Abstract: BACKGROUND: In the developing world, more than half of infant and childhood mortality is related to childhood diseases particularly- acute respiratory infections (ARI) and diarrhoea. The situation is worse among underprivileged population such as tribals and people living in tea garden areas. AIM: To identify the morbidity pattern and the associated factors among tribal, under five, children living in tea garden areas of Darjeeling district. MATERIALS AND METHODS: A cross-sectional study was conducted in three randomly chosen tea garden areas of a block in Darjeeling District, West Bengal, India from September 2013-February 2014. The collected data was analysed using SPSS software and binary logistic regression was applied to test association between morbidity and other epidemiological correlates. RESULTS: Morbidity was noted among 74 out of 192 children studied. Major causes of morbidity were- diarrhoea (26%), acute respiratory infections (24.5%) and fever (16.7%). Proportion of underweight children according to their age was 64.4%. Morbidity status was found statistically significant with some factors, like- religion, socio-economic status, immunization status and number of siblings. CONCLUSION: There is high prevalence of diarrhoea and ARI associated morbidity in this part of the country.

Abstract: In 58 patients with acute recurrent or persistent flank pain, straight x-ray (kidney, ureter, bladder region) detected stones in the urinary tract in 50 cases (86.2%), whereas ultrasonography detected stones in the urinary tract in 55 patients (94.8%). Ultrasound also detected unilateral hydronephrosis in 20 patients (34.48%). The presence of calculus was subsequently proved by intravenous urography/surgery or spontaneous passage. Ultrasonography is safe, quick, reliable and most effective diagnostic tool in such cases in rural areas. Intravenous urogram should be reserved for cases which need surgical intervention.

Abstract: To describe the long term morbidity and functional outcome of Japanese encephalitis in children. Laboratory-confirmed Japanese encephalitis cases were enrolled in the study from January, 2016 to September, 2017 and surviving cases were prospectively followed up for 2.5 years to document various morbidities. Outcome was functionally graded at discharge and during follow-up using Liverpool outcome score. Out of 56 children enrolled, 10 (17.9%) died during hospital stay; severe sequelae was observed in 17 (30.4%) at discharge. At the end of study, among 37 children under follow-up, 23 (62.2%) recovered fully, 2 (5.4%) showed minor sequelae, 3 (8.1%) had moderate sequelae, and 9 (24.3%) were left with severe sequelae. Common long term morbidities were abnormal behavior (n=10, 27%), post encephalitic epilepsy (n=8, 21.6%), poor scholastic performance (n=8, 21.6%) and residual motor deficit (n=7, 18.9%). Improvement of morbidities was noted mostly within initial 1 year of follow-up. More than half of the Japanese encephalitis survivors recovered fully, most within the first year after discharge.

Abstract: Introduction An extremely rare case of a pair of nails impacted in the right pyriform sinus, presenting 72 hours after the incident, is reported. Case Report X-ray soft tissue neck revealed the presence of a pair of nails in the neck at C4-C5 vertebral level. The foreign bodies could not be seen on hypopharyngoscopy due to severe mucosal oedema. Lateral pharyngotomy was done to remove the nails with C-arm guidance. Discussion Retained foreign bodies in the aerodigestive tract are known to produce complications. Delayed presentation makes the management difficult. C-arm guidance during surgical exploration is helpful to locate a radiopaue foreign body in difficult situations.

Abstract: Desmoplastic small round cell tumor (DSRCT) is rare and highly malignant neoplasm. DSRCT affects usually young males but can occur in adults also. Intra-abdominal pelvic region is the preferred site. Though confirmed by histology, immunohistochemistry (IHC) plays a key role in diagnosis. IHC profile is characteristic, it shows simultaneous expression of epithelial (epithelial membrane antigen (EMA) and cytokeratin (CK)), muscular (desmin), and neural (neuron-specific enolase) markers. Many cases of DSRCT are diagnosed as poorly differentiated carcinoma due to lack of proper panel of IHC. It is difficult to predict if there has been a true increase in incidence. Prognosis is uncertain in such an aggressive neoplasm as chemotherapy (CT) or radiotherapy (RT) shows various outcomes. Here in; we report four cases, all of which showed diagnostic dilemma and uncertain prognosis.
