Abstract: Background : The Thalassaemia is an inherited haemoglobulin disorder causes hemolytic anemia which usually requires life-long blood transfusion therapy. Therefore transfusion dependent thalassaemia major patients suffer from the effect of deposition of excess iron in the liver, heart and endocrine glands. Objective : The aim of present study was to find out the association of serum ferritin and SGPT in transfusion dependent thalassaemia major patient. Methodology : This cross sectional study was carried out in Day care unit (DCU) of Transfusion Medicine department, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, during January 2014 to December 2014. Total of 110 clinically diagnosed Thalassaemia major (TM) between (5-35 years) cases of either sex from the above mentioned department attending the DCU were selected purposively for this study. Data were collected on clinical examination findings, laboratory investigations, ABO and Rh Blood Group, Serum ferritin and SGPT (Serum glutamic pyruvic transaminase) after taking informed consent. Results : The following observations and results were obtained in this study. The mean age was found 17.15±8.77 years with range from 5 to 35 years and male to female ratio was 1.2:1 (male 59 and female 51). Sixty eight (68.0%) patients came from rural area, 43.6% patients were student, 77.3% patients had completed primary education, 57.3% of the patients came from middle class family and 72.7% patients were unmarried. About 46.4% of the patients had moderate anaemia, 28.2% had jaundice, 79.1% had hepatomegaly, 82.7% had splenomegaly and 30.9% had complication due to iron overload. Mean haemoglobulin (Hb%) was found 6.65±1.63 gm/dl, mean serum ferritin was found 3785±17637 ?g/L and mean SGPT was found 134.8±77.94 U/L. There was a significant positive correlation (r=0.259; p=0.006) between serum ferritin and SGPT. Conclusion : Considering the study result it can be concluded that there is a positive significant correlation between serum ferritin with SGPT level. Abstract: Bleeding of extradural space is called extradural haematoma. This was a prospective study. In this study included 30 patients of extradural haematoma. Mean age of patients was 29 ±15year. 26(83.3%) of patients were male and 5(16.6%) of patients were female. Commonest cause of haematoma were road traffique accidents. Among them 28(93.3%) patients were treated by surgical procedure and 2(6.6%) were treated by conservative procedure. After surgery among the 30 patients 26(83.3%). patients were fully recovered. Postoperatively two patients recovered with disabilities (one had visual field defect and one had Psychological disturbance). Two patients (6.6%) died on 3 rd and 6 th post operative day respectively due to aspiration pneumonia. Mortality and morbidity were more among those who were operated after 48 hours of occurrence. Abstract: Objectives : To make a comparative evaluation of efficacy of brachial plexus blockade between supraclavicular subclavian perivascular technique and axillary transarterial technique with a tourniquet and also to compare the latency & potency of the block, to ascertain the risks of complications between the techniques and to compare the haemodynamic stability of the patient. Methods : For this purpose a total of 100 undergoing surgery for distal to the midarm. They were randomly selected by odd and even numbering method. These patients were divided into two groups (Group A: subclavian perivascular supraclavicular technique bearing all the odds numbers and Group B: transarterial axillary technique with a tourniquet bearing all the even numbers; fifty patients in each group. Results : The only exception is the mean on set time of sensory block was 11.0±2.6 minutes and 14.3±2.1 minutes in Group A and Group B respectively. The mean on set time of motor block were 18.3±3.9 minutes in Group A and 21.4±3.2 minutes in Group B. So the mean on set time of sensory block and motor block were statistically significant (p 0.05) difference was found in the present study. Adequate efficacy of block were predominate in both groups and no significant (p>0.05) difference was found. The level of satisfaction of surgeon and it was found that 92.0% in Group A and 94.0% in Group B were satisfied. Patient’s co-operation was found 92.0% and 94.0% in Group A and Group B respectively. No significant (p>0.05) difference were found in terms of surgeon satisfaction and patients cooperation between two groups. Conclusion : It can be concluded that the supraclavicular subclavian perivascular technique and axillary transarterial technique with a tourniquet both are equally effective and safe method for providing brachial plexus block distal to midarm (Lower half of the arm, elbow, forearm and hand).
