a new framework of healthcare operations Here, we present our quality improvement process for the adaptation and application of the Medically Necessary Time-Sensitive (MeNTS) toolto gynecologic surgical services beyond the initial COVID response and into recovery of surgical services; with analysis of the reliability of the modified-MeNTS tool in our multi-site safety net hospital network This multicenter study evaluated the gynecology surgical case volume at three tertiary acute care safety net institutions within the LA County Department of Health Services: Harbor-UCLA (HUMC), Olive View Medical Center (OVMC), and Los Angeles County + University of Southern California (LAC+USC) We describe our modified-Delphi approach to adapt the MeNTS tool in a structured fashion and its application to gynecologic surgical services Blinded reviewers engaged in a three-round iterative adaptation and final scoring utilizing the modified tool The cohort consisted of 392 female consecutive gynecology patients across three Los Angeles County Hospitals awaiting scheduled procedures in the surgical queueThe majority of patients were Latina (747%) and premenopausal (671%) Over half (524%) of the patients had cardiovascular disease, while 130% had lung disease, and 138% had diabetes The most common indications for surgery were abnormal uterine bleeding (332%), pelvic organ prolapse (196%) and presence of an adnexal mass (143%) Minimally invasive approaches via laparoscopy, robotic-assisted laparoscopy, or vaginal surgery was the predominant planned surgical route (548%) Modified-MeNTS scores assumed a normal distribution across all patients within our cohort (Median 33, Range 18-52) Overall, ICC across all three institutions demonstrated "good" interrater reliability (072) ICC within institutions at HUMC and OVMC were categorized as "good" interrater reliability, while LAC-USC interrater reliability was categorized as "excellent" (HUMC 073, OVMC 065, LAC+USC 077) The modified-MeNTS tool performed well across a range of patients and procedures with a normal distribution of scores and high reliability between raters We propose that the modified-MeNTS framework be considered as it employs quantitative methods for decision-making rather than subjective assessments

TL;DR: The sliding compression screw-sideplate combination is currently the most widely empoloyed device for internal fixation of stable and unstable intertrochanteric fractures of the femur as discussed by the authors.
Abstract: The sliding compression screw-sideplate combination is currently the most widely empoloyed device for internal fixation of stable and unstable intertrochanteric fractures of the femur. The normal and abnormal radiographic appearances of this device in the immediate post-operative period are discussed. Potential long-term complications including mal- or non-union, intra-articular penetration, metal failure, rotation of the proximal fracture fragment, disengagement, trochanteric bursitis, leg length discrepancy, delayed cervical stress fracture, and ischemic necrosis are reviewed.

TL;DR: Patients with primary hypertension follow a hierarchical pattern of hemodynamic and neurohumoral responses to laboratory and environmental mental and physical stress, and the magnitude of the neurohumorical activation can predict the effectiveness of therapy with sympatholytic and other types of drugs used to treat hypertension.
