Abstract: Pneumatosis Cystoides Intestinalis is a rare condition characterised by gas-filled cysts in the wall of the smallgut and sometimes in its mesentry. When the process is limited to the large intestine, the term pneumatosis coli is used . The exact cause of the condition is unknown but it may develop in diseases like peptic ulcer, ulcerative colitis, regional enteritis, chronic bronchitis etc. The condition is usually diagnosed incidentally during abdominal scanning or investigation for the primary disease. Presentation as a case of gastric outlet obstruction (proximal jejunal obstruction) as happened in this case, is rare. A young adult male cultivator presented as a case of gastric outlet obstruction due to suspected chronic duodenal ulcer ( a common complication of Peptic ulcer disease in our country ). Per-operatively, gas filled cysts in proximal jejunum with almost obliteration of the lumen of the intestine was detected and diagnosed as a case of pneumatosis cystoides intestinalis and was treated with resection and anastomosis of the affected portion of gut. Abstract: Introduction:Evidence suggests that the decline in ovarian function with menopause is associated with spontaneous increase in proinammatory cytokines, especially IL-1, IL-6 and TNF-α. Objective:To evaluate the possible relationship of pro-inammatory cytokines and vasomotor symptoms in menopausal women on lipid lowering drugs- statin and fenobrate.Materials &Method: A prospective randomized clinical trial was carried out in the Dept. of Obstetrics and Gynaecology, I.P.G.M.E.R., Kolkata enrolling 100 women and dividing them into six groups. They were given either atorvastatin or fenobrate or no drug. The main outcome measures were to estimate changes in cytokine levels and vasomotor symptoms and their correlation. Results: Although the cytokine levels (except TGF-β) & incidence of vasomotor symptoms increased with menopause, there was only some positive correlation between TNF-α with increasing symptom at three month postoperative period. With atorvastatin therapy, moderate positive correlation (r >0.5) exists between decreasing levels of IL-1, IL-6 and TNF-α and decreasing vasomotor symptoms & strong negative correlation [r = -0.79] PB PB in case of TGF-β in natural menopausal women. In surgical menopausal women treated with atorvastatin correlation exists between TNF-α and TGF-β levels and vasomotor symptoms.There was no correlation found between any of the cytokine level and vasomotor symptom in women given fenobrate in natural menopausal group.but correlation found between TNF-α and TGF-β levels and vasomotor symptoms in the other group of subjects (for TNF-α r =+0.35 and for TGF-β r =−0.49 ). The levels of inammatory cytokines IL-1, IL-6, TNF-α increased within PB PB Conclusion: three months of surgical menopause while the level of anti-inammatory cytokine TGF-β decreased. Atorvastatin and fenobrate- both are effective in alleviating vasomotor symptoms, though statistically insignicant. The levels of inammatory cytokines, TNF-α and TGF-β are best correlated with vasomotor symptoms in menopausal women.

Abstract: Dysregulation of dopaminergic neurotransmission has been implicated in the etiology of major psychoses. The dopamine D1 receptor (DRD1) plays a role in some brain functions and mechanisms of psychotropic drugs. Therefore, the DRD1 gene makes a good candidate gene for molecular genetic study in delusional disorder. In the present investigation, the association has been studied between DRD1 gene and delusional disorder patients. No association was found between the DRD1 gene and delusional disorder, either in the whole group of patients or in subgroups divided by disease type or predominance of DRD1 positive or negative patients. Moreover, there were no significant differences observed between the delusional disorder patients and normal healthy controls when they were compared for different clinical and demographic variables. These findings suggest that this gene may not be involved in the pathogenesis of delusional disorder.

Abstract: Objective: Childhood disabilities are common and impose a significant handicap on the affected children and their caregivers. There are limited data on the prevalence of childhood disabilities as assessed by specialist evaluation. We aimed to explore the spectrum of childhood disabilities in a cohort of rural children in North Bengal. Materials and Methods: A descriptive observational study was done in a tertiary care center of rural North Bengal over 2 years. Children aged between 2 and 12 years with some form of disability were included in the study. The demographic and socioeconomic profile and potential risk factors were recorded. Assessment was done by a multidisciplinary team involving pediatricians and faculty from other relevant disciplines. Relevant national guidelines were followed in assessing individual disabilities and their severity. Results: Four hundred and ninety-one disabled children were included in the study. The prevalence of childhood disability was approximately 2/10,000. Majority had multiple disabilities (56.62%). Sixty-one percent of the children were male, and the majority hailed from low-income families. Combination of locomotor disability and mental retardation was the most common form of disability, pure locomotor disability being the next most common. Proportions of visual and hearing impairment were 15.28% and 13.65%, respectively; 20.57% of the children had moderate to profound mental retardation, and 2.65% were diagnosed with mental illness. Consanguineous parents were noted in 12.42%, and one or more adverse perinatal events were recorded in the great majority of cases. Conclusions: This study exclusively on children by team of specialist medical personnel can serve as an index study profiling childhood disability in rural India.
