TL;DR: NPO2‐NC‐AC is a simple and effective CPR technique for severe acute cardiopulmonary asphyxia in piglets and may be clinically applicable for supportive or rescue use.
Abstract: Summary. Objective: To evaluate the efficacy of a simple and novel cardiopulmonary resuscitation (CPR) technique, nasopharyngeal oxygen with intermittent nose-close and abdomencompression (NPO2-NC-AC) in an animal model. Design: Prospective piglet study. Setting: In an animal physiology laboratory of a medical university. Piglets: Six healthy piglets <14 days old. Interventions: Spontaneous breathing of the anesthetized piglets (n ¼ 6) was medically ceased until severe bradycardia (<20 beat/min). NPO2-NC-AC CPR trial was then initiated by delivering NPO2 at 1.0–1.5 L/kg/min and intermittently performing the NC-AC maneuver: (1) NC for inspiration, occluded nostrils for 1 sec; and (2) AC for expiration, released nostrils, and compressed the abdomen for 1 sec. NC-AC was repeated at a rate of 30/min for 5 min. This CPR trial was repeated three times in each piglet. Cardiopulmonary variables were monitored, recorded, and compared. Measurements and Main Results: A total of 18 CPR trials were performed. All of these acute life-threatening asphyxia events rapidly improved within 1 min of CPR. Cardiopulmonary variables recovered to the baseline levels and oxygenation continually increased. The intratracheal pressure (Ptr) values of positive end expiratory pressure and peak inspiratory pressure values were 4.3 0.8 cmH2O and 26.2 4.1 cmH2O, respectively. Chest wall movement was observed when the Ptr reached 17.5 3.1 cmH2O. All six piglets fully recovered after 63.8 7.2 min of experiment without additional support. Conclusions: NPO2-NC-AC is a simple and effective CPR technique for severe acute cardiopulmonary asphyxia in piglets. It may be clinically applicable for supportive

TL;DR: The majority of SAD-II ankle fractures involved marginal impaction of the tibial plafond, and plain radiographs were able to predict planned operative management after review of CT imaging with an 87% PPV and 75% NPV.
Abstract: Background:The purpose of this study was to describe the incidence of plafond impaction in supination-adduction type II (SAD-II) ankle fractures and assess the accuracy of plain radiographs in dete...

TL;DR: A unique case of Warthin tumor encased by a separate high‐grade mucoepidermoid carcinoma that was first diagnosed on FNA and the presence of chronic sialadenitis in the surrounding nonneoplastic salivary gland raises the question of a possible causal relationship.
Abstract: Fine-needle aspiration (FNA) is a cost effective and low morbidity procedure in the initial assessment of salivary gland tumors. However, cytological assessment of ipsilateral synchronous tumors (which make up less than 0.3% of all salivary gland neoplasms) may pose diagnostic challenges. Therefore, a wholesome approach, including FNA with clinical and radiological correlation, is of utmost importance. Here, we report a unique case of Warthin tumor encased by a separate high-grade mucoepidermoid carcinoma that was first diagnosed on FNA. Another striking feature seen was the presence of chronic sialadenitis in the surrounding nonneoplastic salivary gland. The presence of two different neoplasms in the background of chronic sialadenitis raises the question of a possible causal relationship. Traditionally, there has been diagnostic difficulty when dealing with synchronous tumors of the salivary gland and the background of chronic sialadenitis may further complicate the diagnosis. FNA is very helpful and can give important cues to the diagnosis. Diagn. Cytopathol. 2010. © 2009 Wiley-Liss, Inc.

TL;DR: This work compared those who experienced early post-traumatic seizures to those who did not; it found network differences months after seizure activity stopped, and examined correlations between network measures and a common measure of injury severity, the Glasgow Coma Scale.
Abstract: Traumatic brain injury (TBI) is the leading cause of death and disability in children, and can lead to long lasting functional impairment. Many factors influence outcome, but imaging studies examining effects of individual variables are limited by sample size. Roughly 20–40% of hospitalized TBI patients experience seizures, but not all of these patients go on to develop a recurrent seizure disorder. Here we examined differences in structural network connectivity in pediatric patients who had sustained a moderate-severe TBI (msTBI). We compared those who experienced early post-traumatic seizures to those who did not; we found network differences months after seizure activity stopped. We also examined correlations between network measures and a common measure of injury severity, the Glasgow Coma Scale (GCS). The global GCS score did not have a detectable relationship to brain integrity, but sub-scores of the GCS (eyes, motor, verbal) were more closely related to imaging measures.

TL;DR: It is suggested that this surgical approach is a safe and effective method of simultaneous termination of unwanted pregnancy and childbearing capacity in selected cases in which hysterectomy would be medically indicated.
