TL;DR: A case of 53-year-old immunocompetent lady who presented with quadriparesis and MRI (magnetic resonance imaging) of spine revealed multiple lesions involving cervical, thoracic, lumbar, and sacral region without any involvement of intervertebral disc.
Abstract: Tuberculosis of spine is common in a developing country like India. However, involvement of spine at multiple levels along with involvement of rib and tubercular breast abscess in an immunocompetent patient without any pulmonary involvement is extremely rare. Here we report a case of 53-year-old immunocompetent lady who presented with quadriparesis and MRI (magnetic resonance imaging) of spine revealed multiple lesions involving cervical, thoracic, lumbar, and sacral region without any involvement of intervertebral disc. On detailed examination she was found to have a lump in right breast. Fine needle aspiration cytology of both paravertebral collection and breast lump revealed presence of acid fast bacilli. She was put on antitubercular drug for one year and she responded well to therapy.

Abstract: The practice of anesthesia has changed dramatically since the days of John Snow. The modern anaesthesiologist is now both a consultant and a primary care provider. The consultant role is appropriate because the primary goal of the anesthetist- to see the patient and comfortably through surgery- generally takes only a short time. However, because anaesthesiologist manage all "noncutting" aspects of the patient's care in the immediate perioperative period, they are also primary care providers. The "Captain of the ship"doctrin, which held the surgeon responsible for every aspects of the patient's perioperative care is no longer valid. The surgeon and anesthesiologist must function together effectively, but both are ultimately answerable to the patient rather than to each other. Patient can select their own anesthesiologist, as well as the surgeon. On the other hand an anesthesiologists have every rights to choice a surgeon, or refuse giving anesthesia to a certain patients. The practice of anesthesia is no longer limited to the operating room. The anesthesiologist are now routinely asked to monitor, sedate and provide general or regional anesthesia outside the operation room. Anesthesiologist have traditionally been pioneers in cardiopulmonary resuscitation and continue to be integral members of resuscitation team. Preoperative evaluation of patients, perioperative and postoperative monitoring and care is absolutely essential. Surgeon-anesthesiologist, anesthesiologist-patient rapport/ co-ordination should be maintained. Documentation is important for both quality assurance and medico legal purpose. Preoperative and intraoperative anesthetic record must be preserved as it serves many purposes. This record should as pertinent and accurate as possible. The written consent with a witness must be taken. The postoperative notes are also essential as many of the patients files complaint against the anesthesiologist, surgeon, and hospitals. Abstract: Porokeratosis of Mibelli (PM) is a rare genodermatosis. It is caused by proliferation of abnormal clones of epidermal cells in response to several stimuli, the most important of which is sunlight. Giant porokeratosis is thought to be a variant of PM. We report two cases of disseminated PM and one case of disseminated giant porokeratosis from the Bankura and Bardhaman districts of West Bengal in India; presenting to a single observer in the summer of 2012. Interestingly we have noted that the majority of cases of porokeratosis in India have been reported from West Bengal. Few patients with a single lesion of giant porokeratosis have been reported in world literature. As far as we know, this is the first case report of disseminated giant porokeratosis in world literature.

Abstract: Background and Objective: Despite liberal abortion law in India, majority of abortions are unsafe. Behavioral theory suggests that women's attitudes may influence their abortion decisions. The present study was conducted to find out women's attitude toward abortion and its predictors. Materials and Methods: A community-based cross-sectional study was conducted in Naxalbari block of Darjeeling district, West Bengal, India, among 420 women aged 15–49 years using a predesigned, pretested interview schedule. The women's attitude regarding abortion practices was determined based on Likert's three-point scale. Two-stage cluster analysis was used to classify the overall attitude of women. Binary logistic regression was used for finding out predictors of abortion attitude. Results: More than half (53.5%) of the women had an overall pro-life attitude and 46.5% women had an overall pro-abortion attitude. Women who were less educated with
