Abstract: Aim Hypersensitivity to the anti-retroviral agents is a common problem encountered in the treatment of HIV/AIDS patients. However, in resource-limited settings, where only fixed-dose combinations of the drugs are the only available option and expensive facilities of pharmacological analysis are not available, isolating the offending agent can be a challenging task. Background We, at our centre, follow a protocol to isolate such offending drug – a protocol of gradual de-challenge and re-challenge – by a method of exclusion. Materials and Methods Here, we have described a series of four cases, providing examples of usage of our own protocol to isolate the offending drug by a method of exclusion. Results and Conclusion In a resource-limited setting, where we have to use fixed-dose combination of anti-retroviral agents, management of a patient with drug hypersensitivity is a problem. Our protocol of systematic de-challenge and re-challenge, as exemplified in these cases, can be of help where expensive facilities are not available.

Abstract: Introduction : Diagnosis of patients with severe acute pancreatitis as soon as possible is critical for achieving optimal outcomes. Management depends largely on severity. Medical treatment of mild acute pancreatitis is relatively straightforward. Treatment of severe acute pancreatitis involves intensive care. Surgical intervention (open or minimally invasive) is indicated in selected cases. Methodology : A total number of 59 subjects were enrolled in this present prospective study in the department of Radiology and Imaging, BIRDEM during two years aim to find out the correlation between CT severity index and patient serum enzyme levels in acute pancreatitis. Patients with clinically suspected and biochemically diagnosed acute pancreatitis referred to Radiology and Imaging department from department of gastroenterology (GHPD), BIRDEM for imaging investigation were enrolled in this study. Severity of acute pancreatitis was measured by both clinical and imaging staging (Modified CT severity index) in mild, moderate and severe groups. Subjects clinical information’s and serum enzymes were recorded and analyzed by computer software SPSS (Ver. 20-IBM). Result : Mean age of the study subjects was 36.43 ± 13.85 years. The age ranged from 20-63 years and the maximum number was found in 21 to 40 years age group. Male female ratio was almost 1.56:1. It was seen that diffuse pancreatic enlargement was noted in 20.33% subjects. Pancreatic inflammation with and without peripancreatic fat involvement were observed in 22.72% and 57.62% subjects respectively. Pseudocyst formation (11.86%), ascites (23.72%), renal fascia involvement (37.28%) and pleural effusion (30.5%) were seen in CT scan. The results of the interpreter analysis was Kappa = 0.852 with p < 0.001 when association of severity by clinical scoring and CT severity scoring was compared and revealed that measure of agreement, while statistically significant, was almost perfect agreement. CT severity index in acute pancreatitis had statistically significant (p is less than 0.05) relation with serum amylase (0.738), lipase (0.638), SGOT (0.581) and SGPT (0.365) during admission which was found on simple linear regression test. Conclusion : This present study concluded that modified CT severity index correlated well with patient’s enzymes level (Amylase, lipase, SGOT, SGPT) as well as clinical findings in acute pancreatitis Birdem Med J 2015; 5(1) Supplement: 18-24

Abstract: The aim of study is to elucidate the status of vitamin D level in adult patients with cardiovascular disease. Measure serum 25 (OH) Vitamin D level in adult patients diagnosed with cardio vascular disease. Lipid profile level in diagnosed patients with cardio vascular disease. Blood glucose level in diagnosed patients with cardiovascular disease. Assess those patients with ECG parameters. Compare the level of serum 25 (OH) vitamin D level in adult patients of cardio vascular disease with respect to age and sex matched apparently healthy controls. Observational, non-interventional, hospital based cross sectional study and case control studies were done for clinical correction. Patients with diagnosed Cardiovascular Disease were selected from OPD and IPD of the department of Medicine, Midnapore Medical College and Hospital, Paschim Medinipur and analysis of biochemical parameters were done in the Department of Biochemistry, Midnapore Medical College and Hospital, Paschim Medinipur. In Control, the mean Vitamin D (mean± S.D.) of patients was 40.3500± 1.9456. The mean Vitamin D was significantly higher in cases compared to control which was statistically significant. We concluded that serum vitamin D level in patients was significantly correlated with coronary artery diseases in a tertiary care hospital.

Abstract: An 11-month-old boy presented with numerous asymptomatic small bumps on his body for the preceding four months. Examination revealed multiple skin-coloured, non-tender, firm papules and nodules (ranging in diameter from 2 to 10 mm; Fig. 1) over his trunk, face and limbs sparing the palms and soles. Only a few scattered lesions were found in the flexural areas. Oral mucosa, ocular and systemic examination was noncontributory. Complete haemogram and biochemical panel including lipid profile were normal. Histopathology of a representative lesion showed a dermal infiltrate comprising of foamy histiocytes, foreign body giant cells, and a few Touton giant cells. However, no scalloping of the macrophages was found. What is the diagnosis?
