Abstract: Dr Parafulla Kumar Sen, popularly known as P K Sen was a great Bengali surgeon of Twentieth century. Regarded as the founder of cardiothoracic surgery in India, Dr Sen performed the first intracardiac operation of any kind, the first open heart surgery and also the first heart transplantation in this subcontinent. He also made very significant contribution in the development of transmyocardial revascularization, a new technique for aortic arch replacement and described a group of nonspecific vasculitis. Dr P K Sen was born in Kolkata, the then capital of undivided Bengal in 1915. But he is little known in Bangladesh. This article is an attempt to notify the deeds of this Bengali surgeon. Abstract: Background: Extended-Spectrum b-Lactamases (ESBLs) producing bacteria are increasing in number and causing more severe infections because of their continuous mutation and multidrug resistance property which make its treatment difficult. Thus reliable, sensitive and low cost method to detect ESBLs producers, therefore, is of major interest. The present study was undertaken to compare the sensitivity between double disc synergy test & phenotypic confirmatory test to detect ESBLs producing bacteria. Methods: All the isolates were identified by standard procedure of identification & isolated gram-negative bacteria initially screened by Minimum Inhibitory Concentration (MIC) ESBLs breakpoints. Then suspected ESBLs producers are confirmed by double disc synergy test & phenotypic confirmatory test. Results: In the present study, total 176(74.89%) bacterial strains were isolated from 235 samples of wound swab & pus, and urine. Among the isolates, 150(85.23%) were gram-negative and 26(14.77%) were gram-positive bacteria. The gram-negative bacteria were screened for suspected ESBLs & then subjected to confirmatory test where Phenotypic Confirmatory Test (PCT) detected 89(62.68%) and Double Disc Synergy Test (DDST) detected 74(52.11%) ESBL producers. So 15(10.57%) isolates were missed by double disc synergy test. In this study, we determined sensitivity, specificity, positive predictive value & negative predictive value of Phenotypic Confirmatory Test (PCT) were 100%, 77.9%, 83.1% & 100% respectively and those of Double Disc Synergy Test (DDST) were 83.1%, 100%, 100%, and 77.9% respectively. Conclusion: Between these two tests, phenotypic confirmatory test found to be more sensitive procedure than double disc synergy test for the detection of ESBLs producing organisms. Abstract: Myxomas are rare tumours but are the most common benign tumours of the heart. They can arise from any heart chamber. However, they arise more frequently from the left atrium. They have rarely been described as originating in early age. A case of left atrial myxoma successfully removed using cardiopulmonary bypass in a 8-year-old child is presented. Review of the literature emphasizes the rarity a nd clinically aggressive behavior of this tumor in this age group. The object of this case report is to present myxoma in children and to evaluate possible differences between young and adult patients. Abstract: This was a thirty cluster survey; carried out to identify the factors which limit ORS intake during a diarrhoeal episode in rural Bangladesh. 420 mother-child pairs were investigated. In addition to the verbal interview, the index child was examined physically and mother's skill of preparing ORS was assessed. The ORS was known to more than 95% mothers but used by only 33.3%. The average intake of ORS was 320 ml per diarrhoea day. The correct amount of oral rehydration fluid was received by 145(34.5%) cases. Twenty eight socio-demographic-maternal-host(child)-environmental factors were studied for their probable influence on intake of correct amount of oral rehydration fluid. In bivariate analysis, fourteen factors were found to be associated significantly with oral rehydration fluid intake (P < 0.05-< 0.0001). These fourteen factors were treated with stepwise logistic regression analysis. Sensitivity and specificity were the key measures for decision of best fitted model. A combination of child age, mother's education, socio-economic condition and method of feeding ORS provide the maximum sensitivity (90.20%) and specificity (74.19%) for predicting oral rehydration fluid intake. The study emphasized the need for further strengthening of health education incorporating the findings of the research to ensure required amount of oral rehydration fluid intake during diarrhoeal episode.
